LIFTING POCKET CARD

LIFTING CHECKLIST

1. Load

Confirm actual Lift weight including Rigging & accessories Confirm Head height & the crane radius required? Is Centre of Gravity of the load below the Rigging & Crane hook Does load require a lifting beam Is the content of the load secure & the load protected Are Tag lines required

2. Equipment

Is Rigging fit for purpose & recently checked Have all Rigging & Crane Certificates been checked Is the Safe Working Load of Rigging marked available on the equipment Is the SWL of the crane & its mode of operation suitable to complete the lift Does all Lifting accessories eg Chain blocks –come-along shackles meet the

above requirements3. Operators

Has a lifting team been assigned eg Supervisor / Banks man - Rigger /CraneOp – Load Handler

Confirm all members of Lift team are suitably trained & competent toundertake their individual roles

Confirm Crane operators certificate of training is commiserate to the type of crane being used eg Pendant / Remote Control or Cab ( Each require differenttrg courses )

4. Planning

Identify if the lift is Routine or Non Routine Lifting Operation For Non Routine Lifts make sure there is a new or existing Lift plan which

has been completed & reviewed for this operation Make sure Job Risk assessment has been completed and reviewed for this

operations Make sure that Lift team has the Required authority or Permit to Work to

proceed with the lift5. Environment

Confirm ground conditions are free from underground hazard & is strongenough to sustain anticipated loadings

Confirm there are no overhead hang ups or vertical hazards eg OHLE –SteelStructures etc

Check that turntable of mobile cranes are free from any fixed structures whenin normal operation minimum clearance being 750mm

Check there is there adequate light & visibility to complete the lift Check Wind speed is not beyond specific cranes recommendations Ensure suitable physical barriers have been erected to deny unauthorized

access to the lift

6. Communications Has pre lifting meeting being conducted with lift team & all safety

arrangements understood Have primary & secondary means of communication been established and

tested Have Suitable warning signs been erected communicating in an appropriate

language the hazards and exclusion zones7 Lifting Operation

Is the area for lifting load & lifting travel route clear of obstructions &personnel

Is the area for landing the load properly prepared clear & strong enough toaccept the load minimizing risk of load becoming unstable following

Ensure Fall arrest arrangements are in place when rigging & de riggingcomponents that are due to size at an elevated position
