OPERATION AND MAINTENANCE REPORT JULY - 2016

PREAMBLE

NHAI amidst national competitive bidding appointed M/s.Sowil Limited vide

NHAI/ROHYD/11014/1/13-RJY/5587 Dated 28th June 2013 as an independent Engineer during the

Operations and Maintenance for AP – 15 packages (Km.200 to Km.253) i.e. (901.753 to 848.746)

being executed by M/s. Rajahmundry Expressway Limited respectively.

Prior to our mobilization, M/s.Louis Berger Group Inc., and M/s.Artefact Projects – Zaidun

Leeng.sdn.bhd (JV) have conducted as Independent Engineer to this project since the

implementation period.

The O&M activities have commenced from, 24th Sept. 2004 as discussed in Minutes of Meeting

with NHAI, IE & Concessionaire, (refer letter no.AP:ZL/Proj-Rajah/Site/2004/C/1538,

dt.24.09.04), during September 2004. During that meeting, Organization Chart regarding

deployment of Staff and Technicians was discussed and finalized during the same meeting and

accordingly manpower and resources were deployed by Concessionaire.

The Concessionaire has submitted O&M Manual (Operations) vide letter

no.REL/RDRP/CE/1(C)/2993/2004, dt.12th April 2004. The official unveiling of O & M Manual

and inauguration of Maintenance Yard Building took place by the Member Tech, NHAI on 10-10-

2004 at Maintenance Yard.

The other milestone events of the project are as below;

Provisional Completion Certificate as on 20th September 2004 was issued by IndependentEngineer vide letter no.AP: ZL/ Proj-Rajah/Site/2005/C/1794, dated 6th May 2005.



Completion Certificate was issued to the Concessionaire vide letter no. no.AP:ZL/Proj-Rajah/2007-08/2511, dated 25th April 2007 as per the approval of Competent Authority of NHAIvide their letter no. NHAI/20/Vol.9/PIU-RJY/2007-08/9637 dated 19.04.2007.

Pushkara Lift Irrigation Scheme has been undertaken as per the 2nd supplementary Agreement dated15th March 2007.

Completion certificate for Irrigation Bridges has been issued as of 31st July 2007.

As a part of concession Agreement, 5th & 10th year periodic maintenance works are completed.The Project MPR has been submitted in six chapters covering various facts of the projects.

1. CHAPTER

1.1 EXECUTIVE SUMMARY

1. Mobilization of O & M crew:

a) The O & M activities are in progress in accordance with Minutes of Meetingwith NHAI, IE & Concessionaire, dt.24th Sept.2004 (refer letter no.AP-ZL/Proj- Rajah / Site / 2004 / C/1538, dt.24.09.04), during Sept.2004.

b) The Quality Assurance / Quality Control Laboratory located at 859.203(Ch.242+540) near village Chinnempeta has been established and operationalsince COD.

c) This Package is having a Maintenance Yard at Chinnimpeta (Ch.242.540)with the facilities for relief in emergency situations such as Hydra toe awayCrane, Escorts JCB 3D excavator cum loader, Ambulance with first aid kit,Highway Patrolling Vehicle, daily inspection crew vehicle. Also since thenthe quality control lab with all highway material testing equipments is inoperations. Stores is well equipped with Highway Safety material, first aidkit required by the Emergency Response Protocol Team as well as trafficfurniture like metal crash barrier, delineators for guard posts, trafficsignages, and other essential material set is available.

d) The Concessionaire has submitted all different statutory O & Mrequirements as per Schedule – I of the Concession Agreement to theIndependent Engineer. The Safety Plan, Emergency Response Protocol,Environmental Plan, Traffic Management Plan and Method Statement forwork and O&M manual have all been submitted and accepted in principleby the Independent Engineer in consultations with NHAI.



1.2 PROJECT BACKGROUND

1.2.1 Introduction

The Government of India through the Ministry of Road Transport and Highways have proposed tocarryout a major exercise to improve (strengthening and widening to 4 lane) the highwaysconnecting various cities. One of the gigantic proposals is a Golden Quadrilateral connecting fourmetropolitan cities viz.

a) Delhi to Kolkatab) Kolkata to Chennaic) Chennai to Mumbai via Bangalore andd) Mumbai to Delhi

Amidst International Competitive bidding, the National Highways Authority of India awarded the53 km from Rajahmundry to Dharmavaram Road upgradation to M/s. GAMMON INDIALIMITED on Build, Operate and Transfer (BOT) Annuity basis. The Project was executed througha SPV M/s. Rajahmundry Expressway Limited.

M/s.Zaidun Leeng Sdn. Bhd. In joint venture with Artefact Projects were the Independent Engineerstill 20th July 2009. Subsequently M/s.The Louis Berger Group, Inc. have commenced the servicesas an Independent Engineer for the project vide letter No.NHAI/12014/2009/RD/GM(CK-I)/Tech/1005 dated 13th July 2009 and completed the service on 30th June 2013 vide letter No.NHAI/66/LBG/PIU-RJY/2013-14/16089 dated 28th June 2013.

Further M/s Sowil Limited have commenced the services as an Independent Engineer from 1 st July2013 for the project vide letter No.NHAI/RO-HYD/11014/1/13-RJY-5587 dated 28 th June2013

The milestone events of the project are as under :

a) Provisional Completion Certificate as on 20th September 2004 was issued vide I.E letterno.AP:ZL/Proj-Rajah/Site/2005/C/1794, dt.06.05.05.

b) Completion Certificate was issued to the Concessionaire vide letter no.AP:ZL/Proj-Rajah/2007-08/2511, dated 25th April 2007 as per the approval of Competent Authority of NHAIvide their letter no.NHAI/20/Vol.9/PIU-RJY/2007-08/9637 dated 19.04.2007 no.AP:ZL/Proj-Rajah/Site/2005/C/1981 w.e.f. 30th December, 2005.

c) IRRIGATION CANAL Under Pushkara Lift Irrigation Scheme is crossing the corridor atkm.879+083 & km.889+310 (Mallepalli village & Murari village). At these two crossings twoBridges were constructed as Deposit Works under 2nd Supplementary Agreement dated15.03.2007.These Two Bridges were completed and Completion Certificate of these IrrigationCanal Bridges was issued Technical Consultants vide letter No.AP:ZL/Proj-Rajah/Site/2007/IRRI/2703 dt.31.12.2007.

2.2 PROJECT STATUS

2.2.1 Provisional Completion Certificate: -

Provisional Completion Certificate as on 20th September 2004 was issued byIndependent Engineer – M/s. Zaidun-Leeng Sdn. Bhd. & Artefact Projects (JV) videletter no.AP:ZL/Proj-Rajah/Site/2005/C/1794, dated 6th May 2005.

2.2.2 Completion Certificate:-

a) Completion Certificate was issued to the Concessionaire vide letterno.AP:ZL/Proj-Rajah/2007-08/2511, dated 25th April 2007 as per the approval ofCompetent Authority of NHAI vide their letter no.NHAI/20/Vol.9/PIU-RJY/2007-08/9637 dated 19.04.2007, subject to admissible exclusion of the following:

i. Protection work to the 2 Nos. Pipe Culverts (Stone Pitching, Filter Media andApron on RHS) at Ch.229+160 km and Ch.229+356 km.

ii. Construction of Toe Drains 570m.

iii. Footpath (755 m) and Covered drains (65m).

The estimated cost of these omitted items amounting Rs.3, 15,085/- (Rupees ThreeLakhs Fifteen Thousand and Eighty Five only) shall be recovered from the paymentdue to the concessionaire against Change of Scope Order issued time to time.

The above amount has been recovered from the Concessionaire payments.

b) Completion Certificate for the construction of 2 Nos. Irrigation Canal crossingsunder Pushkara Lift Irrigation Scheme has been issued to the Concessionairevide letter No. No.AP:ZL/Proj-Rajah/Site/2007/IRRI/2703 dt.31.12.2007 by theTechnical Consultants (M/s. Zaidun-Leeng Sdn. Bhd. & Artefact Projects).Presently the

c) bridges are under maintenance with the Concessionaire as per the 2nd

Supplementary Agreement.



3. CHAPTER

MAINTENANCE ACTIVITES

(A) ROUTINE MAINTENANCE

Description of Activities Status of Activities

Cleaning of pavement, removal of debris orany unwanted material from Project highway.

Cleaning of pavement, removal of dead animal & debrisetc. is being done on regular basis.

Repair/Replacement of road furnitureincluding crash barrier, road marking, signage,guard post, railing, kerb etc.

Fixing of broken/spoiled/removed Guard Posts, MetalBeam Crash Barriers and reflective stickers is beingreplaced. Fixing of damaged PG Rails Work in Progress.

Repair of damaged earthen shoulder,embankment slopes, drain, removal oflandslide etc.

a) Repair of damaged earthen shoulders to be rectified

b) Maintenance of Cross drainage work is in progress

Maintenance of plants & shrubs to ensureproper growth.

The Trimming, manuaring & watering is being doneregularly. Replacement of dead plants / missing plantshas been replaced.

Removal of weeds & other vegetation frommedian, earthen shoulders & slopes.

Though it is being carried up on daily basis. Removal ofRank Vegetation and vegetation on slopes up to ROW isin progress.



(B)EMERGENCY MAINTENANCE

(C) OTHER MAINTENANCE


Maintenance of all Illumination Work

Cleaning of luminaries, replacement of damagedluminaries is being done as and when required. Thedamaged electrical poles and burnt cables are work inprogress.

Maintenance of highway landscape The trimming, manuaring & watering is being doneregularly some more attence is suggested.

Maintenance & cleaning of public placessuch as bus bays and bus shelters.

Cleaning, removal of debris at Bus bay & Bus shelterlocations has done on daily basis.

Maintenance of median drains andlongitudinal drains

Cleaning of median drains which are deposited with silt,sand and waste. Disposal of debris from longitudinaldrain from time to time. Covered drains silt to beRemoved in built up areas.

Maintenance of Highway Signs and RoadMarking. Any damage to traffic signswhich reduces or threatens to reduce fulland clear visibility shall be rectified withintwenty four (24) hours of its occurrence.

All Traffic Signs has been kept in clean and correctalignment and position. The fixing of missing Signboards work in progress.

Maintenance of Safety Barriers Replacement of damaged safety barriers like MBCB,PG rails etc completed.

Maintenance of road furniture Repair / replacement of road furniture is being done.

Maintenance yard, Fee plaza &Administrative building.

The hygienic conditions at Maintenance Yard & Trucklay –bye - maintained regularly.



(D) SAFETY AND TRAFFIC MAINTENANCE

Encroachments wherever occurred are being removedand unauthorised access are observed in some locations.The Concessionaire is instructed to be more vigilantthrough Route Patrole vehicle to curb encrochments /unauthorised access.

Safety during the maintenance of road

Safety precautions during the road maintenance worksare being taken by the Concessionaire like Flag menwith necessary jacket, Warned sign Boards, diversionboards, cones etc.

Safety during the Road accidentsThis is being maintained with the cooperation of localpolice, fire stations and hospitals with their mobilenumbers.

5.2 Operation and Maintenance Report

On verification of Concessionaire’s O & M records, the Independent Engineerfound that the Concessionaire has performed prescribed inspections in the followingformats.

i. Accident Report

ii. Road Maintenance

iii. Maintenance of Structures / Underpass etc.,

iv. Maintenance of Highway Road marking and Signages, Lighting.

5.3. Inspection / Observation Report

Inspection / observation report has been forwarded to Project Director NHAI,Rajahmundry during the month and copy of the report is in Appendix – ‘A’.



CHAPTER. 6

6.0 MONTHLY INSPECTION REPORT

6.1 Visual Inspection Report:

6.1.1 Side slopes: The Concessionaire is replacing damaged pitching on side slopes maintenanceregularly.

6.2 Close Inspection Report:

6.2.1 Pavement Riding Surface:

· The Concessionaire is maintaining the pavement for good riding surface.

6.2.2 Expansion Joints.

· The Concessionaire is maintaining the “Silt up Expansion Joints” regularly.

6.2.3 Median Kerb:

· Damaged kerb is being rectifying as a part of routine maintenance.

6.2.4 Other facilities-Vegetation and Landscaping:

· Landscaping at PIU is being maintained satisfactorily.

· Maintenance of Median Plantation has been carried out, Pruning has also been carriedout.

· Maintenance standards of Avenue Plantation have to be improved. Periodical manure,watering to be carried out regularly.

· Missing/ Dead plants are being replaced during rainy season. Concessionaire has beenreplaced.

6.2.5 Other facilities –

· Illumination is being maintained regularly at Built up areas, Service Roads andjunctions needs to be improved desired required percentage as per ConcessionAgreement.



6.2.6 Other facilities – Way side Amenities:

· Bus bays are being maintained regularly.

· Maintenance yard is being maintained regularly.

6.2.7 Drainage – Side / Toe drain:

· Re sectioning of the toe drain, cleaning of lined drain, etc. is being maintained.

6.2.8 Bridge - Hand rail:

· Damaged hand rails replace & painted.

6.2.9 Traffic operation Facilities - Marking:

· Faded thermoplastic pavement marking is to be maintained properly.

6.2.10 Traffic operation facilities – delineator (Guard Posts)

· Road Delineators (Guard Posts) are functioning satisfactorily.

6.3 Thorough Inspection Report:

6.3.1Traffic operation Facilities signs:

· All sign boards are being maintained satisfactorily..

6.3.2 Traffic conditions:

· Overall condition of traffic management is satisfactory.

· Smooth traffic flow has been ensured during the month. Accident vehicles have beencleared after obtaining necessary clearance from local police.

6.3.3 Encroachments:· 24 hour patrolling is being carried out to monitor encroachments.· Encroachments wherever occur are being removed as a part of maintenance.



Inspection Report of the Independent Engineerfor the month of July, 2016:

In pursuance of Schedule-I, after receipt of Concessionaire's O&M StatusReport for the month of July, 2016, a Site Inspection was carried out by theTeam Leader of Independent Engineer on 02-08-2016. The following are theobservations:

O&M Activities :

· The Concessionaire have been attending the maintenancerequirements and safety requirements communicated by theIndependent Engineer. Most of the existing Highway is observed withpot holes. The Concessionaire has attended the work and it is inprogress.

· Drainage spouts on bridges are cleaned and it is observed thatthe work is in progress.

· Roughness Index test was conducted in presence of IndependentEngineer and report submitted.

· BBD Test also conducted by the Concessionaire in presence of theIndependent Engineer and report submitted

· Removal of Rank Vegetation up to ROW is in progress.

· Replaced the Metal Beam Crash Barriers wherever damaged withnew one.

· Median Plantation & median grass work is maintained by theConcessionaire, satisfactorily.

· Safety requirements are not complied as informed by theIndependent Engineer. Even the instructions given during the lastinspection are not attended.

· Foot Paths, cover drains in built-up locations & service roads arenot being cleared.

· Illumination in the corridor has to be improved and desirable limitnot yet reached.

· Cat-eyes / pavement makers are to be fixed.

· Side berms / gravel shoulder are badly damaged at manylocations. Need to be rectified immediately.



Incidence Management:

The Independent Engineer has inspected for AP-15 & AP-16 Packages on 18th

& 19th July, 2016 and the following observations are made:

a) Ambulance :

The following items of basic life support system are verified:

i. Automatic loading stretcher;

ii. Spine board full, Vacuum splint kit;

iii. C-Collars;

iv. Oxygen Cylinder with pressure indicators;

v. BP Instruments;

vi. Stethoscope;

vii. Resuscitation bag;

viii. Manual foot operated suction pump;

ix. I.V. Fluid and I.V. Sets;

x. Tourniquet;

xi. First-Aid Box;(Dressing materials / Antiseptic lotion/Analgesicetc.)

xii. Blanket;

xiii. Fire extinguisher;

xiv. Laryngeal mask;

xv. Torch lights;

xvi. Drinking water;

xvii. Registers of incident / accidents;

xviii. List of Medicine Register etc.

xix. List of Hospitals;

xx. Mobile phones for communication.



The following paramedical staff with proper uniform and name of theperson written on the uniform:

1. Trained Paramedical Staff with knowledge of first aid : 3 Nos.

2. Driver for ambulance: 3 Nos.

b) Route Patrol, Hydra Crane & JCB Vehicles :

Round the clock Highway Patrolling is being done by Patrolling Vehiclesfor security and safety of the project and to prevent the unauthorizedencroachment / obstruction in the entire project length.

The following items are verified in the above Vehicles:

i. Fire extinguisher;

ii. Gas cutter with protective glass;

iii. Water container with fresh water;

iv. Rubber Gloves;

v. Brooms one hard bristle, other soft;

vi. Rain coat; Blanket;

vii. Torch lights with spare batteries;

viii. Flashing light;

ix. Hydraulic jack, towing chain, rope;

x. Toll set (with standard set);

xi. Digital Camera, measuring tape;

xii. Paper pad, pen / pencils;

xiii. First aid kit;

xiv. List of Hospitals;

xv. List of ROW details.



Each vehicle should also carry Traffic Management Equipment, such as

a) Sign boards – “Accident ahead”; “Lane merging”; “Direction

Arrows”; “Speed Limit”; “Keep Left / Right”;

b) Flags, whistle, reflective hand signal;

c) Traffic cones;

d) Barricades, tape;

e) Reflective jackets

f) Mobile phones.

The following manpower deployed with patrol vehicle:

1. Route Patrol In-Charge;

2. Route Patrol Assistant;

3. Driver, with knowledge of vehicle repairs.
