YMCA’S BLOOD PRESSURE SELF-MONITORING PROGRAM

• Evidence-based program• Focused on key behaviors and education

to support lifestyle modification• Blood press self-monitoring• Nutrition education• Personalized support from trained

PROGRAM DETAILS• 4 month program• BP Cuff provided• Two personalized appointments per month

(10-15 minutes), and weekly check-ins via phone, email, or text.

• Monthly nutrition seminars• Overview of the DASH Approach• Reducing sodium intake• Tips for shopping, prepping and

cooking food for better BP management

• Heart Healthy Eating for Life

PARTICIPANT CRITERIA• Inclusion criteria

• Age >18 years of age• Diagnosis of hypertension and/or on

anti-hypertensive medication• Two or more elevated BP reading

within the last 12 months• Exclusion Criteria

• Recent cardiac event (last 12 months)

• Atrial fibrillation or other arrhythmia• At risk for lymphedema

MOVING FOR BETTER BALANCE

• Beginner level class• Based on the principles and movements of

Yang-style Tai chi• Ideal for those at risk, or a history, of falling• Goal – Feel strong, steady and safe

PROGRAM DETAILS• 3 month program progressive curriculum• Participants will

• Complete a brief pre- and post-program fitness assessment

• Learn eight forms and a variety of warm up exercises

• Learn modifications as necessary• Improve balance, muscle strength,

flexibility, and mobility• No equipment needed

• Evidence-based program for cancer survivors• Ideal for those finished with treatment• Meet participants where they are physically• Goal – help participants find a new normal

• 3-month program• Classes of up to 12 people meet 90

minutes twice per week• Weight training and cardiovascular

• Balance, flexibility, etc• Specialty classes

• Pre- and post- fitness assessment and a Promise 29 questionnaire
