Supporting the Jewish Home is an investment in multiple generations. We serve today’s seniors in need and simultaneously build our capacity to meet future demand. By bridging multiple perspectives, an annual report is perfectly suited to addressing these dual priorities: taking a look back at yesterday’s progress and putting it in the context of tomorrow’s growth.

This report comes at an auspicious time on both fronts. Keeping the Vision Expand, Extend, Sustain – our $215 million campaign to build facilities and expand and support programs – has met with great success, already generating more than $118 million in funding commitments. Over the past year, fueled in part by the robust generosity of families across our community, we continued to make progress on the development of the Gonda Healthy Aging Westside Campus in Playa Vista, and our goal of

serving 5,000 seniors by 2015 is within reach. We also established a firm footing for the Brandman Centers for Senior Care, the Home’s first Program of All-inclusive Care for the Elderly (PACE), on the Grancell Village Campus, and we launched a wide range of other comprehensive senior healthcare services, all while maintaining a high level of quality care.

Looking ahead, the Jewish Home is poised for even greater accomplishment. Whether it is supporting the construction of new buildings and the remodeling of current facilities at our Grancell and Eisenberg Villages; developing the Hirsch Family Campus in the San Fernando Valley; expanding our renowned Annenberg School of Nursing; supporting the Home’s operations; or contributing significant capital to our endowment, the coming year is sure to be an exciting and fulfilling one. I look forward to sharing it with you, and I thank you for the extraordinary partnership that makes it possible.

At the conclusion of another meaningful year in our development, it strikes me that the Jewish Home is like a fine wine: strong, bold, and better with age. In 2013, our programs and services – already national models for quality senior care – continued to set new standards for innovation and excellence.

I am particularly proud of the newly launched Connections to Care. With a single phone call, seniors and their family members or health advisors can now access the Jewish Home’s wide array of in-your-home, community-based, and in-residence programs and services. These include independent living, adult day healthcare, community clinic, home health and personal care services, short- and long-term care, hospice care, palliative care, acute psychiatric care, and personalized bridge programs. Today, the Jewish Home is both a leading provider of senior healthcare services for the community and a premier residential care facility. Connections to Care enables all seniors in need to benefit from the Home’s life-sustaining warmth and expertise.

It is the unflagging commitment of supporters, volunteers, staff, and countless others that paves the way for the Jewish Home’s track record of achievement. Your energy, passion, and generous contributions of time and resources have allowed us to sustain our services, to expand our capacity, and to establish groundbreaking initiatives, from Care Transitions and the Brandman Centers for Senior Care to the ongoing development of our remarkable new Gonda Healthy Aging Westside Campus in Playa Vista.

Of course, there is still much work to be done. Meeting the full breadth of seniors’ needs, throughout our community, will require even greater dedication, more collaboration, and a shared vision of where we want to go. The coming year is sure to bring fresh challenges and new opportunities. Working together, we will continue to do exceptional things.

The Los Angeles Jewish Home has been recognized as a national leader in the provision of high quality senior care for more than a century. In 2013, our legacy of distinguished service continued to flourish in ways the Home’s founders could scarcely have imagined.

Today, the Jewish Home has transformed into a comprehensive healthcare system, providing in-your-home, community-based, and in-residence programs and services that enrich life and enhance wellness at every level of independence. And our work has never been more crucial: Seventy-five percent of the seniors we serve rely on government assistance. For them, the Home is an indispensable lifeline – coach, caregiver, comforter, and more.

Foremost among our recent achievements was the launch of Connections to Care, an exciting new approach to meeting the full range of seniors’ healthcare needs. With a single phone call, Connections to Care now allows seniors and their family members – wherever they live – to access all of the Jewish Home’s award-winning programs and services directly.

Two thousand thirteen also witnessed the naming of the Jewish Home as a lead agency for the federal Care Transitions program. Designed to improve the coordination of services to Medicare patients following hospital discharge, Care Transitions is already playing an important role in our ability to reduce hospital re-admissions across our community. And we fostered continued growth at the Brandman Centers for Senior Care, providing support so eligible frail seniors are able to live safely in their own homes while utilizing a wide range of Jewish Home health services.

This year, we worked at the intersection of care and community to redefine “home,” enabling more seniors to leverage our vast resources in convenient and innovative ways.

In 2013, we moved steadily forward in realizing a long-held dream: creating a Jewish Home in West Los Angeles. Located in Playa Vista, the Gonda Healthy Aging Westside Campus continued to take shape; when it opens its doors, it will bring the Home’s signature programs and services to a whole new population of seniors.

The two-and-a-half acre campus will inspire comprehensive health and wellness and enable seniors to age in place. With 175 apartments for independent living in Fountainview at Gonda Westside, a Continuing Care Retirement Community, 24 apartments for assisted living and memory care, and priority access to skilled nursing care – all provided by the Home – the campus will serve as the hub of an extensive network of senior care services and transform our ability to meet growing community needs.

Fountainview at Gonda Westside will feature 18 unique layouts including one-bedrooms, two-bedrooms with media rooms, and penthouses. With a debut planned for 2016, the facility will host a full complement of amenities – from concierge services, dining venues, and fitness facilities to libraries, gardens, and a center for the performing arts. In 2013, we released floor plans and pricing and also began accepting priority deposits.

This year, we built beyond the horizon, channeling the hard work, commitment and generosity of our community of supporters toimagine a new place for seniors to call home.

At the Jewish Home, we join in common purpose to provide seniors in need with quality care, compassionate healing, and cohesive community. In 2013, we pursued this vital mission by advancing our ambitious $215 million campaign: Keeping the Vision Expand, Extend, Sustain.

Our objective: increasing the number of seniors we serve to 5,000 by 2015. With more than $118 million in commitments, in 2013 we laid down a solid foundation to help us meet this goal.

Significant leadership gifts included the Ben B. and Joyce Eisenberg Foundation (Joyce Eisenberg-Keefer Court/Gallery and Joyce Eisenberg-Keefer Sky Terrace on the Gonda Healthy Aging Westside Campus in Playa Vista); Janet and David Polak and family (Polak Family Memory Care Building, which will provide residential memory care on the Grancell Village Campus); Joyce Brandman and The Joyce and Saul Brandman Foundation (Brandman Building on the Gonda Healthy Aging Westside Campus in Playa Vista and The Brandman Centers for Senior Care); Nita* and Robert Hirsch (Hirsch Family Campus in Reseda); Abner and Roslyn Goldstine (Goldstine Family Synagogue on the Grancell Village Campus); and the Goldrich Family (for a special project on the Grancell Village Campus).

Funding from the campaign will also help us as we prepare to:

• Construct new and remodel existing facilities at our Grancell and Eisenberg Villages

• Build the new Hirsch Family Campus in the San Fernando Valley

• Purchase and renovate a 60-bed skilled nursing facility to serve Westside seniors

• Open additional Brandman Centers for Senior Care, the Home’s Program of All-inclusive Care for the Elderly (PACE)

• Build the new Fountainview at the Gonda Healthy Aging Westside Campus in Playa Vista

• Increase capacity of current community-based programs

• Contribute significant capital to the Home’s “Endowment for Extraordinary Care”

• Provide $50 million in annual operating support during the five-year campaign

• Sustain ability to continue all programs and services

This year, we worked to implement a singular vision, moving ahead to create a Jewish Home that digs deeper, reaches further, and stretches toward a new frontier in world-class senior care.

Throughout a century of service, the Jewish Home has built an impressive track record of achievement, proving time and again we are limited only by the scope of our own ambition. Our accomplishments in 2013 continued to highlight the enormous impact of our programs and services on seniors and their loved ones citywide. Consider that we:

Raised over $15 million in unrestricted donations and over $18 million in restricted donations.

Began planning the Gonda Healthy Aging Westside Campus, and initiated pre-sale activities at Fountainview at Gonda Westside, the Home’s new Continuing Care Retirement Community in Playa Vista.

Opened the Home’s first PACE site, Brandman Centers for Senior Care, on the Grancell Campus, in pursuit of our goal: to reach more seniors in need and provide in-home support.

Obtained a contract with the Centers for Medicare and Medicaid Services to launch Care Transitions, enabling the Home to assist 1,320 Medicare patients annually with improved coordination of services and reduced hospital readmissions.

Completed zoning and entitlement work for the Hirsch Family and Grancell Village Campuses.

This year, we channeled big thinking into outsized results, scoring important successes that strengthened the fabric of our community and empowered Los Angeles seniors to thrive.

* 2011 includes retroactive reimbursement of actual costs for one of the Home’s facilities for 2007 through

2010. The facility was opened in 2007. In 2011 the preliminary rate used since opening was adjusted by the Medi-Cal program as a result of their review of the facility’s actual costs during 2007, 2008, 2009 and 2010. The retroactive reimbursement for actual costs incurred during these years totaled $7.9 million.
