SECTION I - INTRODUCTION & REQUEST FOR SUBCONTRACTOR PREQUALIFICATION
Prequalification documents are sought from potential subcontractors for the following project.
Muir Biology Building 3rd Floor Renovation Project No. 4708/A4L-373/962290 University of California, San Diego La Jolla, California – 92093-0916
1.0 REQUEST FOR PRE-QUALIFICATION FROM PROSPECTIVE SUBCONTRACTOR BIDDERS:
DPR Construction has been awarded the competitively bid contract as Construction Manager- General Contractor for the UCSD Muir Biology Building 3rd Floor Renovation. The Subcontractors who desire to submit bids on this project must be prequalified. Prequalified Bidders will be required to have the appropriate current licenses issued by the State of California Contractor’s License Board for the work the subcontractor would perform.
The prequalification of prospective subcontractors will be determined by the scoring of the Prequalification Questionnaire utilizing a combination of pre-established point rating system questions, and pass/fail questions. The Prequalification Questionnaire includes, but is not limited to the following topics:
Prequalification Declaration California Contractor’s License/ Classification & Business Operations Construction Experience Proposed Staff Experience Contracts Liquidated Damages Safety Record Surety Insurance Claims History Financial Statement
The evaluation of the submitted information is solely for the purpose of determining, in a timely manner, potential subcontract bidders who are deemed qualified for the successful performance of the type of work included in this project.
2.0 GENERAL DESCRIPTION OF THE PROJECT:
Interior renovation of the existing Biology Lab at the third floor of the Muir Biology Building. The project is approximately 8,000 GSF of the overall 14,000 GSF floor plate and its proposed scope includes complete renovation of lab spaces to provide an open lab configuration. Specific upgrades include but not limited to: fire/smoke detection systems, fire rating components, fire protection systems, HVAC system renovation, fume hood replacement, plumbing systems, and upgrade of the existing electrical system including interior lighting and standby power requirements. Other upgrades include: accessibility compliance and all new interior finishes including casework and ceiling treatment.

This Project is currently scheduled to start in March 2014. The approximate duration of the construction will be 18 weeks. This project will utilize Lean Construction Methodology/Last Planner and Building Information Modeling for the M/E/P/FP and Drywall coordination process as required. The project will include sustainable design measures and will pursue USGBC LEED Silver certification.
3.0 GENERAL DESCRIPTION OF THE WORK FOR EACH TRADE:
The attached Prequalification Questionnaire ONLY applies to the Bid Package / Scope listed below. If a subcontractor wishes to prequalify for more than one bid package then the subcontractor shall complete and submit a Prequalification Questionnaire for EACH bid package / Scope. The Bid Package Number to furnish all labor, materials, tools, and equipment to construct the UCSD Muir Biology Building 3rd Floor Renovation for the scope listed below is being solicited:

SECTION II - PROCEDURES FOR PREQUALIFICATION 1.0 GENERAL INSTRUCTIONS FOR PREQUALIFICATION:

1.1 There are two parts to the prequalification process. The first part, Part A, includes satisfying DPR’s Central Prequalification Department requirements. DPR’s Central Prequalification Department process and requirements are outlined on Page 7. The second part, Part B, includes a project specific prequalification questionnaire. See the table of contents. Both Part A and Part B of the prequalification must be completed in entirety and per the deadlines, otherwise the prospective bidder will be deemed not prequalified.
1.2 Each prospective bidder must answer ALL of the questions and provide ALL requested information. Any prospective bidder failing to do so may be deemed non-responsive and not prequalified with respect to this prequalification at the sole discretion of the University and DPR Construction, Inc.
1.3 It is critical that the prospective subcontractor fills out all information required accurately, completely, truthfully and to the best of their knowledge. Ambiguous or incomplete information may lead to an unfavorable rating and subsequent status as not prequalified.
2.0 PREQUALIFICATION PROCESS:
2.1 There will be an Optional meeting for all prospective bidders for this Bid Package on Monday February 3, 2014, at 1:00 pm at DPR’s office located at 5010 Shoreham Place, San Diego, CA 92122.
2.2 The Prequalification Questionnaire will be available on the UCSD website www.fdc.ucsd.edu , and
at the DPR Office. The Questionnaire will also be emailed to the attendees immediately following the optional meeting.
2.3 The prequalification process requires accurate completion and submission of the
prequalification questionnaire by all interested bidders for evaluation by the University and DPR Construction, Inc. to establish bidder’s prequalification status to participate in a competitive bid for only the Bid Package listed above in Section I, Item 3.0.
2.4 Any questions or requests for clarifications or interpretations of the Prequalification Questionnaire
must be submitted in writing via email only to:

Questions received after this deadline may be answered at the discretion of the University and DPR.
2.5 With regard to Part A of the Prequalification Questionnaire, DPR’s Central Prequalification,
prospective bidders shall follow the instructions on Page 7. A complete package, outlined on Page 7, shall be emailed to DPR’s Central Prequalification contact person NO later than 4:00 P.M. Wednesday, February 12, 2014.
2.6 With regard to Part B of the Prequalification Questionnaire, prospective bidders shall submit
one (1) original, in a sealed envelope, and one electronic copy emailed to [email protected] of the UCSD Muir Biology Building 3rd Floor Renovation Project Prequalification Questionnaire and required attachments, no later than 4:00 P.M. Wednesday, February 12, 2014.
A. The sealed envelope shall be labeled and include all of the following information: 1) the
project name, 2) the subcontractor’s name, and 3) the Bid Package Number associated with the work for which you are pursuing prequalification.

B. The sealed envelope shall be addressed to: Jeff Kresch, DPR Construction, 5010 Shoreham Place, San Diego, CA 92121.
C. DPR Construction (CM/Contractor) intends to bid on the following bid packages

2.7 Part A of the Prequalification Questionnaire and Part B of the Prequalification
Questionnaire will not be accepted after the dates indicated above. However, the University and DPR Construction, Inc. reserve the right, but are not required, to waive non-material irregularities, and to request, to receive, and to evaluate supplemental information after the above time and date at its sole discretion.
2.8 Prospective bidders will receive notification of their prequalification outcome, by email. Once the
bidder has been successfully prequalified, the bidder will be invited to participate in a competitive bid for the bid package / scope for which they were prequalified. The prequalified bidder will be notified when the bid documents will be made available. Only those bidders who have successfully passed the prequalification process may submit a bid for the Bid Package for which they were prequalified.
2.9 Any prospective bidder found not prequalified as a result of the bidder’s answers to the Prequalification Questionnaire may request a review by the University and DPR. Any such request must be received by DPR within three (3) calendar days after receipt by the prospective bidder of the determination. Review request must be submitted in writing by email to Jeff Kresch, of DPR. His email address is [email protected]. The decision resulting from such review is final and is not appealable within the University of California.
2.10 Any person or entity not satisfied with the outcome of the prequalification must file a written notice challenging the outcome within ten (10) calendar days from the date of DPR’s written notice regarding prequalification determination. Any assertion that the outcome of the prequalification process was improper will not be a ground for a bid protest.
3.0 EVALUATION FACTORS FOR PREQUALIFICATION:
3.1 The Prequalification Questionnaire will be evaluated utilizing a combination of pass / fail questions as well as utilizing a pre-established point / rating system. Specifically, Questions 1 thru 14. With regard to Part A, insurance, safety, financials and surety categories are all pass/fail.
3.2 All information submitted for Prequalification evaluation will be considered official information acquired in confidence, and the University will maintain its confidentiality to the extent permitted by law.

1.0. DPR Construction Standard Prequalification for All Projects
Failure to complete DPR’s Central Prequalification process outlined below in the allotted time will result in prospective bidders being deemed not prequalified for the UCSD Muir Biology Building 3rd Floor Renovation Project.
1.1. If your company has never prequalified with DPR, then your company will be required to submit a COMPLETE Prequalification Package to DPR’s Central Prequalification Department NO LATER THAN 02/12/2014 4:00 PM

provided by the Subcontractor to the DPR’s Central Prequalification Department:
• Complete the online form. • Provide a current sample insurance certificate with any applicable endorsement. • Provide your OSHA Form 300A for the past three years. Provide a letter from your Surety Company stating your single project and
aggregate bonding capacity. Provide third party prepared Financial Statement for your most recent fiscal year
end.

D. It is very important that your company follow the instructions above and submit a
COMPLETE package to DPR’s Central Prequalification Department on or before the deadline indicated. If DPR receives an incomplete prequalification package, this will result in the prospective bidder being deemed not prequalified for the UCSD Muir Biology Building 3rd Floor Renovation.
