a. Pawtucket River Bridge No. 550 Replacement Contract (RI Contract No. 2012-CB-004) - This Contract will install a new highway information sign on 95 Southbound on or about the location of the existing “Welcome to Rhode Island” sign north of the Middle Street On-Ramp that is being removed by others.

b. Welcome to Rhode Island Gateways Signing Contract (RI Contract No. 2012-ET-008) - This Contract will install a Welcome to Rhode Island Gateway Sign on 95 Southbound at Exchange Street in Pawtucket. The anticipated completion for the Gateways Signing Contract is Fall 2013.

c. Statewide Tree Trimming Contract (RI Contract No. 2011-CL-012) - This Contract will perform tree trimming and cutting and disposing isolated trees and stumps as called for on the Plans within the limits of the I-95 Sustainability - Phase I Contract.

d. Anti-Graffiti Coatings & Graffiti Abatement Contract (RI Contract No. 2013-CE-013) – This Contract will paint a mural and apply anti-graffiti coatings to the existing concrete retaining wall on I-95 Northbound at Cottage Street north of the Exchange Street underpass in Pawtucket.

6. SPECIFICATIONS TO GOVERN:

Specifications to govern this project are the Rhode Island Department of Transportation Standard Specifications for Road and Bridge Construction, 2004 Edition, all Compilations of Approved Specifications, and the Special Provisions – Contract Specific included in the Contract Documents. Standard Details for this project are the Rhode Island Standard Details, 1998 Edition, with all revisions. All traffic control devices and signage are to be in accordance with the U.S. Department of Transportation Federal Highway Administration Manual on Uniform Traffic Control Devices (MUTCD), 2009 Edition, with all revisions.

7. STORAGE AND STOCKPILING OF MATERIAL:

Storing and/or stockpiling material shall only be allowed within the designated Contractor staging area as shown on the Plans or at an off-site location approved by the Engineer. The Contractor shall remove material from construction activities off -site or to within the designated staging area within 24 hours.

9. SEQUENCE OF CONSTRUCTION:

Action Required By Contractor

A. A.The Contractor must submit for Department acceptance a Project Schedule as required by Subsection 12.108.03 Prosecution and Progress; Schedule Level B of the State of Rhode Island Procurement Regulations, with all amendments. The proposed construction and time schedule must consider and address safe vehicle passage through the project for any temporary shoulder or lane closures. Approval of the work sequence and time schedule is required before the start of any work associated with this contract.

B. All work shall be completed in accordance with the Traffic Related Work Restrictions indicated in the Transportation Management Plan (TMP).

C. The Contractor shall notify the Department in writing at least fourteen (14) days in advance of any intended lane restrictions and/or closures so that adequate public notice can be given. Upon the Engineer’s approval, the Contractor shall coordinate the required traffic control.

D. The Contractor shall coordinate requirements for Uniformed Traffic Control Persons with the Engineer who will coordinate with the Rhode Island State Police as outlined in Paragraph 14, Police Compensation.

E. The Contractor shall coordinate, cooperate and schedule his work and all segments thereof with the Engineer, other contractors, utility owners, and applicable local authorities, so as to minimize impacts to the construction schedule.

F. In cases of emergency and/or as directed by the Engineer, the Contractor shall move equipment to allow for the passage of emergency vehicles and/or open closed lanes to maintain traffic flow.

G. All erosion controls shall be in-place and accepted by the Engineer prior to commencing work.

H. Grass Mowing

The Contractor will be required to mow the grass within the project limits as shown on the mowing plans contained in the Specifications-Job Specific under Item Code L02.9901 (approximately seven and five-tenths [7.5 +/-] acres). The mowing of grassed areas (approximately six and nine-tenths [6.9+/-] acres) shall be completed a total of six (6) times each year for the duration of the Contract including the extended establishment period. The Wild Flower Area (approximately six-tenths [0.6+/-] acres) planted under this Contract (I-95 SB Off-Ramp to Middle Street and to Fountain Street) will only be mowed one (1) time each year for the duration of the Contract including the extended establishment period.

10. NOTICE TO CONTRACTOR:

A. Landscape Maintenance and Planting Requirements The Contractor is responsible to perform all landscape maintenance activities in accordance with RIDOT Standard Specifications for Road and Bridge Construction, 2004 version and all compilation of approved specifications, and with these Job Specific Specifications to ensure continued health of the plant material. If any damage does occur which is due to the Contractor’s negligence, it will be immediately repaired at the Contractor’s own expense. Two (2) weeks prior to commencing the work, the Contractor shall submit a schedule for the Engineer’s approval of all landscape maintenance and mowing activities. Contractor shall commence landscape maintenance and mowing activities and carry out the work consecutively without suspension of the work until the maintenance and mowing activities have been completed for the entire project limits.

The Contractor is required to have a qualified foreperson who is experienced in landscape maintenance activities on site at all times during extended establishment period work operations. The foreperson or whoever is directly responsible for spraying and pruning must possess a current R.I. Commercial Applicators License and a current R.I. Arborist License. A copy of these licenses must be given to the Engineer prior to the start of any work beginning on the project and at anytime a personnel change occurs on the Contract. Exclusive of the grass mowing, no work should be performed on the project without either the Engineer and/or a Landscape Representative present. Prior to any plant installations, Contractor will be responsible to stake out locations of proposed plant materials and bedlines for approval by both the Engineer and a RIDOT Landscape Representative. material selection and installation will be in strict conformance with RIDOT Standard Specifications for Road and Bridge Construction, 2004 version, Section L.06 and all revisions. There will be no planting between June 30, 2013 and August 15, 2013 unless otherwise directed by the Engineer.

B. Requests for Information (RFI)

All Requests for Information (RFI) shall be prepared by the Contractor with a unique and sequential number using the numbering convention “RFI ###”. This number shall be cross-referenced in all subsequent correspondence by the Contractor and Engineer until such time as the RFI has been satisfactorily closed. The Contractor shall maintain a RFI Status Log. The RFI Status Log shall be updated bi-weekly and submitted to the Engineer five (5) working days prior to the Bi-weekly Schedule Update meeting. The Contractor shall come prepared to the bi-weekly meeting with a current RFI Status Log.

C. Guidelines for Work around Gas Pipes The Contractor shall note if gas line becomes exposed during construction, Code requires that National Grid be notified to inspect the line before resume working. Also, any damage to the pipe or pipe coating will need to be repaired before resume working. Contact Providence Dispatch office at (401) 525-5513.

12. SPECIAL REQUIREMENT FOR TRAFFIC MAINTENANCE AND PROTECTION: In addition to the requirements of the Standard Specifications with all revisions, and the special requirements of other sections of this contract document, the contractor must adhere to the following requirements:

The Contractor is advised that the signs and other traffic control devices shown on the Traffic Control Plans are minimum requirements. It is the Contractor’s responsibility to supplement the plans and specifications as necessary to ensure the public’s safety. All maintenance and protection of traffic devices shall be in place and approved by the Engineer prior to starting construction at a particular location. All maintenance and protection of traffic shall conform to the latest revisions to Part VI of the Manual on Uniform Traffic Control Devices (MUTCD).

All work shall be performed in a manner to cause the least disruption to existing vehicular and pedestrian traffic for as short a period of time as possible. When work commences in such areas, it shall be expeditiously completed without unnecessary interruptions. All temporary construction signs shall be removed, covered or otherwise concealed when there are no construction operations taking place or when they are not needed to properly warn drivers. This includes the period between erecting the temporary construction signs and the start of construction, as well as when a construction phase is completed or suspended. The Contractor shall be compensated for this under Code 937.0200 “Maintenance and Movement Traffic Protection” during the Phase 1 and Phase 2 planting work. Maintenance and Movement Traffic Protection shall be incidental to the Grass Mowing and Extended Establishment work.

13. TRANSPORTATION MANAGEMENT PLAN:

Included as Appendix A to these Contract Specific General Provisions is the Transportation Management Plan (TMP) for this project. The TMP lays out the set of coordinated transportation management strategies that will be used to manage the work zone safety and mobility impacts of this project. In the event of a discrepancy between information in the TMP and information elsewhere in the Contract Documents, the former shall govern. The Department's latest Training Guidelines for Personnel Responsible for Work Zone Safety & Mobility are available under the "Training" section at http://www.dot.ri.gov/humanresources/index.asp.

The Contractor’s attention is called to the State of Rhode Island Procurement Regulations, with all amendments, for Subsection 12.103.02(a), which describes the requirements for the Contractor’s designation of a TMP Implementation Manager for the Contract.

DESCRIPTION: Subsection 937.05.2; Failure to Comply, part a. Maintenance, of the Standard Specifications requires that a daily charge be deducted from monies due the Contractor for failure to adequately and safely maintain traffic control devices along any portion of the project. The charge for this Contract will be $2,000.00 per day.

Subsection 937.05.2; Failure to Comply, part b. Movement, of the Standard Specifications, requires that a charge be deducted from monies due the Contractor for failure to remove and/or relocate traffic control devices for compliance with the traffic- related work restrictions included in the Transportation Management Plan or to otherwise meet changes in traffic conditions, construction operations, or other conditions affecting the safety and/or mobility of the traveling public. Description: This work will include the mowing a total of approximately seven and five-tenths (7.5 +/- ) acres of grass and wildflower area within the project limits throughout the project duration including the extended establishment period. Grassed areas (approximately six and nine-tenths [6.9 +/-] acres) within the project limits will have six (6) complete mowings each year. The approximately six-tenths (0.6 +/-) acre wild flower area installed adjacent to the I-95SB off-ramp to Middle Street as shown on Landscape Plans Nos. 9 and 10 and I-95SB off-ramp to Fountain Street as shown on Landscape Plan No. 5 under this contract will have only one (1) mowing each year. Materials: This work will involve equipment such as tractor/mowers, walk-behind mowers, and hand trimmers. Construction Methods: The Contractor will be required to mow the grassed areas (approximately six and nine-tenths [6.9 +/-] acres) within the contract, six (6) complete mowings each year. The areas to be mowed are shown on the Mowing Plans. The acreage calculated for mowing excludes all areas to be landscaped and mulched as a bed. It is anticipated the yearly mowings will begin after April 15th and will be: (1) spring; (3) summer; and (2) fall. The Engineer will order the exact time for each mowing. The Contractor will be responsible for any hand trimming at planting bed edges, signs, guardrail, fences, etc. All litter will be picked up at the time of each mowing. The Contractor will be responsible for disposing of all litter in a legal manner off of the job site. “Each Mowing” will be done on consecutive days. The result of “Each Mowing” will be a neat uniform appearance of grass, which is approximately 2 -3 inches tall throughout the entire project limits. The Contractor will also supply all necessary mobilization, traffic protection, maintenance and movement of traffic protection devices, and cleaning and sweeping pavement as part of this item. Method of Measurement: “Grass Mowing” will be measured by the unit “EACH” for the complete mowing of all grassed areas. Basis of Payment: “Grass Mowing” will be paid for by the contract unit price for “EACH” complete mowing. This price will include the six and nine-tenths (6.9+/-) acres of grass mowing; the six-tenths (0.6+/-) acre wild flower mowing, litter pickup and disposal; traffic protection; temporary construction signs; fluourescent traffic cones; maintenance and movement of traffic protection devices; cleaning and sweeping pavement; mobilization; and for all labor, tools, materials, equipment, and incidentals necessary to complete the work to the satisfaction of the Engineer.
