Co-Op Thanks You for Your Support!

School Supply Packages Co-Op is pleased to announce we will be offering pre-packaged school supplies for the upcoming 2020-2021 school year. Let us take all the stress and anxiety out of back to school shopping! This is our first fundraiser for the new school year, and while it is an optional fundraiser, we would appreciate as much support as possible. How exactly does this work? SIMPLE….

1. Fill out the attached order form for the pre-packaged school supplies including any extras you may need.

2. Take a picture of your order form and email it to [email protected] You will receive a confirmation email by next day saying the order form was received. If you do not receive confirmation please re-send your order form via email.

3. Go to the tuition portal, under incidentals select “school supply package”, enter your total from the order form and pay. If you do not have a First Bank and Trust portal please contact Mrs. Andi at 504-394-0757 or [email protected]

It’s that easy! If you have any questions email [email protected] All the school supplies will be delivered to the classroom and your child’s teacher will have them ready for the first day of class. Dear Our Lady of Perpetual Help School Parent,

We are happy to inform you that First Bank & Trust and Our Lady of Perpetual Help School are partnering for the 2020-2021 school year for the management of tuition and fees. Welcome to First Bank and Trust! We offer a convenient, secure portal for the one-time payment of tuition and/or fees or for the financing of tuition. The following contains some important information about the website and your program options.

Easy Online Access: If you are new to the First Bank and Trust website, you may log in at https://tuitionportal.fbtonline.com to set up your user identification and password. If you are already enrolled as a user on the FBT website for another school, you will not have to register again, but simply use your existing login and password to set up your OLPH student(s).

School Fees and Payment Options: Beginning Thursday, February 1st, the FBT website will be programmed with the following fees for the 2020-2021 school year – Tuition, Technology Fee, Classroom Activity Fee, Co-op Dues, and the Non-Supporting Fee. Your registration fee must be paid directly to Our Lady of Perpetual Help School. The payment of other programmed fees will be accepted via the FBT website using a one-time automatic bank draft, or a credit or debit card. There is no fee if you choose to pay using a bank draft. If you use a credit or debit card, a 2.99% processing fee will be added to the amount charged to your card.

Financing Option and Loan Terms: The Annual Percentage Rate on the tuition loan for Our Lady of Perpetual Help School is 8.0%. This rate is fixed over the school term and there is no pre-payment penalty if you decide to pay the loan off early. In addition, there is no credit check required for the borrower(s) and you will not be required to submit financial information. A ten month tuition loan will advance on June 1, 2020 and monthly payments will begin in July 2020. An annual fee of $35.00 will be assessed on the advance date of your loan and billed on your first billing statement.

Convenient Automatic Loan Payments: If you elect to set up a tuition loan, you will be required to set up monthly loan payments on an automatic bank draft from your checking or savings account. You may choose a due date of the 3rd or the 15th of each month. A $25.00 fee will be assessed for any returned payment. If a returned payment results in a delinquent status on your loan, an additional $15.00 late fee will be assessed.

On the FBT website, you will have the option to set up a tuition loan or to pay full tuition and school fees for the 2020-2021 school year.

WHAT YOU WILL NEED TO GET STARTED:

Your school’s 2020-2021 Tuition and Fee Schedule Email Address (Your User ID on the website) Driver’s License or State ID (If you are setting up a tuition loan) Social Security Number for Borrower(s) (If you are setting up a tuition loan) Bank Routing and Account Number for Automatic Bank Drafts if you are setting up a tuition loan

Step 1 – Log into the website at https://tuitionportal.fbtonline.com and click “I Want to Register” to set up your User ID and password. If you are already enrolled on the site for another school, you will use your existing User ID and password.

Step 2 – From the “Summary Tab” on the dashboard, click “You have not added any students. Add one now.” Complete your student’s name, date of birth and then click “SAVE” (you may leave the identification type and number boxes blank).

Step 3 – On the next screen, click “New Registration for this Student”, then click in the box to find your school, then click “Next” to enter your student’s grade for the upcoming school year, and click “Next” again.

Step 4 – The next screen displays your “Shopping Cart” which shows the tuition fee categories and parish classification (catholic, non-catholic, supporting or non-supporting). Please select the appropriate categories and click “Save”.

Step 5 – The next screen shows your updated “shopping cart” with your itemized fees listed for the student entered. If you have another student to add, click “Add Another Student” and repeat the process. If there are no other students to add, click “Save and Continue to Payment.”

Step 6 – The next screen is your “Order”. Scroll down this page to view your payment options. Please refer to your school’s Tuition and Fee Schedule to see which fees, if any can be financed with tuition. If you choose to set up a tuition loan, click the first box that says “Finance Up to (the maximum amount allowed) and proceed with completing the loan application. After completing the loan application you may return to your “Order” to pay any remaining unpaid amount.

General Instructions:

1. Students will choose grade-level books from the list (page 2), or a book of

your choice as long as its on a 5th grade reading level.

2. Students will complete the required elements explained on pages 3 and 4.

3. Students will submit their work with the completed cover page (page 6) and

rubric (page 5) on Monday, August 10, 2019.

4. Grading will be according to the rubric on page 5.

5th grade summer Reading Assignments

Incoming 5th grade students must read one summer reading book and complete required steps 1 & 2 to earn

credit for the assignment. It is due on the first day of school.

Book Jacket: (50% of test grade)

Step #1: Create a book jacket for your book. The cover should include the title, author's name, and an

appropriate illustration. The back should include a 1 to 2 paragraph original summary of the story. The

inside front flap contains an author's biography and a photograph/illustration. The inside back flap will

contain information on the setting, time period, and characters. Along the spine, include the title and

publication date. Do not give away the ending of the book anywhere on the book jacket.

Your cover may be completed by hand or with a computer or a combination of the two. Accuracy,

neatness, and effo1i are imp01iant. Remember, the purpose of a book cover is to create interest in a

reader. In this case, it is also to convince the teacher that you have read and understand the book. See

attached cover example (page 6).

Use a full sheet of paper (at least 8 1/2 X 11 inches), plus attached paper for flaps for your completed jacket.

Flaps may be attached with clear tape. You may also use longer paper or tape two sheets of printer paper

together.

Projects: (50% of test grade)

Step #2: Choose one of the following projects. Complete and turn in with the book jacket and cover page for your final grade.

A. Write 20 questions for the book with complete answers. Write a variety of questions that show a depth of thought and do not simply refer to the plot. Imagine you are leading a book club or class discussion. Some sample questions are:

I. Who was the main character and where did he live?II. What was the dilemma faced by this character?III. Would you have handled his problem in the same way?

B. Compare the book to its movie version. Compare and contrast at least three points of the stmy. ( 5 paragraphs)

I. Paragraph One - Introduce the book by briefly discussing the author & story lineII. Paragraph Two -One point of comparison/contrast between the book and the movieIII. Paragraph Three -Second point of comparison/contrast between the book and the movieIV. Paragraph Four-Third point of comparison/contrast between the book and the movieV. Paragraph Five -Concluding paragraph which evaluates the book and the movie, discusses

what you liked/disliked about each, and which one you like better and why you like it better

C. Take photographs or create illustrations and make a photo album that depicts the experiences of a character. Include a minimum of 10 original photographs/illustrations. Photos/illustrations should include captions that indicate relevance to the book. Captions are explanations that accompany a picture or illustration. They may be fragments or complete sentences and should be punctuated accordingly.

D. Create a set of flashcards to teach and review facts from your reading assignment. Topics should

include examples from all of the following: vocabulaiy words (words that you had to look up tounderstand), facts about the sto1y itself, facts about the author, and facts about the setting or characters.You should have at least 40 cards with the answers included on the back of each card.

Hello Incoming Fifth Grade Parents!

I hope this finds everyone safe and well. In place of the traditional summer math packet, students are to complete 45 minutes a week on xtramath.org for 10 weeks over the summer. The goal is to build fact fluency. All minutes are due by August 9th. The xtramath minutes will be collected as a 100 point summative assessment for the first quarter of the 2020-2021 school year. Below I have listed the student expectations and grading scale pertaining to this summer assignment.
