Please contact the Practice Manager in person or by phone, letter or email if you have a comment, suggestion or complaint. You are responsible for Giving at least 48 hours notice if you have to cancel or change an appointment. We will not charge you for missed NHS appointments – but if you miss more than 2 appointments we may no longer be able to offer you treatment. Private patients may be charged Following your dentist’s advice to prevent tooth decay and gum disease Paying your bill promptly Bringing proof of entitlement when claiming help with the cost of NHS treatment. If no proof is shown your eligibility will be checked and you may have to pay for treatment Treating our staff with courtesy and respect We may refuse to treat patients who are violent, fail to pay their bills or refuse to cooperate during treatment. In this case, we will inform appropriate authorities and NHS England. We operate on a zero tolerance policy. Your dental records Your dental records will remain confidential and secure. However, where records detail NHS care, from time to time we may need to release these to NHS England or the NHS Business Services Authority as part of our ongoing clinical review process. We will try to contact you before releasing your dental records to another organisation. URGENT TREATMENT AND OUT OF HOURS CARE

Urgent treatment means treatment that the dentist considers necessary to relieve severe pain or prevent your oral condition deteriorating before you can make a normal appointment. For NHS patients such treatment should be provided within 48 hours.

Should you have an urgent problem we need to know as soon as possible.

If you need urgent treatment outside of our opening hours you can access advice and/or treatment by telephone by dialing 111. This service is not provided by the practice but commissioned by NHS England. Alternatively call 111.

ACCESSIBILITY

Our practice is accessible for disabled patients. Please contact reception prior to your appointment to notify us of any additional special requirements.

NHS SERVICES AVAILABLE

We have agreed with NHS England to provide NHS services to adultsand children. NHS dental treatment includes all treatment necessary to secure and maintain your oral health. We also provide a full range of private treatment options for patients who choose to have treatmentthat is not available under the NHS – for example large white llings in back teeth or cosmetic treatment such as tooth whitening.

We are happy to discuss these options with you.

APPOINTMENTS

Making an appointmentIf you ask us for an appointment, we will try to oer you one as soon aspossible at a convenient time. You can make an appointment by phoning 0208 843 9323. See below for information on our opening hours.

You can choose which dentist you would like to see at the practice, butplease note that this will depend on the appointments available and theworking hours of each dentist.

Reminders and recallsAt the end of your course of treatment, your dentist will discuss with you when you will need to see a dentist again. NHS dentists follow guidelines issued by the National Institute for Health and Clinical Excellence (NICE). This means you should attend as often as is neededto keep your teeth and gums healthy and that you may no longer needa check-up every six months.

CancellationsIf you are unable to keep your appointment please let us know as soon as possible so that we can give the appointment to someone else. Missed appointments waste NHS time. If you miss more than 2 appointments without notice, we may not be able to complete your treatment or oer you NHS care in the future. Private patients may be charged for missed appointments.
