SUMMARY OF OVERSIGHT ACTIVITIES

On February 24 through 27, 1992, PRC Environmental Management, Inc. (PRC),conducted oversight activities for remedial action (RA) performed by Warzyn, Inc. (Warzyn), atthe Hagen Farm site in Stoughton, Wisconsin. The RA consisted of installing extraction wells fora pilot study of source control operable unit I (SCOW I). The summary of oversight activities isincluded below. Field notes from the oversight activities are presented in Attachment 1.

Monday, February 24, 1992

Peggy Flaherty of PRC, arrived at the site at 8:10 a.m. Joe Larson, Warzyn geologist, andSteve Termont, Warzyn project manager were staking locations for extraction wells EW1 andEW1A and gas probes GP-1 through GP-8. The locations of extraction wells and gas wells areprovided in Attachment 2. Drillers from Environmental and Foundation Drilling were alsopresent. The landfill, referred to as Area A, was very muddy and wet. Mr. Termont did notwant to begin drilling because of possible damage to the clay cap.

Mr. Larson and Mr. Termont conducted a health and safety meeting in the on-site trailerand decided to postpone drilling operations. The drillers began decontaminating equipment andpreparing a plywood base for the placement of gravel, which was to be used during wellcompletion.

Tuesday, February 25, 1992

Peggy Flaherty of PRC arrived at the site at 8:05 a.m. The surface of the landfil l was notas muddy as it had been the previous day, but the top 2 to 3 inches were still soft. Warzynpersonnel and the drillers left the site to get a different type of gravel mix. The gravel alreadyon-site consisted of crushed limestone, which Mr. Termont thought might react with the acidicground water.

Drilling began at GP-4. Mr. Larson used an HNu to perform regular air monitoring nearthe borehole. No readings were detected. All cuttings from the drilling operation were placed ina 55-gallon drum. After drilling was completed to a depth of 30 feet below ground surface (bgs),the water level in the borehole was allowed to stabilize. Three probes placed in the boreholewere screened at depths of 25.5 to 28 feet bgs, 16.5 to 19.0 feet bgs, and 8.0 to 10.5 feet bgs.Logs of borehole completion materials and screened depths are provided in Attachment 3. Theprobes were made of 1-inch, scheduIe-,010, polyvinyl chloride pipe. After the probes wereinstalled, a metal casing was placed to enclose the top portion of the probes. A metal cap was

then placed on the casing and secured with a lock. One minor deviation from the work planinvolved using bentonite instead of concrete at the top of the bore hole. Warzyn decided to usebentonite because concrete has a tendency to heave and crack.

After completion of GP-4, drilling operations moved to GP-3. After drilling GP-3 to adepth of 31.5 feet bgs, work stopped for the day. The water level in GP-3 was allowed tostabilize overnight.

Wednesday, February 26, 1992

Peggy Flaherty of PRC arrived at the site at 7:30 a.m. Probes were placed in GP-3, and ametal casing and cap were placed over the probes. Drilling operations moved to GP-2. The HNudetected 15 parts per million (ppm) near the borehole at a depth of 30 feet bgs, but no readingswere detected in the breathing zone. A reading of 4 ppm was detected in the split spoon samplefrom 30 feet bgs. After the water level was allowed to stabilize, probes were placed in theborehole.

After completion of GP-2, the drill rig was moved to GP-1. No readings were recordedon the HNu at the borehole during drilling. Drilling was completed at 30 feet bgs. Work endedfor the day, and the water level was allowed to stabilize over night.

Thursday, February 27, 1992

Thomas Wiberg of PRC arrived at the site at 7:25 a.m. Probes were placed in theborehole, and a metal casing and cap were placed over the probes. After completion of GP-1,the drill rig was moved to GP-8. No readings were detected with the HNu while drilling orsampling. Drilling was completed to 36.5 feet bgs at GP-8. After the water level stabilized,probes were installed in the borehole, and a metal casing and cap were placed over the probes.

Due to wet and muddy conditions, drilling operations at the site were discontinued.Warzyn believed that further drilling could damage the landfill cap.

CONCLUSIONS

The RA activities were being conducted in accordance with the U.S. EPA approvedIn-Situ Vapor Extraction (ISVE) Pilot Study Work Plan.
