The following documents should be submitted to the Admission Office of Graduate School of Medicine. ＊Please fill out in Japanese or English and send the original forms provided in this pamphlet (1) Application Form (1 copy): Prescribed form before applicants decide their Specialized Area for Research. Approval by the professor or associate professor of the field must be required. (2) Admission Ticket for the exam・Photograph Card: Prescribed form (3) Photograph (2 copies) :Attach each photograph (4cm×3cm, upper body, bareheaded, front face, taken within the last 3 months) to the Application form(1) and the Photograph Card(2) (4) Purpose of Application (1 copy): Prescribed form (5) Certificate of Graduation / Expected Graduation (1 copy): Graduates from Nagoya University are not requested. (6) Transcript of Scholastic Record (1 copy): Officially sealed by the graduate university or college attend. (7) Envelope for Reply (3 pieces): a. One to send the Admission Ticket for the Examination and One to send pass-fail notification: Prescribed envelope (small size) with clearly self-addressed, signed and stamped equivalent with 350 yen (as an express mail charge). b. One for notification of entrance procedure: Prescribed envelope (33cm×24cm) with clearly self-addressed, signed, stamped equivalent with 470 yen (as an express mail charge). (8) Certificated Letter of Alien Registration (1 copy): This is for those applicants currently residing in Japan; available at your local ward office. If you are unable to obtain Alien Registration Card, please contact the Admission Office of Graduate School of Medicine. (9) Application Fee: 30,000 yen is required (non-refundable). In Japan, please use a Japanese Postal Money Order with no entry in section “To be paid to” or “受取人指定欄”. Applicants with Japanese Government Scholarships are requested to submit the certificates of receiving scholarships instead of application fee. (10) Receipt (for applicant), Receipt (for office use only), Original label (for office use only) (1 copy): submit with making an enter of All the required items.
