Operate hospital services in Sale, Heyfield and Maffra. Allied Health Services the Sale, Maffra and Rosedale area. Following referral, a Care Coordinator undertakes an initial visit to ascertain eligibility andlevel of services required. A pool of volunteer drivers take eligible clients to medical appointments both regionally and to Melbourne.

Residents of Phillip Island and areas between Island & Melbourne. residents of other Bass Coast areas when available

For Bass Coast Residents attending medical appointments treatments & who can't access public transport; live in remote areas; are unable to drive; can't afford a taxi; are not eligible for alternative schemes such as Ambulance Patient Transport or DVA transport, are able to get into and out of a domestic passenger vehicle with minimal assistance, sit safely and travel for long distances/times; require no medical treatment or monitoring during transit; Are able to walk unassisted or negotiate at least two steps the help of a walking frame or stick; Are unable to drive or be driven to appointments by family/friends.

Elderly, frail aged, people and people with disabilties. Services incorporate Briagalong, Heyfield, Loch Sport, Maffra, Rosedale, Sale and Stratford.

Frail older people and adults with disabilities, who can enter and exit a standard vehicle with minimal assistance. Not eligible are people living in hostels or nursing homes.

NON-EMERGENCY TRANSPORT SERVICES DIRECTORY

Volunteers drive and escort clients to medical appointments and other approved activities. Assessment of eligibility is required. Limited transport and escorting available to assessed clients who require occasional assistance to getting to a local health related appointment.

Free inter-hospital shuttle operates between the three sites. Call 9496 2997 for veteran services.

Limited volunteer transport service for physiotherapy patients without family members to take them to appointments, and who lack ready access to transport.

Transport can be provided to assist residents to get to medical, hospital, dental or other health appointments during business hours.

Door-to-door service available Monday to Friday for medical appointments and other specialist activities. Fully trained volunteer drivers. All volunteers have satisfactory Police checks and Working with Children checks. Associated with Traveller's Aid medical companion railway program.

Aims to provide assisted (non-urgent) transport to medical and theraputic appointments.Priority will be given to people attending medical appointments. Clients must be medically stable to be eligible and in some cases my require a medical certificate to show proof of this. Socially isolated or housebound residents who are unable to use mainstream transport due to frailty or disability.

Mansfield Shire Council has volunteers who work in community transport.

Clients must be eligible under Home and Community Care funding. Provision is based on assessed needs. Service depends on volunteer and vehicle availability.

Community transport is organised from four locations within the Shire. Moira Healthcare Alliance manages two cars in both Cobram and Numurkah and Nathalia has one care owned by hospital and managed by the Community Association, whilst Yarrawonga Hospital owns and manages another.

Frail aged, people with a disability and their carers living in Mallacoota, Genoa and Gypsy Point, who do not have friends or family to take them to medical appointments.

Provides transport assistance to eligible people to attend specialist appointments outside Mallacoota. This service extends to Bega (NSW), Canberra (ACT), Orbost, Lakes Entrance, Sale and Bairnsdale (VIC).

Regional and rural travellers who are aged, or living with a disability and who otherwise rely on family members, friends or community transport to drive or accompany them to Melbourne.

Assessment is required to ensure eligibility for the community transport service. Availablity is dependant on volunteers and vehicles. Clients need to be mobile enough to get in & out of cars unassisted. Wagons are not wheelchair-accessible, however buses can be used for medical transport if necessary.

Provides individual transport to medical appointments at medical centres, doctor and specialist rooms, hospitals etc.

Clients will be asked:-How long do you think the appointment may take; and When you will be ready for the return journey.

If the client is unsure then SWCT will allocate an hour and ask the client to contact SWCT from the appointment if they will be ready earlier or later than the hour allocated.

Frail aged people or people with a disability who require support to maintain their independence, and carers seeking support, living within Pyrenees Shire.

Frail older people, people with disabilities and carers unable to access other means of transport to appointments.

A pilot project is available until June 2012. Trained volunteer medical companions will meet travellers at Southern Cross OR Flinders St Station and then accompany them to and from their appointments in central Melbourne.

Older and frail persons with moderate, severe or profound disability, younger people with moderate, severe or profound disability, and carers of these people.

Door-to-door service, very limited resources available.

Volunteers are available to provide transport to medical appointments or to assist people when travelling on public transport/taxis. Volunteers may be able to remain with people to provide additional support during medical appointments. Referrals shoudl be made at least 5 working days prior to date of required transport.

Frail elderly and people who are 60 years and over, who have no other means of transport, aren't in residential/supported accomodation and who live in Portsea to Safety Beach area.

Frail aged people, people with disabilities and their carers without family or social support and who are unable to access private or public transport. Services are within 55km radius of Mildura and part of MRCC and into the Millewa.

Appointment required to access service. Transport available 8.15am-5pm. Volunteers are registered and trained. Medical appointments cover: doctor/specialist, pharmacist/chemist, podiatrist, pathology, radiography, physiotherapy, chiropractor. Escorted medical appointments support the client in any way they desire to assist in a positive outcome from a medical appointment, ensuring they can continue to live independently.

HACC target group: people living in low-cost housing, transport disadvantaged people. Across shires of: Kingston, Bayside, Glen Eira, Stonnington & Port Phillip. Priority is given to eligible people who live in supported accomodation and are on a statutary income.

Managed by volunteers, the medical transport service coordinates transport to & from medical appointments for clients across the community. At least 3 days notice required. Mon-Fri 9am-1.30pm

Timboon & District Healthcare Service provides this service throughout the southern section of the Corangamite Shire. To attend appointments, such as with a dietitian, dentist, podiatrist or physiotherapist, clients will be requested to use the service closest to them. For travel to specialist appointments or hospital admissions, clients may be referred to the Red X for assistance. Please give 2 days notice for local trips and 5 days for longer trips.

People who cannot drive them- selves or do not own a car; have no partner or relative who is able to drive them, and have no access or are unable to afford public transport.

Priority given to: transport disadvantaged; continuing frequent trips after bone marrow transplants; have low blood counts ie, at a high risk of infection; are undergoing active treatment; undergoing supportive therapies, eg, blood transfusions; require admission or day investigations.

VPTAS assists rural Victorians with travel and accomodation costs incurred when travelling long distances for specialist medical/dental treatment.

Veterans and war widows/ers

Victorian residents living in a Dept of Health designated rural region, receiving specialist medical treatment and needing to travel more than 100km on way or an average of 50km per week.
