Supported living accommodation offers purpose-built accommodation with tailored in-home personal and life skills development and personal care.

NDIS program planning services help our clients get the most from their NDIS funding.

Day services at our centres include personal interest, independent living and social network development, as well as other community support links.

In-home care services provide help around the home, help with shopping, personal development, wellness and clinical services and programs, and personal engagement and leisure services.

Employment and personal growth programs support our clients to prepare for employment by developing skills with travel and transport, budgeting, decision-making and participating in community activities.

Health and wellbeing programsinclude nutritional eating plans, personal training, group exercise, social activities and access to nursing and allied health services.

Behaviour supportis tailored to each client with the aim to develop appropriate ways for them to behave, communicate and interact with their family, peers and the community.

Graceville Centre, Sunshine Coast • Strengthen Our Unique Lives (SOUL) provides support to

people living with an intellectual, cognitive or psychiatric disability, either in Graceville’s accommodation or in their own homes.

• Personal Development and Housing (PDH) provides shared, purpose designed accommodation and support for people living with an intellectual or cognitive disability and/or a psychiatric disability, and who have faced barriers in accessing rental accommodation or maintaining a tenancy.

• Your Life Your Choice is a Queensland government disability support scheme and the Graceville Centre is an endorsed Host Provider.

Keystone Services, Woodridge • Disability Development Program supports people living with

an intellectual disability to live as independently as possible through a combination of day respite and goal oriented self development programs.

• Home Support Services assist people living with a disability to remain at home and continue to develop their independence.

Karawatha Supported Community, Woodridge • The Kawawatha Supported Living Community offers adults

with a disability who are currently living with their ageing parents, the opportunity to make a smooth transition into a new home.
