Full Wizard (Green) Forms Available to Users • Once testing at HQDA is complete we will place the wizardized versions of revised military evaluation forms on AKO “My Forms.”• Forms include digital signature, help keys, and embedded regulatory logic.• Allows creation of final evaluations from support forms.

Electronic Submission to HQDA • Once testing at HQDA is complete we will initiate the automatic “send to HQDA” function on AKO “My Forms.” • Until this time, users can send reports to HQDA in the manner currently available to them (mail, upload, OER by Email, etc).

• Last day to send old versions of forms to HQDA. Users must submit revised forms after this date.• Users prepare and submit delinquent or missing reports from previous rating periods before this date. Hereafter, all OER/NCOER must be on the revised form.

- PLS read the MILPER MSG when sent –OER Closeout dates, where to send documents to add for expedited processing, and where to send letters to the board

- Have a DA photo taken close to board date for boards that are important to you – the new norm

- Check a box when you visit “My board file” – the selection board members may interpret ‘not reviewed’ to mean ‘does not care’

- File Clutter (Certificates of Appreciation for Bake Sale after Bronze Star, etc.)
