• HomeTown Health, LLC (HTH) is an organization founded 19 years ago in the SE to pursue ways to help rural and small hospitals survive in this environment of tremendous budget cuts from the state and federal level.

• HTH works hospitals through grant work and education programs nationwide.

• HTHU is the education arm of HomeTown Health started 12 years ago offering accredited continuing education, online courses & certifications, and live training.

• As Project Manager, HomeTown Health's responsibility is to provide comprehensive management and oversight of the State of Iowa’s SHIP Program, such as reporting, budgeting and managing other grant requirements with HRSA.   HTH is also charged with the coordination of group purchases of goods and services that align with the participating hospitals’ designated activities.

The purpose of this program is to help small rural hospitals of less than 50 beds implement quality and operational improvement efforts: • enable the purchase of equipment and/or training to help hospitals attain 

value‐based purchasing provision in the Patient Protection and Affordable Care Act (ACA); 

• aid small rural hospitals in joining or becoming accountable care organizations, or create shared savings programs; &

• enable small rural hospitals to purchase health information technology, equipment, and/or training to comply with meaningful use, ICD‐10 standards, and payment bundling.

The HomeTown Health Hospital Transformation Consortium (HTC) is built around the “Triple Aim” areas of pursuit for improving the health care system:• improving the experience of care, • reducing per capita costs of health care, • and improving the health of populations. 

The two focus areas for the Iowa HTC FY18 will look at putting together the pieces of population health for small or rural hospitals, while maintaining a focus on Financial Stability.

Financial Stability Program OverviewIn the midst of transformation, rural hospitals face the challenge of being successful in our 

current payment system, while also preparing for new value‐based payment systems that are 

being rolled out.  In this environment, we have to:

• maintain a strong financial foundation,

• look for further efficiencies in our revenue cycle, and 

• continue to prepare our financial teams and our clinical teams to better understand one 

another.

The goal of the Financial Stability Program is to provide training, resources and support in order 

to maximize reimbursement in the current system while also preparing for new value‐based

HomeTown Health offers this monthly Medicare Update to help hospitals stay on 

top of the latest changes in state and federal policy. The experts in the field come 

together to share their knowledge and expertise in the world of Medicare. This live 

webinar includes a reimbursement update on the most current Medicare policies 

and procedures. Dale Gibson, Medicare Expert, reviews the most current changes in 

Federal policy and procedures as it relates to filing claims to Medicare.  Intended 

Audience: Medicare Billers, Business Office Managers and CFO's. Webinars held at

Why Population Health?

Population Health Program OverviewRural hospitals face unique challenges in implementing Population Health initiatives ‐ but also have inherentadvantages for program success. Many hospitals have already been pursuing population health to some degree;however, whether an organization is just beginning a program or there is a broad, integrated population healthfocus, the HTC will focus on building out new focus areas, leadership and staff training, and “putting the piecestogether” in order to make the transformation to population health successful in each unique rural community.

Population Health Program OverviewWellness & Disease Management SeriesMary Jane Clark, MS, RN, CHES(r), CHWC®, Grant and Wellness Coordinator, ICAHN

Wellness and Chronic Disease Management programs are the hands of a Population Health Program. For many hospitals developing population health initiatives, beginning a program with your own hospital staff first has been successful, before rolling out a community program. By leveraging data analytics, health plan data, and other resources, we are able to identify health trends within our employee population, build impactful wellness initiatives around these trends, and improve employee health. By building a culture of health and engaging employees in wellness, healthcare costs can be better continued and employee’s report changed lives. Outside of just hospital staff, a community‐based wellness program can help lead sustainable and impactful change, improving the quality of life of the population, as well as the economic standing of the community and community pride.

Unlike other series, the Wellness and Disease Management series content will not be finalized until the submission of the Readiness Assessment, so that we can help participating hospitals in the areas of most need. However, the general outline for training and for training dates is below:

We recommend that each participating "Population Health Puzzle" leader assigned at your hospital meets together as a group to share provide a report of training and projects on a quarterly basis at a minimum.

Other Population Health Program Training & Resources

Quarterly Compliance Officer Training & UpdatesPreviously, the OIG updated its public-facing Work Plan to reflect adjustments once or twice each year. However, the Office of

Inspector General's (OIG) make adjustments throughout the year to meet priorities and to anticipate and respond to emerging

issues. In order to enhance transparency around the continuous work planning efforts, effective June 15, 2017, OIG began to update

its Work Plan website monthly. To meet the need for Compliance Officers and hospitals teams to keep up with these updates, this

new program from HomeTown Health and healthcare Attorney, Michele Madison, partner at Morris, Manning & Martin will be

offered. It will include ongoing quarterly compliance updates on regulations, updates from OIG, case studies, examples of self-audits

to perform, and Q&A. These updates will also be important training for executives and board members. Intended Audience: Rural

Other Population Health Program Training & ResourcesRural Case Management Virtual Book ClubThere is no doubt that case management plays an increasingly important role in the management of quality outcomes of care. Case

managers, and others responsible for helping patients to coordinate the complex environment, help to ensure that patients get the best care

and can help contribute to their hospital’s bottom line. In addition to these opportunities for Case Management, there are also numerous

challenges- many of which are heightened in the rural setting. Case managers find themselves at the forefront of collaborating with multiple

providers, managing discharges and transitions of care, and increased scrutiny in areas of compliance and financial audits. The day-to-day

case management struggles alone can be overwhelming.

The objective of the Case Management Virtual Book Club is to provide ongoing training, support, and an opportunity for collaboration and

sharing of best practices among those involved in case management in a small or rural hospital. Utilizing the Book Club Resource, CMSA

Core Curriculum for Case Management (3rd Edition), the case manager book club will include a discussion and review of a set of book

chapters each month, moderated by Dr. Angie Charlet. Participants will be provided with a textbook, and will be asked to come to the Book

Club ready to discuss the chapter homework each month.

Staff Wellness Support Courses: Hospital Caregiver Trauma StewardshipStaff wellness support courses will be provided by a licensed professional counselor, based upon the theories of Trauma

Stewardship. Often in hospitals, much of the work our staff do in the service of others takes a personal toll on them, their

coworkers, families, and community. Staff morale may be low, they are tired, may be constantly feeling like they can’t do

enough, or cynical. These support courses are meant to help staff personally in their pursuit of wellness, as well as help

to identify how to help others.

Professional Coach Advocate TrainingDesigned for clinicians or those working with wellness programs. It teaches coachability and

is a great starting point for coaching tools within a wellness program. This is the next step in

helping individuals set goals and considering the goals within their environment. This train-

the-trainer class will be offered live at a conference during the grant period, and would benefit

a hospital’s nurse educator, clinic manager, care coordinator, or a wellness committee

member.

Consumer Wellness Advocate (CWA) TrainingThe CWA training is “CPR for wellness,” and designed to help caregivers focus on their own

needs. Covering the “10-Dimensional Prevention through Wellness” model, this Train-the-

Trainer content is a great segway into helping and teaching individuals in the community

about taking care of themselves. Live four hour workshop training will be offered for those

interested in wellness or taking programs out to the community. Participants will receive live

training and materials during a live conference throughout the grant year.

NEW! Using Cost Report to Estimate Service Line Profitability‐ Draffin & TuckerThe goal of this project is to equip hospital leadership with an enhanced understanding of where the hospital is winning and losing financially on the services that are currently being offered using information contained in the most recently filed Medicare hospital cost report in conjunction with patient level detail information obtained from the hospital’s information system. With this fundamental understanding, the  hospital will be able to develop strategies to improve financial performance.

Financial Stability/Telehealth Projects:

Returning: Telemedicine/Paramedicine Project: Direct to HospitalFunding for the individual hospital purchase of equipment installation/useor training related to mobile health equipment and or Community paramedicine.  Allowable expenses include any training and/or equipment installation  that supports the application  and implementation of  telehealth and/or telemedicine.  Technology/hardware investments are allowed if theyare used by staff to improve operational  efficiencies and telehealth services. Also allowable  for expenses related to Community Paramedic Program (CPP) development. If the hospital and/or  hospital‐owned ambulance units has a formal CPP, then equipment can be purchased to support the CPP to reduce emergency medical services (EMS) and emergency department misuse and readmissions. However, use of SHIP funding for general EMS equipment is not allowable. Please note  that other unallowable expenses under the SHIP grant  include, but are not limited to, travel costs, hospital services, hospital staff salaries, or general supplies. 

Amount of funding depends on whether your hospital selects another Quality related  project, or if this is your sole project selection.

Returning: Chronic Care Management ImplementationTraining and Support - ICAHNThe regulatory world surrounding Chronic Care Management has evolved over the last two years for the rural community. With this it has become increasingly difficult to navigate through all the changing opprtunities, and to figure out how to get your CCM program moving forward.Led by a Care Management Specialist, the goal of the program is to provide healthcare facilities with the most up to date information, and to help them identify how to implement a successful CCM program within their current processes. The program will provide education on regulatory requirements, common hurdles and barriers to implementation, care coordination workflows, data and quality improvement impacts, and reimbursement processes and opportunities. At the end of this support and training, facilities will be equipped with the knowledge and the tools to implement a compliant and billable CCM Program.

NEW! Crisis Prevention Intervention (CPI) Training-ICAHNIn healthcare, we might deal with angry, hostile, or noncompliant behavior every day. Our responses to defensive behavior is often the key to avoiding a physical confrontation with someone who has lost control of their behavior. Through CPI Training, at least one person should be trained at your organization who can then train others in the de-escalation of violent patients or family members in the safest, most effective way possible. For small & rural, hospitals, it can be difficult to maintain this standard, so hospitals may also want to consider collaborating with others regionally or within systems to participate in this specific “train the trainer” program. Training will be held over a 4 day class on September25-28, 2018 in Princeton, IL. (Selection of this project allows for a $1,500 scholarship towards the individual certification if another project option is also section: cost to hospitals will be $1,700 out of pocket. Or, select only this project for no out of pocket cost to hospital.)

NEW! Sepsis Certification training - ICAHNWith a focus on Sepsis from Joint Commission, as a part of VBP

investment activities, ICAHN will be offering an online learning

collaborative to share best practices and train on disease specific survey

standards. The audience who would benefit from this training include

Infection control practitioners and middle management. It will also

include one face-to-face or small group meeting throughout the year.

(If your hospital is accredited by The Joint Commission, this it is a part of

the process for earning The Joint Commission's Gold Seal of Approval®

for Sepsis Certification. For hospitals not a part of TJC, this training help

your hospital to build your own sepsis program.)

Communication ConsortiumIn seeking to improve transitions, communication

between rural hospitals and their community nursing

homes is critical. In this group, ICAHN and Kerry

Dunning will provide a foundation of training around

improving transferring care communication. It will also

offer an onsite moderator for your hospital to host

a luncheon or event with area nursing homes to

facilitate conversation, perform strategic planning/SWOTS,

and set goals for improvement and tracking.
