NHS App – Personal
Health and Care Record
There is going to be a new online service
for Sherwood Forest Hospitals (SFH)
patients via the NHS App and Patients
Know Best (PKB). This will give people
secure access to their Personal Health and
Care Record from their smartphone, tablet
or computer. It’s a free service which
already works with a number of other NHS
organisations. Features people will be able
to access as part of this service include:
consultations virtually via a questionnaire, video, voice or secure messaging
the ability to self-monitor, track symptoms and test results
virtually access care plans to help manage their care safely at home
easily share their health data with family, carers or any health and care professionals anywhere if they choose.
As part of this process people will start to
receive an email or text from PKB to ask
them to register for the service. So far it is
being piloted in the critical care Covid-19
wards to help staff keep carers/relatives
updated on their care. Already 20 relatives
and carers have been invited to use PKB
so SFHFT can communicate remotely with
carers/relatives during this time when they
are unable to visit.
It is already being used by many of our GP
Practices and we will start to roll it out to
other services within SFH over the coming
months.

Plans to re-establish health and care
services:
Nationally and locally plans are being
developed for a restoration and recovery
period, with restoration referring to the re-
establishment of critical service and
recovery the wider set of health and care
services.
This work will look to return health and
care service to their broad range of
functions, while also seeking to retain
some of the benefits and lessons learned
from some of the transformational
changes we have made (e.g. speeding up
safe discharges, increased use of remote
GP consultations).
