OBJECTIVE

1. Reception:- This is the First module of the System that can be used in Reception. This module is divided in some sub-module (Different forms). The modules are –

Registration

Appointment Scheduling

Room/Ward Availability

New Room Creation

Administrator

Login

Medicine Master

A) Registration: This form is used for Registering Patient in a general manner. It consist of two type- 1.Inpatient 2.Outpatient.

In both the forms we have to register the details about patient like: name, age, sex, D.O.B, city, state, blood group and symptoms. In OP registration we have to enter some extra details about doctor name, doctor ID, department. For both the registration, unique ID will be generated.

B) Appointment Scheduling:- Appointment scheduling is the main module for fixing the appointment date & time for outpatients. User have to mention the details such as patient ID, name, phone no., doctor department, doctor name, doctor ID. In this form user can search the patient details by three ways-1. Search by appointment no. 2. Search by doctor name 3. Search by appointment date.

C) Room Creation:- In room creation ,user can create a new room no., bed no., ward name and rent per day for the room. We can view all the details by corresponding room no.

D) Room/Ward Availability:- It contains information about the room/ward whether it is occupied or vacant. User can view the ward status by datagridview.

D) Administrator:-This module is divided in some sub-module (Different forms). The modules are-

A) Login:- In this form user have login to use the software. Only authorized person can use that software. User have to enter the user name & password.

B) Doctor Master:- In this form administrator can enter new doctor name along with the specialization. The corresponding doctor ID will be automatically generated.

C) Medicine Master:- In this form administrator can add new medicine along with company name, type, manufacturing date, expire date. The medicine no will be automatically generated. User can view details by selecting medicine no.

2. IP:- This Part consist of Interfaces which will help to manage the Inpatients of the Hospital. The sub-modules of this part can be described as follows –

Discharge Summary

A) Inpatient Admission: This form contains the details about inpatient such as admission date, admission time, relative details, deposit paid, ward name, room no, bed no, department, referred by and consultant name. In this form user can view ward status and according to the status user can admit the patient to the appropriate ward.

B) IP Case Sheet: It consist of all information about patient. This form includes details like BP, temperature, weight, diagnosis, complaints, surgery and others.

C) IP Previous History: This form should be used to view the previous details of the Patient. In this user can view the Details about the Patient’s like IP Number, Patient ID, Age, Sex, Phone No., Date of Admission, Time of Admission, Ward, Room No., Bed No., Referred By, Consultant In charge, Surgery, Date of Surgery, Time of Surgery, Attending Consultant, Habits, BPsist., BPdist., Weight, Temperature, BMI, Heart Bit Rate, Blood Sugar, Allergies, Illness, Diagnosis, Medication, Course, Investigation, Treatment Given, Condition on Discharge, Advice, Date of Discharge, Time of Discharge by selecting the patient ID .

D) Discharge Summary: This form shows about when the patient gets discharged. During discharge what is the condition, advice are given to the patient, brief history, course in hospital, investigation given of the patient are saved in this form.

3. OP:- This Part consist of Interfaces which will help to manage the Outpatients of the Hospital. The sub-modules of this part can be described as follows –

Medical Observation

Doctor Prescription

A) Medical Observation: This form shows the information about whether the patient is medically observed or not. This form consist of information about patients medical condition like, BP, BMI, blood sugar, temperature etc.

B) Doctor Prescription: This form consist of previous report, current diagnosis, medicine name, type of food, duration etc. Here we can include all tests and select the test which was prescribed by doctor.

3. Laboratory :-This module is divided in some sub-module (Different forms). The modules are-

Lab Test

A) Lab Master:- This form is the master form for all tests. It consist of test category, under category corresponding test type, units, normal values and cost. Here user can add manually all the test types and their corresponding details under category and user can remove unnecessary tests.

B) Lab Test:-This form is common for both IP & OP. It consist of test category, test type, value, normal value, unit . Test category, test type, normal value and unit will be fetched from lab master.

4. Cash Counter :- This Part consist of Interfaces which will help to increase the economical growth of a Hospital. The sub-modules of this part can be described as follows–

A) IP Fees Details :This form is consist of all type of IP charges such as consultancy fee, admission fee, ambulance fee, suturing fee, dressing fee, nursing fee etc. User can enter some other fees if it is necessary. At last user can subtract all charges with advance and can get the payable amount.

B) OP Fees Details :This form will shows all OP patient fees details. User can enter consultation fee and other fees (if any).

B) ECG Charges :This form will show only ECG charges. User can get both IP & OP patient details by selecting type IP or OP.

B) X-Ray Fees : This form will show only X-Ray charges. User can get both IP & OP patient details by selecting type IP or OP. User can enter all X-Ray type and their corresponding fees. They can add new X-Ray type.

B) Lab Charges :This form will show only lab charges. It is common for both IP or OP. User can enter all lab test type and their corresponding fees. They can add new lab test type.

B) Advance Payment Registration: This form is the master form for all advance payment. Here user can add advance payment for corresponding IP number. In this form user can see the advance fee along with the date.
