KSRE Nutrition ProgramsEvaluation Decision Tree
The Decision Tree is your guide to selecting the appropriate evaluation instrument(s) to use
based on your program’s audience and the total number of programming hours.
Evaluation Instruments
• For an FNP program of 3 hours or less, only participant totals are required to be entered into K-PICS.
• Tracking responses is optional and primarily for your own use.
Hand Raising Activity
• Use this instrument for a FNP session or series of any length and with any audience.
• Conduct this activity just one time to get a sense of your participants’ nutrition behaviors, or use it at the beginning and end of your series to collect and compare pre- and post-program data.
• To prepare, select up to 5 questions or statements from the question banks that match the audience, content and priorities of the session or series you will be presenting.
Before Program and After Program Feedback Forms
• These are the recommended evaluation instruments for a series of 3 or more total programming hours with children in grades PreK through 6.
• Ask teacher/program leader to complete the Before Program Feedback Form at or before the first session, and plan to have the teacher/program leader complete the After Program Feedback Form at or after the last session.
Shopping Cart and Chef Ally Activities
• These are fun evaluation activities designed for children in grades PreK – 3.
• They are designed to provide you with information about the knowledge the children may have gained from participating in your session or series.
• Tracking responses is optional and primarily for your own use.
Family Nutrition Program Shopping Cart Activity
Instructions: Today we are going to go shopping for foods from the _____________*. Circle the __________* foods that should go in the
shopping cart.
*Fill in the name of a food group and then place 3 pictures/examples of these foods as well as 2 other food items around the shopping cart.

3-5 Pre-Program and Post-Program Surveys
• Reserve these surveys for programs of 3 or more total programming hours.
• To build survey, select 2 – 6 questions from the question bank that match the content and priorities of the series you will be presenting.
• Administer the Pre-Program Survey at the beginning of the first session, and follow up with the Post-Program Survey at the end of the last session.
6-12/Teen Pre-Program and Post-Program Surveys
• These are the recommended evaluation instruments for a series of 3 or more total programming hours with teens (or youth in grades 6-12).
• To build survey, choose 2-8 questions from the question banks that match the content and priorities of the series you will be presenting.
• Administer Pre-Program Survey at the beginning of the first session, and plan to follow up with the Post-Program Survey at the end of the last session.
Adult Pre-Program and Post-Program Surveys
• These are the recommended evaluation instruments for a series of 3 or more total programming hours with adults.
• To build survey, select 2-8 questions from the question banks that match the content and priorities of the series you will be presenting.
• Administer Pre-Program Survey at the beginning of the first session, and follow up with the Post-Program Survey at the end of the last session.
Retrospective Survey
• Use this evaluation instrument with teen or adult participants for an FNP program or series of 3 or more total programming hours.
• To build survey, select 2 - 8 questions from the question bank that match the content and priorities of the series you plan to present.
• Administer this survey at the end of the last session.
Participant Follow-Up Survey
• Use this evaluation instrument to gather follow-up data from teen and adult participants 1-6 months after they have completed an FNP program of 3 or more total programming hours.
• To build interview protocol, select up to 3 questions from the question bank. At least 1 question should align with the FNP objective(s) you emphasized in the program.
Adult with Developmental Disabilities Pre-Program and Post-Program
Surveys• These are the recommended evaluation instruments
for a series of FNP sessions with a total of 3 or more total programming hours for adults with developmental or cognitive disabilities.
• Administer the Pre-Program Survey at the beginning of the first session, and plan to follow-up with Post-Program Survey at the end of the last session.
• To build survey, select 2 - 6 questions from the question bank that match the content and priorities of the series you will be presenting.
