ISO 2015, Risk Management, and FMEA’s

On September 15th the ISO 9001:2015 standards were released. A significant part of the

standard that has been revised is an update around Risk Management. To comply with the

standard, ISO is interested in whether your organization has sound practices for Risk Manage-

ment and Corrective Action. As a former manager of an organization’s ISO standards and au-

dit system, I know just how much of a challenge it can be satisfying the standard’s require-

ments for these areas of concern.

One of the best tools and approaches I have found for Risk Management is the use of Failure

Modes and Effects Analyses (FMEA’s). The FMEA is a form of risk analysis that centers

around a series of key questions that is focused on identifying “potential” failure modes of your

system (of a product, process, or service) and the effects the failure modes can have on your

customers and organization. Because the tool is looking for potential issues, issues that typi-

cally have not happened yet, it is a great tool for Proactive Risk Management.

I am aware that many organizations do not like using FMEA’s because it can be a difficult tool

to use. But those who do use it find that it is worth more than the effort it takes to help their

organizations manage the potential risks that might come their way in the day-to-day efforts.

I know from my personal experience that external ISO auditors are quite ecstatic when they

find organizations using the FMEA process for their Preventive Risk Management. I have

seen few to no audit findings in this area of the code in the organizations that are effectively

using FMEA’s.

There are several approaches to doing Failure Modes and Effects Analysis. The most thorough

standard to support doing FMEA’s I have found can be purchased from the AIAG organization.

vite you to the Iowa Quality Center’s Lean Six Sigma Network Meeting on Wednesday, Decem-

ber 9th, at 7:30 AM at the Kirkwork Training & Outreach Services (KTOS) Center at 3375 Ar-

mar Drive in Marion, Iowa. I will be giving a short presentation on the use of FMEA’s and will

be having an open discussion on other organization’s uses of this tool.
