APPOINTMENTS Appointments are released from 8am Monday-Friday for both routine and same day emergencies with a GP and Nurse Practitioner. Healthcare assistant and practice Nurse appointments can be booked after 11am. Appointments can be made by calling us on 02086301111, online or coming into the practice. We continue to review our system regularly in order to give patients swift access.

PREFERRED GP All patients’ registered GP is Dr Talac Mahmud, this does not stop you from seeing other GP’s in the practice. If you would like to see a preferred GP please inform reception when booking an appointment.

TELEPHONE CONSULTATIONS M a n y i t e m s c a n b e d e a l t w i t h appropriately over the telephone quickly and efficiently. This includes discussing invest igat ion results, prescript ion enquiries, issuing medical certificates and some minor ailments. Please call between 08:00-10:00am Monday to Friday to speak to a GP.

TEST RESULTS Patients will receive a call or text message 7 to 14 days after an investigation has been performed, these results can be viewed online.

REPEAT PRESCRIPTIONS Requests for repeat prescriptions can be made online, in person, by fax, post or through a pharmacy. We do not accept requests by telephone. Please allow at least 48 hours before collection. We can send your prescription electronically to your local pharmacy, speak to your pharmacist or our reception staff for more information.

ONLINE ACCESS This service allows you to view, book and cancel GP appointments via the internet 24hours a day. You can also request your repeat prescriptions, view your coded medical records including blood tests results and vaccination history. Please enquire at reception for your username and password.

HOME VISITS Home visits are for the house- bound and for patients too ill to come to the surgery. If you do need a visit please let us know before 10:00am. Whenever possible please try to attend the surgery, as you will be seen more promptly.

DISABLED ACCESS The premises is accessible to wheelchair users. We have a lowered reception desk for wheelchair users to speak to reception, a disabled toilet, a hearing loop and numbers in brail on all the doors. Please speak to reception staff if you require any assistance.

REGISTRATION INFORMATION We are a growing practice and welcome new patients living within our practice boundary (TW3, TW4, TW5, TW7). All new patients are requested to attend a health check when they register. This is a 15 minute appointment with the healthcare assistant. Patients can register at any time during opening hours Monday to Sunday in person or online through our website. Please bring forms of identification including proof of address.

COMPL IMENTS , SUGGEST IONS AND COMPLAINTS We aim to provide an efficient and courteous service and we welcome feedback of all kinds. if you wish to comment on any aspect of our service, whether positive or negative, please bring it to the attention of the practice manager ([email protected]). We welcome suggestions on how to improve our services.

THE PRIMARY HEALTH CARE TEAM The clinic staff and administrative staff work together as a team, so we can coordinate high standards of care. Every team member plays an important and valuable role in allowing the practice to run efficiently. Many of the traditional roles that were undertaken by a doctor are now done by the nurse practitioner, practice nurse or healthcare assistant all of whom have a wealth of medical expertise. This allows more time for the GP to concentrate on more difficult cases. The doctor may arrange for you to have follow up appointments with other members of the team. We encourage our patients to take responsibility of your own health by not smoking, drinking alcohol in moderation, exercise regularly and eating a sensible diet.

PERSONAL HEALTH INFORMATION Patients have the right to expect that their personal information will be held in confidence by the practice. From time to time the practice puts together statistics which support audit and research; patient i n fo rmat i on w i l l a lways be kep t anonymous whenever possible. You will be free to decline to participate in research studies if you wish with no impact on you medical care.

INTERPRETER SERVICES We can arrange interpretation services to patients who do not speak English. We will need at least 48 hours notice.

CHANGE OF PERSONAL DETAILS If you change your name, address, or telephone number please let us know. Name changes must be backed by supporting documents. Please advise us of your mobile number and email to improve communication from the practice.

PATIENT RIGHTS AND RESPONSIBILITIES Patients have a right be treated with respect and courtesy at all times and practice staff also expect the same courtesy from patients. Patients have the right to except that their personal information will be held in confidence by practice staff. You will be seen by your preferred doctor wherever possible. We ask that you attend appointment times promptly and inform us if you cannot attend 24 hours in advance.

ZERO TOLERANCE unreasonable /offensive /threatening behaviour or attacks on members of the staff or the general public and damage to the surgery premises or property will result in patients being removed from the practice list and the police will be contacted if necessary. DISCRIMINATION The practice will ensure that all patients and visitors are treated equally with dignity and respect, and will not tolerate any discrimination against, or harassment of any visitor for reason of age, gender, marital status, race, ethnicity, disability, medical condition, social class, appearance, sexual orientation, religion or belief.

OUT OF HOURS If you require medical attention or advice outside surgery hours please call 111.

URGENT CARE The local Urgent care centre is located at West Middlesex University Hospital, Twickenham Road, Isleworth TW7 6AF
