The purpose of this Board is to control and regulate the licensure of all physical therapists and physical therapist’s assistants within the State of Nevada to ensure public protection. Items relating to the function of this Board can be placed on the Agenda by writing this Board.

• Items on the agenda may be taken out of order. • The Board may combine two or more agenda items for consideration. • The Board may remove an item from the agenda or delay discussion relating to an item on the

agenda at any time. Public Comment Procedure: 1. Public Comment will be taken at the beginning and at the end of each Board meeting; 2. Public Comment may also be taken at other such times as requested so long as the

request that Public Comment be taken will not interrupt ongoing Board business; 3. Those individuals wishing to present concerns to the Board are to sign in prior to

speaking; 4. Depending on the number of individuals wishing to address the Board, a reasonable time

limit may be set. The Board will not restrict comments based upon viewpoint; 5. No action may be taken upon a matter raised during Public Comment until the matter

itself has been specifically included on an agenda as an item upon which action may be taken.

Item 1- Call to order / roll call to determine the presence of a quorum. Item 2 - Pledge of Allegiance. Item 3 – Public comment period. (Discussion only). Item 4- Applicant Andrew Reed requests to be licensed. This appearance is necessary

due to the applicant’s arrest record. (Action). Possible closed session for the Board to discuss the character, professional competence, alleged misconduct, or physical or mental health of the person as allowed under NRS 241.030.

Item 5 - Licensee Mindy Renfro requests the Board to reconsider the terms of her probationary license. (Action). Possible closed session for the Board to discuss the character, professional competence, alleged misconduct, or physical or mental health of the person as allowed under NRS 241.030.

Item 6- Applicant Alvin B. Lagarto requests to be licensed. This appearance is

necessary to review the applicant’s work history. (Action). Possible closed session for the Board to discuss the character, professional competence, alleged misconduct, or physical or mental health of the person as allowed under NRS 241.030.

Item 7- Applicant Ercel Binalla Basilio requests to be licensed. This appearance is

Item 8- Applicant Tina Marie Thornton requests to be licensed under the provisions of

Item 9- Applicant Valeska Naire Villagran Sosa requests to the take the National

Physical Therapy Examination for physical therapist’s assistant’s a fourth time in accordance with NRS 640.100(4). (Action). Item 10- Applicant Anthony R. Soto requests to the take the National Physical Therapy

Item 11- Applicant Julie L. Ornelas requests to be licensed. This request is necessary to

state, not licensed in Nevada, can make an assessment and determination of eligibility for a citizen of Nevada to certify disabled ridership for the paratransit service. (Action).

Item 25- Update from Paula Berkley regarding the activities of the Sunset Commission,

which is reviewing all licensing Boards, and consideration of her proposal to represent the Board during the upcoming legislative session. (Action.)

Item 26- Discussion and possible recommendation for re-appointment of Lisa Mukavitz

and Emilio Puentedura for their terms expiring on October 30, 2012. (Action).
