Assist at all levels of homeless drug addicted persons by connecting with programs available per their specific needs,

Including Veterans, runaways, mentally ill, and everything in between. This organization works with churches

In the area, The General Relief Program, and several drug and detox facilities in the Los Angeles area, including

Hollywood which is our target base (starting at Hollywood and Vine). This is a donation based passion. We also

Work with Prop 47 clients who want their records either eradicated, or lessened whichever is appropriate. Food,

Transportation to treatment, clothing, blankets are the main staple given.

Duties: Interact, assess, assist clients with all matters of recovery including transportation to meetings, and personal

Issues. Contact Counselor when appropriate, document each 30 minutes on every client (19 to date), monitor detox

clients every 15 minutes, witness medication implementation, and assist site manager with additional tasks as

needed. Walk all clients to on site meetings, groups, and counseling sessions. Tally and balance client funds, petty cash,

populate schedule, tabulate medicine count for 19 persons, clean common areas when necessary, interact with all levels

of the company, task duties to Client Advocates, attend scheduled meetings for the week. Drug tested 3-5 times weekly,

and complete room searches.

9/15/14 – 2/28/15 SOBA Recovery, LLC –WLA

RRW/TECH-Overnight

Duties: Transport clients to necessary appointments, groups, meetings, and shopping. Interaction with clients regarding their

Daily routine, feelings, concerns, assist when asked for help on any level of concern to resident, and/or refer them to the proper

person regarding the issue in question. Facilitate positive and nurturing environment by showing support on all levels, including

life skills training, crisis intervention, relapse prevention, suicidality, and alerting our direct supervisor when the last

three issues cause alarm. Complete shift logs, medication count, document and log all issues regarding client. Witness client’s

taking of medication with the client signing off that he/she has taken it. Pick up and drop off of Rx to local pharmacy,

and remind client when he/she is running low on medication. Perimeter checks and room checks are completed every 2 hours

per regulation. Ensure all residents are checked in, and are UA’d every two days. Keep location clean and supplies stocked for

clients, as well as the facility. Other tasks include staging units or rooms for new prospective clients when others have

transitioned from our sober community, and interaction with various team members, and Therapists on behalf of the client

when deemed necessary.

Assisting the homeless with all need based issues from medical to SSI/SSDI applications and processing additional requests on behalf of homeless clients requested by Social Security and other entities assisting to end homelessness. Processing of Low Income, Section 8, and room and board through the SHARE! Program and other affiliated programs and entities.

Locating bed space for those ready to enter rehabilitation for drug and alcohol use. Transporting clients to hospitals, programs, Social security Office, Welfare department, and completing all documents needed for processing. Explaining all benefits available to homeless clients that may enable them to get off the street. Locating relatives of homeless, including adolescents that are homeless and reconnecting those relationships when possible. Identifying dual diagnosed, mentally ill and charting information by completing a psychosocial evaluation that clients agree and sign authorizing their information to be released to appropriate programs/doctors/agencies that may be of assistance
