Shawn Halcsik DPT, MEd, RAC-CT, CPC elected to NASL Board of Directors for 2014-2015

Evergreen Rehabilitation’s Vice President of Compliance and Clinical Services Shawn Halcsik, DPT, MEd, OCS, RAC-CT, CPC, CHC has been elected to the National Association for the Support of Long Term Care’s (NASL) Board of Directors.

The National Association for the Support of Long Term Care (NASL) is a trade association founded in the Fall of 1989 by advocates of professional medical services

to long term care facilities. NASL now represents the legislative and regulatory interests of approximately 100 companies providing physical, occupational and speech-language pathology therapies, portable x-ray/EKG and ultrasound; health information technology systems; and products such as complex medical equipment; parenteral and enteral supplies, equipment and nutrients; and additional specialized supplies for long term and post-acute care settings nationally.

The mission of NASL is to represent ancillary service providers and suppliers in the long term and post-acute care community on healthcare policy and advocate for high-quality, cost-effective care. Combining a wealth of expertise and intricate, daily knowledge of managing facilities, NASL allows members to work together and celebrate the hurdles and successes in working within the long term care ecosystem. We are tremendously proud of Shawn’s efforts and achievements and we look forward to the positive impact this will have in the future for both NASL and Evergreen Rehabilitation.

In honor of National Rehab Week September 21st through the 27th 2014, Evergreen Rehabilitation is shining a spotlight on our Rehab Stars & Rehab Teams that go above and beyond the call of duty to promote not just extraordinary patient care and outcomes, but also patient satisfaction and enjoyment as well. Part of making therapy truly effective for our patients is being able to engage in a fun, friendly way and able to show a little creativity. Evergreen Four Star Award Winner, Brandi Hall COTACongratulations to our latest Four Star Award winner! Brandi works at Hillcrest Healthcare Center in Ashland, TN and was nominated by her rehab director, Dan Hall. In his nomination submission, Dan had the following to say about Brandi:

“I am writing to nominate Brandi Hall, COTA at Hillcrest Healthcare for this year’s Four Star Award. It is difficult to limit her value to this department, facility, and its’ residents within a few short paragraphs. I have worked with Brandi for over 4 years now as her OTR, and she has blessed me with being both a wonderful discipline partner and as an inspiring co-worker. She is beloved by her co-workers and always works wonderfully within the interdisciplinary team. Without question, Brandi is one of the most important, caring, and influential employees at this facility. She provides for all patient care needs with outstanding quality, no matter the degree of hardship or level of personal sacrifice.”

It would be impossible to count the number of times I’ve seen Brandi go above and beyond the requirements of her position to provide excellent patient care. This has become Brandi’s norm. She provides residents with a shoulder to cry on, constant encouragement, and other forms of uplifting emotional support and love on a constant basis. She always presents with a positive attitude and approach. She regularly makes decorations, cards, and other items at personal expense and on personal time. She is truly selfless in her pursuit to provide daily needs to her patients and other residents.

Brandi excels at determining patient needs, desires, and preferences. It can be easy as therapists to become locked into our daily routine. Often we become so focused on the diagnosis, our goals, and our treatment plans that we do not focus on the individualistic healing that the patient needs in order to truly return to their PLOF (prior level of function). Brandi provides our therapy staff with the constant reminder that by treating the person AND the hardship, optimum progress and improvement can be better achieved.

In regards to the profession and the OT discipline itself, Brandi has a constantly eager mind and attitude. She has a very high motivation to absorb information for various deficits and diagnoses so that she can be most efficient and effective with her individual patient treatments. No matter the level of difficulty, time constraints, or complexity of treatment, she has always demonstrated a patient-first mentality. As a result, she has become a highly competent, knowledgeable, and experienced therapy assistant. She is a great ambassador for the profession and is an irreplaceable representative for Evergreen Rehabilitation.”

And now some words from the FSA winner herself. Here are Brandi’s thoughts on working with the residents at Hillcrest:

“Growing up in the city of Nashville, TN was wonderful, but shortly after graduation I found myself settling down in the small town of Ashland City, TN. After the birth of my daughter in 1997, I began caring for my grandmother. Her dementia diagnosis took us all through a 3 year journey that became my inspiration to pursue a career in geriatrics. Finally, at the age of 30 I decided to pursue a 2nd career as an Occupational Therapy Assistant. Luckily, I found a position close to home at Hillcrest Healthcare. I joined the Hillcrest therapy team 6 years ago. At that time, they were contracted with a different therapy company. After a year or so with the old company, Evergreen Rehab took over the therapy contract.

Since that time, Evergreen has improved our therapy department and helped to place us on a path to success. I consider myself lucky to be working at Hillcrest where the residents radiate the small town atmosphere that I love so much. Most of all, our therapy group has become my extended family. We have developed a unique dynamic that fosters an atmosphere that helps residents to adapt to environmental changes and more quickly begin their recovery process. When outside of work, I enjoy and love being a mother the most. Recently, my daughter and I have teamed with her cheer gym to establish a competition dance program for children with special needs. We are hoping it will be the beginning of a program that will last for years to come.”

Editor’s Note: Sarah works as a physical therapist at Northpoint Medical & Rehab Center in Oshkosh, WI.My desire to be a physical therapist began in high school. I knew I wanted to work in the medical field but I wasn’t sure about which area until I heard about physical therapy. I was very interested in the idea of working with people with a variety of diagnoses and ages and helping them to achieve their goals. I interviewed for PT school when I was a high school senior and was guaranteed acceptance into the program if I continued to meet certain criteria throughout college. I graduated with my Doctorate in 2007 and joined Evergreen Rehabilitation in 2008. Throughout my time working in Oshkosh, I have worked with some very unique personalities and cases. I worked with one gentleman who had whole body gout, and when he first came to the facility he could not even sit at a 45-degree angle in bed due to the fact that he was lacking range of motion (ROM) in all joints.

He was on and off caseload as his condition changed and when he left the facility he was able to walk without an assistive device on his own. I’m also reminded of another gentleman who suffered a cerebrovascular accident (CVA) and was dependent with all mobility and cares and couldn’t follow a one step direction. His doctors didn’t think he had rehab

potential but when we asked to work with him they signed the orders. When he left our facility he was able to walk out the door with a cane! One of the most rewarding aspects of working in a nursing facility is the opportunity to learn about our patients and their lives. I have worked with former police officers, veterans, and former professional athletes; people who have overcome drugs and abuse, and people who had been in prison. Each person has their own story that has shaped who they are and influenced their expectations of both themselves and others.

They have taught me many lessons including to believe in yourself, strive for your goals despite what others believe is possible, and to be thankful for the abilities you do have. One of my favorite lessons, however, is that no matter how old you are, enjoy your life and you don’t have to act your age. Dance when you can, say what you want, laugh often, and no matter your age, flatulence is always funny.

Employee Spotlight: Lisa Locke, PTA1. What initially sparked your interest regarding the field of physical therapy?When I was a senior in high school I was playing soccer and tore my ACL. I subsequently had surgery and many months of physical therapy. That is when I decided that this is what I want to do when I graduated college. I wanted to help people. I graduated from UCF in Orlando with a BSc with an emphasis in health and social sciences and then applied for a PTA program.

2. What is your personal Evergreen Rehab story? How did you join the team?Before joining Evergreen Rehabilitation almost 5 years ago, I worked in outpatient physical therapy for 12+ years. As rewarding as it was, I wanted to challenge myself and learn more. I never had a clinical rotation or position in a skilled nursing facility (SNF). The week I decided to research skilled nursing facilities I received a flier in the mail about joining the Evergreen Rehab team. I went online that night and filled out an application, and approximately 10 minutes later I received a phone call from Barbara Reed to schedule an interview with the Director of Rehab (DOR) at Manatee Springs. And the rest is history.

3. What unique experiences/patients have you encountered as a therapist?Last year we had a male in his late 40s who suffered a TBI (traumatic brain injury). He was extremely low level with a trach/PEG tube/PICC/catheter... Within a couple of months of rehab, he went from not having any trunk control or safety awareness to sitting in a wheelchair to ambulating with a RW with CGA and eating like a horse. After discharge he continued with outpatient therapy to maximize his function and patient/caregiver education. It is very rewarding when you have a hand in one's success. On a different note; I have the opportunity to be a clinical instructor for local PTA programs. I take 3+ students under my wing a year ranging from 4 to 8 full-time weeks. We both learn during that time and I love it. It is rewarding when they pass their boards and enter the working world as a licensed therapist. To know that I had a small part in their success is very gratifying. In fact, some of my past students work PRN (per diem) for Evergreen!

4. Is there a specific disease process and/or impairment that particularly interests you, and why?Working with patients with dementia/Alzheimer's Disease are fun. You have to be creative in your treatment approach. It is rewarding and 99% of the time it puts a smile on my face. We have all been challenged with someone you can't budge, no matter what!!

5. What makes working in skilled nursing facilities (SNFs) rewarding?I enjoy working in a SNF because it opens up the concept of working as a team to provide quality care to the patients. One individual or discipline can't do it all on their own. If I can walk down the hall and get a smile or hello from someone then I know I have made a difference in someone's day; even if it was for a temporary moment. I leave work everyday knowing I have done the best I could to help patients reach their maximum function.

6. Any lessons that patients have taught you?Thankfulness, patience, and compassion.

7. Is there any personal/family history you would like to share?I married my best friend Christopher Locke 17+ years ago and we have 2 beautiful, energetic, witty, fun-loving boys - Benjamin (9 years old) and Cameron (6 years old). I enjoy spending time with my family, exercising, going to the beach (we are fortunate to live near one of the top 10 beaches in the country in Sarasota, FL) and going on vacation.

October was National Physical Therapy Month! Every year in October, the American Physical Therapy Association (APTA) celebrates the ongoing contributions that physical therapists (PTs) and physical therapy assistants (PTAs) make every day in helping “transform society by restoring and improving motion in people’s lives.”

“Transforming society by optimizing movement to improve the human experience,” is the modus operandi as the APTA continues to raise awareness by educating and informing patients and families about how important the therapists’ and assistants’ skills and training are in helping to improve the functional outcomes of each patient.

Though modern physical therapy as we understand it came about in the 1920s, the act of rehabilitation through physical therapy dates back dozens of centuries and is thought to have been instituted by Hippocrates, a Greek physician, often referred

to as the Father of Western Medicine. By advocating massage, manual therapy and hydro-therapy, Hippocrates was on the forefront of his time - creating new ways to diagnose and treat patients. Modern physical therapy can be traced back to founder Mary McMillan who in 1921 set about organizing the first Physical Therapy Association (now known as the American Physical Therapy Association (APTA). Led by President McMillan, an executive committee governed the Association in which there were 274 charter members. By the end of the 1930s, the Association changed its name to the American Physiotherapy Association. Men began to be admitted and membership grew to just under 1,000 members.

Therapists help patients overcome obstacles!

“In the 1960s, APTA membership reached almost 15,000 and the number of education programs nationwide grew to 52. Now headquartered in Alexandria, Virginia, APTA represents approximately 76,000 members throughout the United States. A national professional organization, APTA’s goal is to foster advancements in physical therapy practice, research, and education.”

Physical therapy as a discipline has grown by leaps and bounds since the days of Hippocrates, and even since McMillan’s time. Modern physical therapy utilizes a variety of modern technologies, research, and modalities to optimize patient outcomes. As a contract therapy provider for skilled nursing facilities throughout the United States, Evergreen Rehabilitation employs hundreds of PTs and PTAs nationwide.

We are committed to providing ongoing education to all of our skilled therapists to keep them up-to-date on what they need to know to provide patient centered, individualized, evidence based care. During the month of October, we will be celebrating PT month by providing PT-centric lunch-and-learns, featuring several of our PTs and PTAs on our blog, as well as having weekly challenges and prize drawings (through e-mail). Employee Spotlight: Carter Ordaz, PT

1. What initially sparked your interest regarding the field of physical therapy?The opportunity to help others. This is one of the guiding principles that led me in search of a career that would allow me to help other people and at the same time find financial security. It’s been my passion to help and serve others, especially the elderly, and [before PT] I would often find myself doing volunteer jobs in our community. Another driving force that led me to PT is my intense curiosity in human anatomy, physiology and kinesiology. It was my mom who prodded me to take this route since at that time physical therapy was a booming profession. From then on out, I knew I had made the right decision - for several reasons. First off, this career fits my extrovert personality since I’m able to be engaging with different patients everyday as well as working with other medical professionals (including OTs, SLPs, social workers, medical doctors, nurses, etc.). The broad diversity of personal interactions and treatment programs makes my work interesting and exciting. Secondly, it’s also important to me that my career provides me with financial and job security - which physical therapy absolutely does.

2. What is your personal Evergreen Rehab story? How did you join the team?It was back in September 2013 when I joined Evergreen Rehab via Ashland Health and Rehab in Mississippi. It was my first assignment and my first professional experience working as a physical therapist in the United States. Even though I have more than 10 years experience working as a physical therapist in different countries like the Philippines, Syria, Jordan, Lebanon and Saudi Arabia (and have had extensive training in Florida on how the US health care system works), this first assignment was the most exciting part of my career. Finally, meeting the team for the first time was exhilarating. After spending a few hours with them, we were already like a well-oiled machine, ready to rock and roll.

3. What unique experiences/patients have you encountered as a therapist?One of the most memorable and unique experiences I have ever encountered so far as a physical therapist was when I was working for a special education school in the Philippines. A cerebral palsy patient was referred to me - he was a 2 year old boy diagnosed with spastic diplegia. His progress was minimal and we were getting frustrated, however, that did not deter us from achieving our goal. The determination of the child was overwhelming. He did not complain and always engaged and participated well with therapy, almost as if he understood that he needed therapy in order to be functional. Finally, our combined efforts paid off. After more than a year of facilitation, stimulation, reflex integration and inhibition, he was able to sit unsupported and we were on our way to training him to stand. However, as fate would have it, during this period a job opportunity for me in the Middle East became available. I finally decided to go back to the Philippines for a vacation. While I was there, my family took me to one of the malls in town and I remember I was standing outside the Apple store when a child with bilateral ankle braces and crutches approached me and said “Thank you, Teacher Carter…thank you for helping me walk.”

4. Is there a specific disease process and/or impairment that particularly interests you, and why?There are a number of different disease processes that interest me and one of them is CVA (cerebrovascular accident, AKA stroke). Aside from being broad and debilitating, it has had a personal impact on me and my family. My grandfather and some of my kin have been victims to this terrible debilitating disease. The impact of disability and complications of CVAs to both patients and their family are tremendous. As a physical therapist, one of my goals is to contribute to the community by finding ways to effectively rehabilitate people with CVA and its’ complications. I constantly strive to find the best way to facilitate normal movement and improve (if not restore to) normal function. It is always said that prevention is better than cure but, at this time, prevention of CVA is only plausible. The majority of people living today, if not everyone, has a family history of CVA. The advent of fast food (which contains high fat and sodium), easy accessibility to cigarettes and alcoholic beverages, and a lack of regular physical activity are high among the reasons why we cannot eradicate CVA.

5. What makes working in skilled nursing facilities (SNFs) rewarding?It is always exciting and a joy to meet different people, but it’s also amazing when you get a chance to hear their interesting stories and pick up a few lessons on how they’ve lived their lives. There is always something that you can learn from your patients’ insights. One of the many interesting patients I have met is a 92 year old stroke patient and a World War (I & II) veteran. Despite his age, he was determined to get well and be independent on all activities of daily living (ADLs).

Another patient I remember was this 70 year old grade school teacher who was reunited with her favorite student who just happened to be a CNA in the facility. She even shared with me her secret for making the perfect baked beans, but I must admit, I didn’t write down her instructions and sad to say, my baked bean recipe is far from being perfect. Working in SNFs also gives me the chance to share my talent in making videos, creating posters, and creating t-shirt designs for both employees and residents whenever there is a special activity.

6. Any lessons that patients have taught you?Enjoy life while you can, never give up, and always thank God. These are the just some of the lessons I have learned from my patients. It does not matter where you are and what your situation is, there is always something that you can thank God for. The experiences shared by a war veteran on the innocence of the young have taught me that if you focus on what is good then good things will happen in your life. Allow me to share with you a personal experience. Working in the Middle East was hard, even for a physical therapist. There is no religious freedom there and privileges are limited, especially if you are an expatriate. However, as soon I got to know their culture, beliefs and traditions, I began to slowly understand them and appreciate my stay with them. Again, there is always a reason to enjoy life, for example, waking up every morning to see your loved ones is enough reason to enjoy the rest of the day. Having good health is another reason. As a physical therapist, our patients depend on us to provide encouragement and they are expecting us to be positive and full of energy at all times. We have the unique gift and responsibility of being their source of hope, and since we are given the gift of healing, we should always be thankful for the opportunity and do our share in helping other people. “Do what you love to do and give it your very best... Life is too short to waste.”

7. Is there any personal/family history you would like to include?Last year was an amazing year for me, and I received a life changing blessing. It was the approval of my working visa in the USA. Then finally, after waiting for 14 months, I was reunited again with my family. It was so exciting to see my loving wife Wendy and our two wonderful kids Candy and Carlisle. My wife and I decided to meet in California so that we could spend a couple of weeks together touring Los Angeles. The kids enjoyed the city tour including Universal Studio, Hollywood, whale watching and meeting up with old friends and relatives. We also went to Las Vegas and stayed at MGM. Now, living in Mississippi, they are enjoying the fresh breeze of their new environment. On a personal note, I would like to share the spotlight with the Ashland Health and Rehab team for their unwavering support and acceptance of me as part of their family. Of course, I must also thank Evergreen Rehab for giving me the opportunity to work with your company and helping me to further develop my career. I would also like to acknowledge my family for inspiring me to do my job the best way I can and for all the support, love and encouragement they provide. Lastly, I praise God for all the blessings and strength that He has given me and my family. Without God’s grace, I could never be a Physical Therapist and would not be where I am today. Theme all week long was “Back to the 80’s”. The therapy team sponsored activities/theme for the day for the entire facility, as well as had daily quizzes and prizes, all therapy related. From splints/assist devices, diet texture education & balance activities. Day after day more facility and staff joined the spirit of the therapy team and truly went back to the 80’s. Of note, the rocker in the Zebra pants is the administrator.
