If no e-mail address is specified, postage fees of £1.50 per invoice may apply.(3) Authorised person to legally sign on behalf of the company

I, representing the End-customer shown above, have decided to transfer the telephone numbers associated with the above account from the Losing Communication Provider (LCP) to NFON (UK) Ltd. NFON UK Ltd is authorised to act on my behalf on this matter.
I recognise that it is my responsibility to arrange cessation of, or changes to, any other services currently provided by the LCP if required.
I understand and accept that the following services will be terminated upon completion of the port: Redcare Alarm, Broadband service, Fax*, PDQ* & Franking* machines and monitoring services such as alarms and utility meters. Other services such as, but not limited to,1571/Call Minder, Call Barring and Call Diversion* will also be ceased. Any messaged left via the 1571/Call Minder service will be lost. I accept to cover any potential contract termination charges which may arise from LCP may apply.

section is about your current phone line. These details must be completed correctly or the porting will get rejected. - Full name and CUPID (Communication Provider Identity) of your current communication provider and Range Holder (Your current communication provider should supply these details). Eg. BT CUPID is 001.- Customer name: The contact name on your telephone bill (leave blank if it's not there) - Company name: The company name, as it appears on your phone bill- Account number: Your account number with your current supplier.- Billing address: Your billing address exactly as it appears on the phone bill.

Porting Information - Filling RemarksThis section lists the numbers you want to port.- Confirm the number of numbers to be ported to avoid errors- List the Main billing number- Confirm the action on the main billing number- Numbers to be ported: The list of numbers and ranges you want to port.- Preferred porting date and time: Single line ports can usually be done in 2 weeks, multi-line ports can take up to 6 weeks in extreme cases. You can specify 'as soon as possible' or you can specify a target date - as long as it's not too soon.
Current phone line - Filling RemarksAny information you feel will help us with the port. Any remote diverts set up? Has the installation address changed recently?If there are associated DDIs which you are not porting, are they to be ceased or left on the current line?
Number porting is charged at £20.00 for the first number + £10.00 for each additional number up to a maximum of £100.00 (excl. VAT). Please note that failure to provide the correct information will result in port rejection. Please note that ALL rejections will be subject to a £15.00 (excl. VAT) charge, please confirm the information requested with your losing service provider prior to submission. Please note that once numbers have been ported over to NFON UK Ltd you are advised to contact your losing provider to ensure that you are no longer being billed for the line rental. If you require any additional information or clarification with regards to this document please call NFON UK Ltd on 020 3740 6740.
