Office of Coal Mine Safety
Following the Crandall Canyon Mine Disaster, where six mine workers
and three rescue workers died, the State of Utah passed the Utah Coal
Mine Safety Act establishing the Office of Coal Mine Safety.
The Office of Coal Mine Safety purpose is to:
Maximize Coal Mine Safety and prevent accidents
Assist in MSHA with all major coal mine accidents
Investigate any report of alleged unsafe acts or conditions in a mine
Manage the State of Utah’s emergency response plan for major
coal mine accidents.
Coal Miner CertificationThis Office administers 12 tests per year, one quarterly test and two retests
for each of the following:
Underground Mine Foreman
Surface Mine Foreman
Underground Electrician
Surface Electrician
Fire Boss
Coal mine certification/training and questions regarding coal mine safety

To report a hazardous condition at a mine to the Federal government’s
