JOB DESCRIPTION
Job Title: Nurse Older People
Remuneration: £26,302 - £30,357 pro rata depending on experience
Responsible to: Nurse Consultant - Older People
Accountable to: Practice Manager–HRBusiness Development Manager – project delivery
Job Purpose Working in the Cathedral Locality Older Persons Team, the nurse is a Registered Nurse responsible for providing a high standard of nursing care which reflects evidence based practice encompasses patient choice and makes best use of valuable resources. This includes assessing, planning and implementing individualised and effective patient care and evaluating the outcome. The post holder will be expected to communicate with members of the internal and external MDT to ensure the best care for patients. In addition the post holder will facilitate training, education and induction programmes for other qualified and unqualified staff.
The post holder works in all care settings (including acute home visits) to undertake telephone and face-to-face assessments of patients with undifferentiated needs, ensuring appropriate clinical prioritisation, diagnosis and clinical management, administering or supplying medicines under patient group directions, administering prescribed treatments and performing clinical procedures without direct supervision.
Cathedral Locality Older Person Team is a joint initiative between three primary care practices; Harcourt Medical Centre, Millstream Medical Centre and Salisbury Medical Practice. They are high quality, high performing organisations which currently delivers primary care services to around 40,000 patients. Directs others in the clinical team to provide care in a safe effective manner in accordance with the appropriate policy and guidance.
Liaise directly with GPs and consultants regarding medicines outside of the individual’s normal sphere of competence and medicine-related uncertainties.
Provide advice and support to Primary Care Nurses, Practitioners and other clinical staff.
Liaise and communicate clinical information verbally and via technical links with Cathedral Locality and health economy partners to ensure continuity of care for the caller.
Maintain accurate, contemporaneous record keeping of all interventions with patients, fully utilising computer software systems in place and/or other mediums.
Maintain confidentiality at all times in accordance with legislative and policy requirements.
Act as the patients advocate when required to.
Participate and assist in service evaluation, clinical audit and research as required.
Participate in the investigation of complaints, incidents and accidents.
Undertake safe clinical practice within health and safety guidelines.
Work within and be aware of the organisations clinical policies and guidelines.
Maintain own awareness of current health policy relevant to practice.
Be aware of the impact your decisions may have upon the patient and their General Practitioner.
Liaise with other professionals to facilitate direct referrals to other health care providers within the policies of the organisation.
Participate in the mentorship of visiting health professionals and students on recognised training programmes, e.g. student nurses, ECP’s, Practice Nurses etc.
Recognise and act upon opportunities through consultation to promote public health.

Job Purpose The Cathedral Wider Primary Care Services is a joint initiative between three GP surgeries; Harcourt Medical Centre, Millstream Medical Centre and Salisbury Medical Practice. As the Locality Educator you will be expected to deliver the Locality strategy in conjunction with the Business Development Manager and provide effective leadership to ensure that the nursing workforce is highly skilled and trained. You will be responsible for overseeing the training of, support and development programme for Health Care Assistants within the Locality and where relevant, other organisations as determined by the Business Development Manager.

Job Title: Research nurse

Job Purpose: Under supervision be responsible for the administration of designated Clinical Trials within Research at Salisbury Medical Practice (SMP) and any associated GP practices as agreed by the Business Development Manager ensuring studies meet requirements with regards to the Department of Health’s Research Governance Framework for Health and Social Care 2nd ed. 2005 and the Medicines for Human Use (Clinical Trials) Amended Regulations 2006.
Under the direction of the Clinical Trials Principal Investigator(s) and Research Lead, the post holder will participate in the active screening and recruitment of patients to research studies. When working on research studies the post-holder will be responsible for the nursing care of study patients and observe for side effects during the course of the study. An important aspect of the role will be the maintenance of accurate and comprehensive records of data derived from the research studies.
The post holder will be involved in ensuring that any research undertaken within SMP and any agreed associated GP practices safeguards the well-being of the patients and is conducted within the International Community on the Harmonisation of Good Clinical Practice Guidelines (ICH GCP) for Research the Medicines for Human Use (Clinical Trials) Amended Regulations 2006 and the Research Governance Framework (2nd ed. 2005). In conjunction with the multidisciplinary team, the post holder will facilitate the production of good quality research
MAIN DUTIES AND RESPONSIBILITIES

Promoting and overseeing the appropriate screening and recruitment of patients to designated clinical trials.

Provide ongoing advice and information to patients and their carers/families with regard to their participation in clinical research in order to facilitate effective informed consent.
Participate in the recruitment and consent of patients to trials; this will require a flexible working pattern.
With supervision assess and evaluate the progress of designated on-going clinical trials and research within the Practice maintaining accurate records of the status of designated studies and providing regular updates to the practice.
Assist other members of the multidisciplinary team with regards to ICH GCP, R&D and REC registration and approval, project development, implementation, completion and dissemination.
Ensure that all ICH GCP and Research Governance Framework required documentation is kept in a clearly auditable system and is stored for the appropriate time.
At all times ensure clear and accurate records, helping with design of databases where required.
With supervision manage own caseload of patients whilst working as part of the multidisciplinary team, planning the care of patients enrolled into designated clinical trials, monitoring and evaluating their progress.
