From 1 January to 5 April 2016, there were 9 crane-related dangerous occurrences (DOs) with the last 3 occurring within a span of 10 days.
Construction
On 13 February 2016, an electric-powered hoist and a metal frame to which the hoist was attached toppled during lifting operations. 1 worker was injured.
Construction On 17 February 2016, a floor crane fell off the edge of a building. The operator who was near the crane fell together with it to the ground about 11 floors below. He was found dead at the scene.
Marine On 28 February 2016, the boom of a dockside tower crane collapsed during lifting operations. 3 workers were injured.

On 8 August 2016 at about 1950hrs, the Occupational Safety and Health Division of Ministry of Manpower was alerted to an accident involving a construction worker at a worksite in Tuas.
It was reported that at about 1745hrs, a group of workers was lifting a formwork for the casting of the walls of a stairwell, when one of the chains used to lift the formwork snapped, causing the remaining chain section to swing outwards and hit a worker standing nearby. The worker was conveyed to Ng Teng Fong Hospital, where he subsequently passed away at about 1900hrs.

Importance of Crane Safety~ Mr Mohamed Abdul Akbar, Chairman of the National Crane Safety Taskforce» We noted that some of the Dangerous Occurrences were due to inadequate
extension of outriggers, insufficient training of crane operators and disregard for crane manufacturer’s requirements and poor maintenance.
» These DOs could have resulted in deaths if we do not adhere to safe work practices. We strongly urge the industry to review all lifting operations to make sure that they are conducted in a safe manner.
