MISSION
The organization’s purpose is to provide high quality nursing care for persons diagnosed with diabetes
beginning with those determined “pre-diabetic” and continuing throughout stages of the disease. We strive to provide the highest standard of care by offering
personalized education for our patients with opportunities for group sessions.
VISION:
To be the leaders in diabetes education and prevent diabetic
patients in our community from suffering complications of the
disease.
PHILOSOPHY:
Patients
Patients have the right to high-quality diabetes education. Patients have the right to be able to determine what education style
and setting works best for them.
Patients have the right to receive support from family and friends and that such can be present during education sessions.
Patient have the right to receive safe and private care

Nursing
Diabetes is a critical area of nursing practice due to the fact that this disease affects every facet of a patient’s life and can have deadly complications.
The number one goal is to provide the diabetic patient with the means to live a life free from complications from the life altering disease.

The nurse is the one that provides educated care of the diabetic.
The nurse will tailor education to the specific needs and learning style of each patient.
The nurse will keep a safe and protective environment that is patient centered.
Nursing practices must help to support the patient using the realms of education,
research and management.
Nurses Nursing Practice
GOALSShort Term:
Decrease hemoglobin A1c lab level of a point at 3, 6, 9 and 12 months or until level is less than 6 mg/dL
Decrease in weight over 3, 6, 9 and 12 months and or until Body Mass Index is at a desirable level.
Decrease daily blood glucose levels on weekly intervals to get blood glucose levels to normal levels.
Long Term: Assist at least 80% of the
referrals that we receive either in education, supplies, or referral services to other agencies if needed.
To see an overall
improvement at the community level with better control over diabetes in the first year.
OBJECTIVES: To provide diabetes education addressing
appropriate range for blood glucose and minimizing the progression of diabetes within the first visit.
To create an individualized plan of care during the first visit.
To provide any additional resources that a community member may need to achieve success in diabetes control and education.

Nurse Administrator
Job Description
Creates policies, makes schedules and gives evaluation reviews. Must use ethical and legal standards, attend administrative meetings and develop new employee education. Must help to organize, plan, implement and administer clinical programs. Must balance budget and be financially responsible for program.
Education
• Master’s in Nursing Administration – required
Certificates/Licenses
• Current RN license in State of Michigan - required
• BLS certified – required
• Diabetes Education Certificate – preferred
Experience
• 3- 5 years as nurse administrator in outpatient setting

Job Description
Provide education, counseling and disease management to patients throughout the course of their diagnosis. Must be able to work with the Nurse Practitioner assess, design, implement and evaluate outcomes for the education of the patient and family. Provides direct patient care and works collaboratively with team members ranging from board members to volunteers.
Education Requirements
• Bachelor’s Degree – required
• Master’s Degree – preferred
Certification/ Licensure Requirements
• BLS certified – required
• Diabetes Education Certificate – required
• Current State of Michigan RN license
Experience
• 3 or more years of outpatient nursing experience
• 3 or more years teaching and patient education

Job Description
An expert in diabetes management that provides patient care and teaching. The NP must be a leader in the organization. The NP is responsible for the care and education of the diabetic patient through assessment, evidence based practice standards, and outcomes measurements. Provides direct patient care and works collaboratively with team members ranging from board members to volunteers.
Education Requirements

• Certified Diabetes Educator
Certification/ Licensure Requirements
• Current NP license in the State of Michigan
• BLS Certification
Experience Required
• 3 or more years in outpatient diabetes education and management
• 3 or more years of leadership experience

Job DescriptionCoordinate all nutritional care and education in all phases of the diabetic’s diagnosis including assessment, planning, monitoring, and education. Provide nutritional service’s based on patient and medical team’s goals. Evaluate nutritional needs of diabetic patient from diagnosis through life span. Develop educational materials for staff and patient’s. Job Description
Plan, prepare, and teach all courses of meals to diabetic patients. Educate patients on shopping, costs, and recipes. Discuss with patients healthier, cost saving meals that are easily made at home. Educate patients and demonstrate how to cook healthy snacks. Work on a multidisciplinary team specifically with diabetic patients and Registered Dietician.
Education
• Vocational, technical or school of culinary arts - required
Experience
• 2-3 year’s executive chef experience – preferred
• Educational background – preferred
• Experience preparing diabetic dishes

Job Description
Work with both groups and individual diabetic patients to provide education in various exercise genres including but not limited to yoga, aerobics, weight training and cardio exercises. Motivate patients in weight management. Work with an interdisciplinary team to promote health and wellness of the diabetic patient.

• 2 – 3 year’s management of fitness program

Job Description
Under the supervision of an RN, a certified nursing assistant will perform vital signs, perform blood glucose testing, and prepare rooms upon a client arrival.
