1.1 IntroductionMumbai Railway Vikas Corporation (MRVC), MMRDA & Doctors for You (DFY)have been working in the M-east ward of Mumbai for the past three years for improve-ment in the socioeconomic and health indicators of the PAP’s resettlement colonies.The interventions are being carried out through two health centres in the area. TheHealth centre in Natwar Parikh Area is functional since 2010 and in Lallubhai com-pound since 2013. The services are provided on daily basis by the team of doctors,paramedical staffs and outreach workers. The services are provided at the health cen-tres on daily basis like general OPD, dental OPD, Tuberculosis treatment, Immuniza-tion, Growth monitoring OPD, family planning and maternal & child health services.In the health centre at Lallubhai compound specialized services i.e. physiotherapy anddisability rehabilitation, Gynaecology clinic, Eye clinic and Skin disease treatment etc.are also provided. The OPD services are backed up by laboratory services and phar-macy. With the opening of health centre, quality health services are easily accessibleand affordable to residents of these PAP’s resettlement colonies and nearby slum ar-eas.DFY has now been providing health care services to more than 1.5 lakh popula-tions.Apart from services at the health centre, health services and awareness activities arealso organized in the area through community meetings, rally, street plays etc. Themain aim of the project is to provide preventive and curative healthcare services to all.

1.2 Highlights of the Month

1.2.1 Tuberculosis awareness meetingOn 28th February, 2017 MRVC-DFY team had conducted Tuberculosis awareness meet-ing at Natwar parikh centre, Govandi, Mumbai. In this meeting DFY team memberswere taking counselling session, disease related preventive measures and beneficiarieswere given free soaps to promote handwashing hygiene. Patients were told to maintaincleanliness to their surroundings.In addition to that DFY team have explained major sign and symptoms of tuberculosislike:

• Evening rise of fever

• Weight loss

• Loss of appetite

• Cough with expectoration more than 15 days if there then they are advice toconsult MRVC/ DFY TB clinic immediately.

1.2.2 Dental Health Camp

On 16th and 25th of February Dental health camp had organized at MRVC-DFY Lal-lubhai and Natwar Parikh Health Centre.In a regular DFY practice and in association with De-addiction centre MRVC/ DFYteam conduct Dental Health Camp. As this area and surrounding 90% people are ad-dicted for pan, tobacco and smoking. So naturally dental health is epidemically at high-est risk. MRVC-DFY team had conducted oral hygiene awareness and dental healthcamp. Beneficiaries were 32 in Natwar Parikh Centre and 68 in Lallubhai Centre.

1.2.3 Diabetes Health Camp and Sugar Check up

On 8th and 14th February, 2017 DFY team had organized free diabetes health campat both MRVC-DFY Lallubhai health centre and Natwar Parikh Centre in afternoonsession. In that session some activity has done like Checking of random blood sugar,blood pressure, Health talk regarding diet, life style modification, exercise, follow upcheckup.Special training was given for nutritionl deficiency and currentive measures. Total 34and 56 patients were benefited from Natwar Parikh Centre and Lallubhai MRVC-DFYHealth Centre respectively. Total Beneficiaries were 90 patients. Some patients de-tected their high level sugar first time in life. That’s aim of diabetic camp.

1.2.4 Malnutrition Prevention sessionDFY team organized a malnutrition prevention session with Anganwadi workers in theLallubhai compound on 16th Feb 2017. During this session 26 women were well edu-cated regarding Malnutrition.In fact all these women were convinced and they have admitted also that “ we were

not knowing that effect of junkfood, stored food, readymade food, cold drinks, choco-lates, maggy, biscuits can cause so much harm for health but DFY team given us goodeducation regarding malnutrition”.

1.2.5 Outreach Mobile Immunization services at Man-dala, Bainganwadi and Shivaji Nagar area in Go-vandi

MRVC-DFY had been conducting outreach immunization camps at Mandala slums,Mankhurd in association with MCGM and Zahara Hasanat Trust, Mumbai. As a partof scaling up of outreach immunization services, new outreach spots have been identi-fied in Bainganwadi and Shivaji Nagar area of Govandi in consultation with MCGM.Awareness sessions are conducted in the areas and beneficiaries are provided free Im-munization services along with Vitamin A supplementation and Deworming services.Two sessions at different spots are conducted each day. Every month new places forconducting the session are identified and added to the list. A target of total 40 Immu-nization spots and sessions in a month has been set for this Mobile Outreach Immuniza-tion Project increasing by 5 spots every month. To incentivize the families availing thisservice and to promote the concept of Hand Hygiene, free soaps also were distributedto the beneficiaries for the promotion of hand hygiene under the Soap for Hope project.

1.3.5 Statistical data of Dermatology (Skin OPD) ser-vices

The Skin OPD (Dermatology) services were started at MRVC Health Centre, LallubhaiCompound in July 2015. The same service has been started at Natwar Parikh fromSeptember 2016. This services are available four times in a month in every Thursdayfor both the centers and out of 1st and 3rd week services have been sponsored by DignityFoundation and rest of are founded by Doctors For You.

1.3.7 Geriatric OPD ServicesMRVC DFY Health Centre, Lallubhai Compound, Mankhurd has started a new Geri-atric OPD services in collaboration with Dignity Foundation from 5th December 2016which have been provided from Monday to Saturday in every week.

1.3.11 Statistical data of Under Five ClinicThis clinic was started for the children of 0-5 year age group. This group is the mostvulnerable age group for malnutrition and subsequent mortality due to under nutritioncomplicated by infections. The aim of Under-five Clinic is to promote health of motherand children of this age group.Services:

• Measurement of Height and weight

• Treatment of common conditions of mother and child

• Assessment of immunization status

• Provide nutritional education to mother

• To provide family planning counselling and services

In this clinic both mother and child are provided comprehensive health care services atfree of cost. Since this is health promotion clinic, mother is asked to follow up everymonth with child for growth monitoring.

1.4 Case stories of patients from Lallubahi com-

pound

1.4.1 Case Story of a ChildA 3 year old boy came with the complaints of black eye all over right side due totrauma. Parents were very anxious and decided to consult with ophthalmologist inMRVC-DFY Health Centre on 4th February, 2017. Child was experiencing severe painand unable to cooperate daily activity. Dr. Faraz ( Ophthalmic Surgeon ) had attendthe boy and find out the diagnosis was Ecchymosis . Doctor has given advice to locallyapply cold compression and anti inflammatory drug. After two weeks the boy hascompletely recovered and gets relief. These are just few examples, but DFY team dealswith many complicated which otherwise would have been neglected by their patentsdue to unavailability of expert doctors and illiteracy.Therefore now Ophthalmic OPD increased from once in a week to three times in a weekdue to overwhelming demands from local area people.

1.4.2 Case Story Of Dental PatientA 24 years young patient came with Chief Complaints Of Missing Tooth. He wanted toget a replacement which will last a life time . DFY team suggested him dental implants.These implants are one of the most advanced branch of dental field. It is milestone ofthe Dental Team & DFY- MRVC health center.

Dr. Zaheer (BDS, Oral Implantologist) had been performed dental Implants surgerywith the help supported doctors of at MRVC DFY Health Centre..

1.4.3 Case Story of a childA 4 years old boy came with the complaints of tightening of upper arm and lowerlimb and not able to put his arm in the mouth and also complaints of not able to standproperly. The boy has been continued his treatment to MRVC-DFY Health Centre sincelast 1 year. When he has come the centre he cried a lot and not to cooperate with doctorsand staffs. Dr. Shivlaxmi Murti (Physiotherapist) attended the child, given advice andcounseled about exercise and diet. After completing many follow up checkup he isimproving with reduce tightening of upper arm and he is now self sufficient to takefood.
