Course Format:  The entire course will be delivered in an on-line format.

The course will use the following educational methods: lecture via PowerPoint (frominstructor and guest lecturers) with lecture notes, narration, or in some cases, both;individual exercises; computer-aided assignments; a required interactiveassignment via Blackboard; and a comprehensive final exam.

Course materials and assignments will be posted in the Blackboard course website.Topics will be divided into weekly units, designated by folders, in Blackboard.Lecture notes and required readings for each weekly unit will be released 2-4weeks ahead of the time as feasible.

This course will use the “Class Discussion” feature with 3 forums: one forcomments and questions related to weekly units, one for posting the requiredassignment during week 12, and one called the “Student Lounge”. The StudentLounge can be used for students to communicate with each other during thecourse. The Student Lounge has the capability for students to post comments orquestions either by name or anonymously, and will NOT be monitored by ETA orthe instructor. Please do NOT use the Student Lounge to ask other studentsspecific questions about how to complete assignments, activities, etc. (Post content questions like that in the weekly unit forum of the Class Discussion feature or e-mail Dr. Haiduven directly. Upon completion of this course, the student will be able to: 

1. Outline the historical development of public health surveillance and disease reporting.

2. List and describe the role of surveillance in public health; uses of surveillance; essential elements of asurveillance system; types of and sources of surveillance, citing disease examples for each.

3. Differentiate between the functional, intermediate, and central levels of surveillance citing severaltasks for each level.

4. Define and describe the 4 categories of healthcare-associated infection (HAI) surveillance and threeadditional types of surveillance conducted in healthcare settings.

5. List and describe the steps to take in investigating a disease outbreak in both the hospital andcommunity settings. Participate in an outbreak investigation exercise to apply these principles.

6. Summarize, interpret and formulate recommendations for communicable disease surveillance inpublic health practice based on an assessment of existing surveillance data.

7. Analyze the U.S. system for biosurveillance.

8. Synthesize and evaluate HIV/AIDS surveillance information from a variety of sources includingInternet sites, published literature, institutional reports, unpublished studies and expert opinion.

9. Define infectious and non-infectious hazards present in healthcare facilities for which surveillancemay be indicated.

10. List and describe at least 3 forms of international surveillance.

11. Summarize important concepts for the following types of surveillance: healthcare facility; situational

awareness, occupational health, HIV/AIDS, bloodborne pathogens & needlestick injuries inhealthcare workers; vaccine-preventable disease; international; foodborne outbreak; and surveillanceduring construction & renovation of healthcare facilities.

Assessment Strategies:(Strategies must be numbered)

1. ASSIGNMENTS: There are 7 assignments throughout the course. Each assignment is worth 20 total

points and 5% of the course grade. Together the assignments are worth 35% (140 points) of the coursegrade. The format of these assignments varies and complete instructions are provided on the coursewebsite for each assignment. Each assignment is due by 9 PM on the Saturday night of the week it iscovered. See “Course Schedule” section in the syllabus for individual due dates. Any assignmentsubmitted past the due date and time will have 5 points deducted from the grade for each hour that it islate. You are able to use specified resource materials for these assignments, just not consultation with any other persons.

2. OUTBREAK EXERCISE: Students will be assigned an outbreak investigation exercise that can becompleted on-line. This assignment is worth 10% of the course grade (100 points) and is due by 9 PMSaturday, September 24. Students will receive their assigned outbreak scenario no later than the third week of class.

3. EXAMINATIONS: The mid-term examination will be administered on-line from 8 AM Thursday,October 13 until Friday, October 14 at 10 PM. The mid-term exam will be open-book and consist ofmultiple choice, and matching questions. Students will be given 2 hours to complete this exam. The finalexamination will be administered on-line from 8 AM, Sunday December 4 until Tuesday December 6,at 10 PM. The final examination is cumulative and will be open-book consisting of a multiple-choice,matching and essay question format. Students will be given 4 hours and 55 minutes to complete thisexam. There will be reviews for the mid-term and final exams posted no later than two weeks prior to each exam. 

4. OCCUPATIONAL HEALTH HAZARDS EXERCISE: Students will be given an exercise to familiarizethem with occupational health hazards, focusing on biological exposures, in a healthcare setting. Thisassignment is worth 10% (50 points) and is due by 9 PM SATURDAY, November 5, 2010.

5. EXTRA CREDIT: There are 2 ways to receive extra credit points in the course.1. Extra credit activities: There are 5 opportunities to receive extra credit points added to the

Assignment points (that make up 35% of the course grade). The first opportunity is during week 4,the second during week 10 and the third, during week 14. For each of these 3 opportunities,submission of the completed extra credit activity by the due date and time will result in up to 2 extracredit points. In addition, there is a 1-point opportunity to receive extra credit on Assignment #6, PartII. The fifth opportunity is that Assignment #3 has a 2-point extra credit question on it.

2. Exams: There will be extra credit questions on the exams. It is possible to receive full, partial, or nopoints, depending upon your answers.

Extra credit points possible: 6 on extra-credit activities added to specific assignments, 2 onAssignment #3, 1 on Assignment #6, Part II, 2 on midterm exam, and 5 on the final exam). Total

possible extra credit points: 16

Note: specific instructions for completing assignments or preparing for exams will be provided on thecourse website.

FEEDBACK ON EXAMS AND ASSIGNMENTS: Students should expect to receive feedback on theassignments, exercises, extra credit activities, and exams no later than 2 weeks after the due date(unless the instructor is out of town). For the final exam, students will receive feedback before finalgrades are posted. Feedback cannot be made available until: 1) all students have completed theassignment or exam, 2) a detailed of analysis of the results has been conducted, and 3) a two-day

Grading Scale andCriteria:

Straight letter grades are given as follows:

Grading Scale: A= (90-100), B= (80-89.9), C= (70-79.9); D= (60-69.9);

The grade will be determined as follows:Midterm (20%) and Final (25%) examinations; On-line OutbreakExercise (10%); On-line Hospital Health Hazards Exercise (10%) andAssignments (35%).

Points breakdown is as follows:Midterm exam (100 points)Outbreak exercise (100 points)Occupational health hazards exercise (50) pointsAssignments #1-7= 20 points each (140 total points)Final exam (100 points)The total possible points for the course = 490 

Grading Policies:  Exercises: In order to receive full credit (100 points for Outbreak Exerciseand 50 points for Health Hazard Exercise) for the 2 exercises, they mustbe turned in on time and all sections completed. Detailed instructions forcompletion of these exercises will be provided on Blackboard. For eachhour that it is late, 5 points will be deducted from the exercise grade. If astudent has an extenuating circumstance resulting in the exercise beinglate, this needs to be communicated to the instructor BEFORE the duedate.

Assignment Due Dates: Assignments are expected to be turned in by thedue date and time. For each hour that it is late, 5 points will be deductedfrom the assignment grade. If a student has an extenuating circumstanceresulting in the assignment being late, this needs to be communicated tothe instructor BEFORE the due date. The course materials are posted upto 4 weeks in advance; students should plan accordingly if they know theywill be travelling or have other conflicts.

Exams: Students will be allowed 2 hours to take the mid-term exam and4 hours 55 minutes to take the final exam. EXCEPTION: See “SpecialAccommodations”. Exams turned in late will have 5 points deductedfrom the exam grade for each 15-minute period that it is turned inlate. Students should NOT communicate to each other during the examsas they are intended to be an individual component of the course grade.Communication with other students during the exams could result in areceiving an “F” for the exam.

Students will have the opportunity to review their exam results onceeveryone has taken the exam. There will be at least a 24-hour periodavailable for reviewing the exam. Instructions and more information will beposted on the announcement page of the course website. For both themid-term and final exams, there will be extra credit questions. Partial credit

COURSE POLICIES

Online Attendance &Participation:

Students are instructed to complete the Practice Test in Lesson Zero andrespond to a test e-mail during the first week of class to verify first classattendance. Students need to complete the 7 assignments by due datesand times, as well as turn in the exercises, midterm and final exams by thedesignated times.

Teaching philosophy: The teaching philosophy of this instructor is based on the principles ofadult learning, active learning, and learning by doing. Thus, applicationand integration of learned principles in practical situations will be the focusof many of the class exercises/discussions as well as the final exam.

Methods and format for feedback to students: Methods of grading and format for students are described in the coursesyllabus under “Types of Assessments”. To achieve maximum points forany assignment or exercises, the student needs to follow the guidelines.

Expectations for the class: 1. Students are expected to keep up with the class, to read the requiredreadings, and to submit assignments and activities by due dates andtimes. As materials are made available at least 2 weeks ahead of whenthey are covered, students should plan accordingly to assure that allrequired components are submitted on time.

2. Students should log on to Blackboard and check e-mail at least everyother day (besides the time to review lectures and complete activities), tocheck for any announcements, updates, posting of feedback, and/orpotential issues related to assignment/activity submissions.

3. Students are expected to independently complete all activities,exercises, assignments, midterm and final exams.

4. Students are expected to read the Required Readings every week,before reviewing the lectures, and are encouraged to read theSupplemental Readings and to complete the activities.

5. There will be one assignment requiring an interactive activity throughthe “Class Discussions” feature during week 12 of the course.

6. To receive maximum points for any assignment, activity, exercises, orexam questions, students need to follow the instructions carefully, followword limits as instructed and use Spell Check. There will be deductions ifthese guidelines are not followed.

7. To be successful in this course, students will need to complete allrequired assignments, exercises, and the final exam. Students will have abetter chance of passing the Certification Exam in Infection Control if theytake a comprehensive approach to all courses in the Infection ControlCertificate Program by reading the Supplemental Readings andcompelting the extra credit activities in this course.

Expectations for ALL assessments in this course: Exercises, assignments, extra credit activities, and exams are expected tobe products of individual students. Students should not discuss any of theexam or activity questions with each other before or during the actualexercises, assignments, or exams.

The University of South Florida has an account with an automated plagiarismdetection service (SafeAssign ), which allows instructors and students to submitstudent assignments to be checked for plagiarism. I (the instructor) reserve theright to 1) request that assignments be submitted as electronic files and 2) submitstudents’ assignments to SafeAssign , or 3) request students to submit their

assignments to SafeAssign through myUSF. Assignments are comparedautomatically with a database of journal articles, web articles, the internet andpreviously submitted papers. The instructor receives a report showing exactly howa student’s paper was plagiarized.

NOTE: An institution may not release a paper to a plagiarism detection softwarewithout the student’s prior consent unless all personally identifiable information hasbeen removed, such as a student’s name, social security number, student number,etc..
