Add a new Service Profile
How to submit a new service profile
Choose “Suggest a new service profile” from the menu on the right of the screen
Ready? Click to update your Service Profile now!
Suggest a New Service Profile.
Complete all the fields that apply to your organization.
If a field does not apply, please leave blank.
Ready? Click to update your Service Profile now!
Executive info will display on the site. Phone number, fax, email will also show.
Alternatively, enter the position title only if you don’t want the name displayed.
The contact name doesn’t have to be entered.
You may enter the position title, or ‘Reception’ for example in the Name field.

Include details regarding the types of services.
Please be as descriptive as possible.
Include if, for example, able to self-refer or if physician referral is needed.

Fields do not appear on the site and are used for database management purposes.
When completed, click on the ‘SUBMIT SERVICE’ button at the bottom of the page.

Enter the name of your organization and press the SEARCH button.
Your organization will appear here (if it has been listed). Click to access and update your service profile.

When you have completed the update of your service profile, please make sure that you include your name, title, organization, phone and email and then click one of the below changes and select SUBMIT SERVICE.
