clinical track:102 COMPLEX BEHAVIORAL DIAGNOSIS & HOW TO PARTNER WITH BEHAVIORAL HEALTH SERVICES Michael Goldsmith | Behavioral Care Solutions LLCEffective interpersonal skills are central to a mental health clinicians ability to form a sound therapeutic alliance both on a micro and a macro level. Within long term care settings, communication skills form the basis of every intervention and implementation of any systemic developments. Within this presentation we will look at the importance of identifying facility specific mental health needs, the need to be deliberate in communicating those needs to appropriate resources and what mental health resources are available for utilization within long term care settings.

ops & tactics track: 105 HEART FAILURE READMISSIONS: THE FREQUENT FLIERS THAT DON’T FLY FOR FREE: PART 1 OF 2Julie Savage | Capital Internal Medicine Associates HospitalistsNearly 6 million Americans are currently living with heart failure, and close to 700,00 new cases are diagnosed yearly costing insurance companies millions of dollars. Since the passage of the Affordable Care Act in 2012, hospitals are incentivized by CMS to improve quality care. One common method is through better use of post-acute care facilities, yet one in four patients discharged to a SNF is readmitted to the hospital within 30 days. This presentation will discuss the financial cost to both hospitals and SNF’s regarding readmitted HF patients. Participants are encouraged to participate in Part 2 of this program, “Heart Failure: Clinical Management Guidelines” as well.

This session will present the Management of Heart Failure in Skilled Nursing Facilities with focus on unique issues of SNF care related to guidelines and recommendations.
general session:108 SHARING WHAT YOU’VE LEARNED TO PROMOTE THE DEVELOPMENT OF OTHERSDr. Stacey Starling | Starling Center for Organizational DevelopmentDirectors of Nursing and other Nurse leaders often attend educational conferences and professional development programs to learn new strategies and information about the state of nursing education, evidence and practice. Yet, it is commonplace that the daily demands of LTC can be at cross purposes with the ability for the DON or nurse leader to transfer the new knowledge into their nursing education practice and also share what they have learned with their clinical/nursing teams at the facility. This general session will explore the benefits of adopting transfer of learning strategies and best practices to better share conference and professional development information with your clinical/nursing teams at the facility to promote enhanced nursing practices for improved resident care. Through this “train the nurse trainer” Dr. Stacey Starling will empower participants to come away with a better understanding of how to share and impart what valuable education is gained through participation in this year’s Annual Convocation and other nurse educational events/programs.
keynote:109 THE NEXT GENERATION OF INTERDISCIPLINARY TEAMS: WHAT NURSE LEADERS NEED TO KNOW & DOLou Ann Brubaker | Brubaker ConsultingLong-term care providers, including skilled nursing and assisted living environments, are experiencing a paradigm shift with the rise of Accountable Care Organizations, Dual-Eligible Medicare/Medicaid programs, bundled payment care initiatives and value-based reimbursement programs. Assisted living is experiencing its own challenges with the rise of resident acuity, the need to look harder at clinical capabilities and optimizing residents’ length of assisted living stay. One of the first changes in response to these new pressures needs to be a rethinking of how the individual disciplines interact, make decisions and measure their success. This insightful presentation challenges participants to more closely examine how their teams collaborate now and what evolution can help assure their greater success.
clinical track:110 BREATHE IN, BREATHE OUT AND CARRY ON WITH DYSPNEA Melanie Flachs | The Peplinski GroupThis presentation will focus on common causes of dyspnea in the long-term care setting whether acute or chronic. We will examine best practices and strategies that the IDT can use to assess, manage and treat residents with dyspnea. We will examine the tools available to facilitate these decisions.

workforce track:111 HOW TO ATTRACT & ENGAGE MILLENNIAL NURSESLou Ann Brubaker | Brubaker ConsultingAsk many people about Millennials and they’ll use words like ‘slacker’, ‘entitled’, or ‘self-involved’. But is that really the case? In this fast-paced presentation, participants will be asked to examine the myths and the truths about Millennials, as well as what strategies provide greater insight into recruiting and retaining what is now the largest workforce age cohort in America. Beyond how you need to attract and keep them in your organization, the program will also explore what Millennials worry about, how they see their role in the world, what they expect in work culture and their commitment to work/life balance.
clinical track:112 SUSPECT SEPSIS. SAVE LIVES.Joyce Turner | Ciena Health CareAfter completion the participant will be able to enhance overall quality of patient care, improve nurses’ knowledge and reduce 30-day hospital readmissions for patients with infections leading to Sepsis using the
“SIRS” screening criteria for early identification and interventions. The program is relevant for all licensed nurses. It is appropriate for the novice nurse who is fine tuning their assessment skills, as well as an excellent review for the most experienced nurse as it is applied to their own nursing practice.
workforce track:113 HIRING & RETAINING QUALITY NURSES IN SENIOR CARE Lou Ann Brubaker | Brubaker ConsultingIt is heartbreaking when tremendous energy has been invested in recruiting senior care staff and then termination or resignation is the outcome. When this happens it puts the financial, regulatory, occupancy and customer delight goals of the facility at risk. In this 90-minute program the goals of hiring great staff and retaining them is distilled into clear strategies that can be immediately implemented by participants. Attendees will learn to interview potential hires more successfully as well as examine how their current orientation strategies may be contributing to a revolving door. The motivators that cause staff to staff in the long term will be addressed, as well as which of four categories all staff fall into and what to do with each type.
clinical track:114 NON-PHARMACOLOGICAL PAIN MANAGEMENT: EXPERIENCING THE BENEFITS OF TAI CHI/QIGONGFred Goff | The Art of Transition LLCQigong and Tai Chi are Chinese energy exercises that are a series of smooth, gentle, flowing physical movements, which combines rhythmic breathing and circular motion with focused intention. The unique feature of both Qigong and Tai Chi, is the internal cultivation of mental strength and tranquility. When done on a consistent basis, Qigong and Tai Chi have been shown to help strengthen muscles, increase flexibility, increase coordination, and help practitioners gain control of their lives by achieving mental and emotional peace.Almost anyone can learn Qigong or Tai Chi, regardless of his or her physical condition. The exercises can be done in a standing or seated position. The degree of exertion, similar to walking, can be easily adjusted to the participant’s ability. Participants are encouraged to wear loose-fitting clothes and a pair of soft-sole shoes or tennis shoes for interactive participation in this “live” class, appropriate for all levels of physical capabilities.
workforce track:115 COACHING YOUR NURSING TEAM TO INCREASED PERFORMANCELou Ann Brubaker | Brubaker ConsultingThe operational demands upon senior care leaders can often mean that daily coaching of staff for higher (and better) performance gets short shrift, with substantial impact on staff morale. The survey environment especially can affect managers’ focus on “catching people doing things right” versus wrong. In this session, participants will explore how informal coaching can be done more effortlessly, spontaneously and meaningfully. Within formal coaching discussions the writing of performance goals will be described in respect to staff evaluations and improvement in deficient performance.

closing keynote:116 DISCHARGE PLANNING – RESIDENT CHOICE OR FACILITY: WHAT’S AN IDT TO DO?Vickie Burlew | Lebenbom & Rothman, P.C.; Michele Conroy | Rolf Goffman Martin Lang, LLPThis session will examine the difference between a resident-initiated transfer / discharge and a facility-initiated one. Nurses will come away with a better understanding of the state and federal requirements for each – critical to ensuring regulatory compliance and promoting optimal resident outcomes. Nurses will discover how an Interdisciplinary Team develops a person-centered Discharge Planning Process and creates a Successful Transition to the Community Program for consideration of implementation in their own facilities.
general session:117 IT’S NOT THE 80’S ANYMORE: A REGULATORY UPDATE FOR DONS & NURSE MANAGERSAndy Rothman | Lebenbom & Rothman, P.C.; Beth Bacon | Health Care Association of Michigan In the skilled nursing setting, we are facing the largest shift in the paradigm of long-term care in decades...It is certainly NOT the 80’s anymore. Compliance with minimal standards is not sufficient. Quality outcomes and staff competency are the priority. This regulatory update will provide an overview of the new regulations, the timeline to implement, and strategies to operationalize these new regulations to ensure regulatory compliance and promote optimal resident outcomes.
general session:118 EMPLOYMENT & LABOR LAW UPDATE FOR DIRECTORS OF NURSINGJonathon Rabin | Hall, Render, Killian, Heath & LymanThis presentation will provide Directors of Nursing and Nurse Managers with an update on recent developments in the area of labor and employment law, including topics such as wage and hour matters, transgender employees, and areas of interest to the U.S. Equal Employment Opportunity Commission. This informative update will also focus on issues that affect day-to-day workplace matters to ensure nurse participants have a better understanding of the current employment and labor law environment to ensure regulatory compliance.
