Introduction

About Southern Health NHS Foundation Trust

Southern Health NHS Foundation (SHFT) Trust was established on 1st April 2011

following the merger of Hampshire Partnerships NHS Foundation Trust and

Hampshire Community Health Care Trust.

SHFT provides specialist mental health services, learning disability services,

community physical health services including community hospitals and specialist

inpatient units, and primary care across Hampshire, and is one of the largest

providers of its kind in the country.

SHFT provides care to around 240,000 people each year, and employs over 6000

people, including doctors, nurses, therapists and support staff.

SHFT is funded by NHS England, local Clinical Commissioning Groups and local

authorities, receiving around £300 million each year. SHFT delivers over 4600

outpatient appointments each week and nearly 1.5 million contacts with patients in

the community each year.

SHFT is part of the Hampshire and Isle of Wight Sustainability & Transformation

Plan. It has an active research department and is a teaching trust, delivering training

and teaching to trainees from a range of clinical backgrounds.

SHFT has five operational divisions, four of which are geographically based and the

fifth, Specialist Services, which includes Learning Disability Services and Secure

Mental Health Services and is the divisional location of this post

An exciting opportunity has arisen for the post of specialty doctor in Forensic

Psychiatry in Ravenswood House. This is a replacement post.

The Specialist Services Division consists of a range of specialist inpatient and

community services for adolescents and adults.

Forensic services are a part of this Division and include both in-patient and out-

patient service provision.

Inpatient forensic services consist of the following:

Ravenswood House – adult medium secure hospital, 79 beds, male only

Southfield – adult low secure hospital, 28 beds, male and female

Ashford – adult low secure hospital for people with learning disabilities, male

only

Bluebird House – adolescent medium secure hospital, 20 beds, male and

female

Austen House – adolescent low secure hospital

Community forensic services include:

Outpatient Personality Disorders & Pathfinder service

Community Forensic Team

Community Forensic Learning Disability Team

Stalking clinic

A new adolescent low secure service is being developed, and there are plans for a

comprehensive re-provision of adult secure services.

Adult Forensic Services

Ravenswood House, Southfield and the Community Forensic Team provide

assessment, care and treatment for adult patients from Hampshire, Dorset and Isle

of Wight, serving a population of 1.3 million people. The two inpatient hospitals

provide treatment and care for individuals, detained under the Mental Health Act,

who present with a range of complex diagnostic and risk related treatment needs.

They accept referrals from hospital and custodial settings.

Ravenswood House medium secure hospital was one of the first regional secure

hospitals in the country, established in 1985 as a result of the recommendations in

the Butler and Glancy Committees. It is located in Fareham.

Southfield low secure rehabilitation hospital, which opened in 2005, has 28 beds and

is located in Tatchbury Mount in Totton. Southfield has both male and female beds

and a strong rehabilitation focus.

Adult forensic services in the Trust are a part of the Thames Valley & Wessex New

Care Model pilot programme along with other regional providers. An important focus

of these pilots is to ensure that individuals receive care close to home where

possible, and this is an area of priority for the service.

Female patients from our catchment area who require medium secure and enhanced

low secure care are placed in Oxfordshire services under the New Care Model pilot

programme.

2. Local working arrangements

The Trust is seeking to appoint a specialty doctor in Forensic Psychiatry to join the

existing consultant and junior doctor team at Ravenswood House. This is a full-time

replacement post.

Ravenswood House has an established multi-disciplinary team, with non-consultant

medical staff, psychology, occupational therapy, social work and nursing and health-

care support work provision. There is provision of primary care and specialist

diabetes input as well as excellent links with secondary care physical health

services.

In addition to having an inpatient caseload, the consultant teams provide community

forensic liaison to local general adult services, for example opinions on risk

management, future possible referrals, post-discharge advice and care pathway

transition. Each consultant team may have approximately 5 community patients at

any one time. Consultant teams also hold a caseload providing in-put to patients

placed in out-of-area beds (low to high security), including reviews for potential

repatriation in-area.

3. Continuing professional development (CPD)

The Trust is committed to supporting development of its staff and is supportive of

mentorship. It is expected that medical staff will become members of a peer group

to review their continuing professional development within the frame work of the

Royal College of Psychiatrists. The post holder is expected to attend relevant

lectures and seminars as part of the internal CPD programme and to engage with

the development programme for SAS doctors within the Trust.

A full range of additional training and development courses and opportunities are

available within the Trust and externally. The Trust also supports incentive for

professional development in mental health and partnership with the University of

Southampton. This provides a diverse programme of high quality multi professional

courses locally involving the participation of national and international experts.

The post holder will be entitled to a maximum of 30 days study leave over a three

year period pro rata. There is an allocated study leave budget for the individual

(£600 per doctor). The post holder will be expected to work within existing

management and leadership structures. Depending on experience, there may be

opportunities for the post holder to be involved in the strategic development of the

services provided by Ravenswood House along with senior team members.

4. Clinical leadership and medical management

The Trust has a management and leadership structure which devolves significant

responsibility and authority, to a local level, through service line management.

5. Appraisal and job planning

Annual appraisal will take place with a trained medical appraiser. This will provide all

the information necessary for the General Medical Council revalidation process. The

post-holder will use the Trust prescribed electronic system for job planning and

appraisal. The Trust has appointed a number of trained appraisers, and specialty

doctors can choose their appraiser from this list.

There is a development programme for all specialty doctors and the post-holder will

be expected to participate in this.

The post-holder will have a comprehensive local induction, in addition to the Trust

induction.

6. Teaching and training

There is significant involvement throughout the Trust in undergraduate medical

education, with staff working in the Specialised Services playing a very active part in

the teaching of medical students from the University of Southampton. At present

students have psychiatry attachments from Southampton University. Each locality

will have a lead and a graduate teaching.

Although the post holder will not have any regular teaching commitments, it is

anticipated they would generally support the ad hoc teaching and education of any

medical students placed within the service.

The post-holder may participate in a wide range of teaching activities. These may

include:

Seminars and clinical teaching to undergraduates especially medical students

Teaching of medical and other staff in psychiatric hospitals and community

teams

Teaching doctors in training

7. Research

Although we are not an academic department, we are from time to time asked to

contribute to research and development within the Trust. Although the post holder

will have no regular research commitment, it is anticipated that they would cooperate

with requests for involvement in research from within the Trust and outside

organisations.

8. Clinical Governance

All staff are expected to contribute to clinical governance. Ravenswood House and

Southfield have clinical governance meetings which are clinically led; these report to

specialised services governance and operational meetings. There is a strong focus

on learning from incidents and embedding quality improvement activity in both

services.

The Trust expects clinical governance to include evidence based practice, risk

management, policy and procedure, user and carer involvement and clinical audit.

9. Clinical duties of post-holder

Clinical duties of the post will include:

1. The post holder will work in conjunction with Trainee Psychiatrists and the

Consultants at Ravenswood House to provide a service for our 79 inpatients.

2. To provide medical management for inpatients on a day to day basis.

3. To provide regular psychiatric reviews of inpatients.

4. To contribute to risk assessment and risk management undertaken by the

Multi-Disciplinary Team.

5. To provide routine psychiatric advice and support to colleagues within the

Multi-Disciplinary Team.

6. To ensure with medical colleagues that cover is provided for Ravenswood

House during normal working hours.

7. It is expected that you will arrange cover for your annual leave with the other

Trainee Doctors and that they in turn will cover your leave.

8. The post holder will ensure that his/her prescribing practice follow Trust

Policies and will ensure that they keep up to date with clinical advances in treatment.

9. Set personal objectives for career development and ensure these tie in with

the other responsibilities of the post.

10. To provide pre-admission assessment and advice for patients who are

referred to our service; and to prepare assessment reports under consultant

supervision

11. To complete reports as necessary, depending on experience, including

Medicolegal Reports, Mental Health Act reports to the Ministry of Justice or the

Mental Health Tribunal etc.

10. General duties

To undertake the administrative duties, including the use of RIO associated

with the care of patients.

To ensure that clinical activity is accurately and comprehensively recorded

and submitted to the Information Department.

To participate in annual appraisal.

To comply with the Trust's agreed policies, procedures, standing orders and

financial instructions.

The post holder will be expected to work with local managers and professional

colleagues in the efficient running of services and will share with consultant

colleagues in the medical contribution to management.

To attend and participate in the academic programme of the Trust, including

lectures and seminars as part of the internal CPD programme

To maintain professional registration with the General Medical Council and to

abide by professional codes of conduct

11. External duties, roles and responsibilities

The Trust actively supports the involvement of the consultant body in regional and

national groups subject to discussion and approval with the medical director and, as

necessary, the chief executive officer.

12. Secretarial support and office facilities

The post holder will have their own desk, access to a secretary and work station,

email and internet access.

13. Support for medical staff

There is a dedicated medical Human Resources team with considerable expertise in

supporting medical staff. This department manages contracts, rotas, sickness,

locums and leads recruitment.

14. Other duties

From time to time it may be necessary for the post-holder to carry out such other

duties as may be assigned, with agreement, by the Trust. It is expected that the

post-holder will not unreasonably withhold agreement to any reasonable proposed

changes that the Trust might make.

15. Work programme

It is envisaged that the post holder will work 10 programmed activities over 5 days.

Following appointment there will be a meeting at no later than three months with the

clinical manager to review and revise the job plan and objectives of the post holder.

16. On-call and cover arrangements

There is no on-call requirement for this post. The post-holder will be expected to

make appropriate cross-cover arrangements in place with the junior medical

colleagues based in Ravenswood House (CT and StR) for annual and study leave.

17. Contract agreement

The post will be covered by the terms and conditions of service for Hospital Medical

and Dental staff (England and Wales) as amended from time to time. Contracts have

been agreed with the BMA local negotiators, but individuals may wish to discuss this

further before acceptance.

18. Leave

Entitlement will be in line with the post-holder’s experience in the NHS. For medical

staff who work part time, annual leave, including Public/Bank Holidays will be

calculated pro-rata.

Relocation Expenses

The successful candidate may be eligible to apply for assistance with removal and

associated expenses in accordance with the Trust’s policy.

Interview Expenses

Public transport rate for expenses will be reimbursed to shortlisted candidates for

costs associated with making a pre-interview visit. Subject to the prior agreement of

the trust, shortlisted candidates who make a second visit may be granted expenses

on this occasion also.

For candidates travelling from abroad, expenses are payable only from the point of
