I will observe strict compliance with all rules, regulations, requirements and instructions related to the NTU Philosophy Enygma Orientation 2015. I note that that NTU Philosophy Enygma Orientation 2015 is a day camp and that there will be no staying overnight in school involved. I acknowledge that participation in the NTU Philosophy Enygma Orientation 2015 comes with inherent risks. I hereby declare that I am aware of the risks involved and am participating at my own free will and volition. I will assume all such risks myself. I consent to the collection and use of my personal data above for the sole purpose of NTU Philosophy Enygma Orientation 2015.
