(Supportive Programs Reengaging Individuals and Nurturing Growth), our initiative that guides every

decision we make in terms of holistic community wellness. By engaging the different aspects of the mind, body, and spirit we promote truer long-term wellness. When you participate in S.P.R.I.N.G. you’ll quickly see an increase in strength of mind and quality of life.

COMMUNITY? A Life Plan Community provides a Continuum of Care to support a variety of lifestyles and changing wellness needs. Within our campus you will find accommodations for Independent Living, Assisted Living, and advanced health services. This allows you to be well in the same community, no matter your needs.

Homemaker, and a Dietitian, our Members can guide their own wellness journey in a setting that is personalized for their unique needs.

At Park Springs, we understand the importance of offering essential support and occasional breaks for home-based Care Partners, spouses, and adult children. Our Day Program is designed to do just that. This weekday care model focuses on individuals with mild to moderate cognitive impairment and relatively stable physical conditioning. Participants are provided opportunities to alleviate social isolation, foster feelings of belonging, and enjoy engagement in activities of his/her choosing. Our goal is to promote independence and to encourage participation in meaningful ongoing engagement.

Pebblebrook at Park Springs provides feelings-focused care that changes lives for the better. Surrounded by the comfort of home created by compassionate Care Partners and Homemakers, Member needs and preferences serve as the guide for their unique wellness journey. • Memory Care is thoughtfully

designed for people with Alzheimer’s or dementia.

• Skilled Nursing is Medicare-Certified and offers both short-term and long-term care overseen by a full-time Geriatrician. Staff members are available 24 hours a day to provide appropriate levels of assistance and support for Members in therapeutic and recreational activities.

• Our Day Program allows an opportunity for caretakers to get time for the things they need to do while their loved one is provided supervised activity and socialization with others experiencing similar stages of dementia.

• Pebblebrook also offers personalized therapy Rehabilitation and Outpatient Therapy programs.

Memory Care at Park Springs is designed to meet and understand the needs of those under our care. In our community, emotionally intelligent Care Partners engage Members on a personal level forming trust and genuine bonds based on their unique life stories. This bond allows all parties to thrive in an atmosphere of true care. Every facet of our program has been developed specifically to improve the quality of life

for our Members. Everything from the colorful décor to our home-inspired design provides stimulation and connection for those living with memory loss.

Assisted Living has all the perks of the Independent Living lifestyle, while offering appropriate levels of support as needed. With a focus on wellness and preventative care, our compassionate Care Partners are available anytime to meet

our Members’ needs. We value the commitment of home-based caregivers, spouses, and adult children and understand the importance of offering them occasional relief to unwind and practice self-care with Respite Care.

Cobblestone at Park Springs offers personalized Assisted Living households for general assistance needs, mild memory care, or more challenging physical needs. Through the Household Model of Care an environment of warmth and family is created between the community of Members and Care Partners. Our memory care program uses research-based methods that are shown to improve and maintain cognitive ability. Members flourish through the prolonging of independence that comes with our care programs. We offer three thoughtfully created households along with a respite program that are designed to meet the individualized needs of our members: • Club Household
