SDMNY Intranet Protocol In your SDMNY Training of Facilitators, you learned about the skills you will need for supported decision-making (SDM) facilitation, and the three phases of the process leading up to signing a supported decision-making agreement (SDMA). This protocol lays out the specific steps you will need in order to use the SDMNY intranet to communicate with your mentor about the SDM facilitation process. LOGIN Shortly after your Training, you should receive an email from SDMNY at the address you provided during the Training. The email will request that you activate your user ID that you will need to access the SDMNY intranet. By default, your user ID will be the first letter of your first name followed by your last name (e.g., msmith). SDMNY’s website was developed using WordPress, and as is common for many WordPress sites, these activation requests will be routed to your Spam or Junk folder rather than your Inbox. Log in to the SDMNY intranet. By clicking the “Log In” button in upper right-hand corner of the page, on the top blue navigation bar. After you have successfully logged in, the “Log In” button will appear as “For Staff.” 1

1. LOGINShortly after your Training, you should receive an email from SDMNY at the address you provided during the Training. The email will request that you activate your user ID that you will need to access the SDMNY intranet. 2. USER PAGESBy hovering your mouse over the “For Staff” tab, you will see a dropdown menu. Under “User Pages,” you should add your contact information and a photograph or image after you activate your user ID. The image will appear in the upper left-hand corner of your screen whenever you are logged in. Under “User Pages,” you can also reset your password if you ever need to.

3. MEETING SCHEDULER FORMWhenever you schedule a meeting with a Decision-Maker (Phase 1) or Supporter (Phase 2) or Support Team (Phase 3), visit http://sdmny.org/meeting-scheduler shortly thereafter to log your meeting. Your Mentor will receive an automated notification that you have a meeting scheduled and will then contact you to set up a time for a Facilitator-Mentor Check-In. After your scheduled meeting, you will also receive automated reminders to upload your Facilitator Recording Template and any meeting notes in the SDMNY intranet portal using the Facilitator Form (see below).

4. FACILITATOR FORMAfter every meeting with a Decision-Maker, Supporter, or Support Team, visit http://sdmny.org/facilitator-form. There, you will upload your Facilitator Recording Template for that meeting and fill out the various fields indicating which Decision-Maker you were working with, the date of your meeting, and the phase of the meeting. You can also upload any additional files, such as an SDMA Worksheet or a Big Four Chart and add brief meeting notes. These will be visible to your Mentor and your Mentor will receive an automated notification after you have successfully completed this form.

5. FACILITATOR MATERIALSUnder the “Resource Library” option, you will find an online repository of “Facilitator Materials.” This section of the Resource Library, most of which is available to the public, is available only if you are logged in. Here, you can find many of the same binder materials shared with you at your Training, as you may need to make additional copies or wish to use electronic copies. This folder is accessible at: http://sdmny.org/facilitator-materials.
