Location of Facility We operate hospitals, health centres and community-based programs throughout Halifax Regional Municipality and the western part of Hants County.

Number and type of Patients/Residents/Clients: 1313 inpatient beds. We provide medical and surgical care, mental health care, community health programs, addiction prevention and treatment, and environmental health services.

MeasuresAt the beginning of the collaborative we were just introducing the Morse Falls Tool so our baseline for documented falls assessment was 0.

Since initiating the tool the data has shown a 97%/98% compliance rate with completing the tools.

Lessons Learned/Key Insights

Place the tool in the admission packs to increase compliance

There is decreased compliance with reassessing patients after a fall or with a change in status

Need to find a better way to capture patients transferred from ICU

When there is high turnover of patients or a lot of changes in condition -the lower the compliance with competing and reassessing patient risk

Continue to reinforce the importance of fall risk and share the information on falls risk – how are we doing during unit education days
