JCMG SURGEON UTILIZES NEW MARGINPROBE TECHNOLOGY for Advanced Breast Lump Removal SurgeriesJonathan Roberts, M.D., JCMG Surgeon and a member of the American Society of Breast Surgeons, was the first in Missouri to use the state of the art MarginProbe system at SSM Health St. Mary’s Hospital-Jefferson City to remove a cancerous lump from a patient’s breast.

“To safely perform breast conserving surgery for breast cancer, we have to remove the entire tumor and we have to know that it is all out,” Dr. Roberts explained. “Until now, there has been no way to tell if all of the cancer was removed on the day of surgery. Patients have always had to wait several days for the final pathology report. If tumor is present at the edge or margin of the removed lump, the patient needs a second operation. With standard treatment, one in three to one in five patients need a second operation, because the cancer isn’t completely removed the first time. Unexpectedly needing a second operation is frustrating for patients, delays treatment and sometimes results in mastectomy.”

The MarginProbe is a new advanced device used in the operating room to detect cancer not visible to the naked eye, which is on the edge of a lumpectomy. If cancer is present on the margin, it can be removed while the patient is still asleep. Getting it all out the first time avoids a second operation. Dr. Roberts said he has been following the MarginProbe since it

was first developed. Trials abroad and then in the United States showed that its use cut the need for second operations in half, so he was eager to bring this technology to our community. Until now it has only been available in a handful centers in the United States.

“The margin probe really proved its worth in this case,” Dr. Roberts added. “The device not only identified tumor on the edge of the first small lump that was removed, it also showed tumor on the edge of the second specimen. Only after even more tissue was taken was the final margin clear. The final pathology report showed that the MarginProbe was right and tumor was bigger than had been seen on her MRI. In this patient it prevented a second operation and it prevented a third operation. Typically the third operation would have been a mastectomy,” Dr. Roberts explained. “The device saved her breast.”

One of the top priorities in the design of the Operating Room at the new St. Mary’s Hospital was to introduce innovative technologies for surgical patients. “St. Mary’s continues to look for ways to improve the patient care experience. MarginProbe is just one of the latest advances that allows us to offer exceptional care with the most advanced equipment available. We are excited to be the first in the state of Missouri to offer this technology to our patients,” said Mike Hyde, SSM Health St. Mary’s Hospital Director of Surgery.

Dr. Roberts has several cases scheduled in which he’ll be able to use the breast conserving procedure. “The key to good cosmetic results during breast cancer surgery is to get the entire tumor out while removing as little normal tissue as possible,” he said. Dr. Roberts and Dr. Anne Petersen, also a member of the American Society of Breast Surgeons, anticipate using this technology extensively at St. Mary’s for their breast cancer surgery practice.

“Surgeons have been hampered because of the inability to see where the cancer begins and ends in the operating room. The MarginProbe lets us see,” said Dr. Petersen. “Now that this technology is available at St. Mary’s, there will not be guess work with lumpectomy surgery. Our patients will have the best chance to save their breast.”

JCMG and St. Mary’s Hospital-Jefferson City have joined together to provide a complete breast program for patients. They offer 3D mammography at both facilities, the MarginProbe surgery at St. Mary’s Hospital and both medical oncology and radiation oncology at the SSM Health Cancer Center at JCMG.

NEEED PROJECT Cultivating Food and FriendshipBY HALEY GLEASON, JCMG COMMUNITY RELATIONS

Are you familiar with the phrase, “An apple a day keeps the doctor away?” Although this may not always be the case, a variety of fruits and vegetables can help you maintain a healthy lifestyle.

Did you know that there is an effort in our community to help those who don’t have access to healthy options to obtain them? The NEEED Project is a charitable, educational and community service organization. The Project is designed as an opportunity for individuals in all demographic groups to volunteer for the benefit of the community.

The NEEED Project was first incorporated into Jefferson City in January, 2013. The NEEED Project focuses on Nutrition, Energy, Environment and Economic Developments. These key concepts built the foundation to benefit, not only the environment, but people in need and provide informal education for the volunteers.

Volunteers for the NEEED Project perform jobs appropriate for the season. From the beginning to the middle of the season, volunteers plant, water, weed and prune the garden. Toward the

end of the season, volunteers harvest and transport the fruits and vegetables locally. All of the supplies are provided by the NEEED Project.

JCMG has been a supporter of the NEEED Project. Their volunteer gardeners kept a focus on their fruits by nurturing the tomato garden. The volunteers gained valuable knowledge that they were able to put to use in their home gardens.

VEIN DISEASE AND DEEP VENOUS THROMBOSIS

Vein disease is not just about being vain. Many serious consequences can occur if vein disease is left untreated. Each of us is genetically unique and how vein disease will impact us can be very individual.

One potential risk factor associated with vein disease is an increased risk for a deep venous thrombosis. A DVT is a blood clot in the deep veins of your legs. These clots can break free and cause damage somewhere else in your body. When blood pools in your legs, it causes swelling. When blood doesn’t flow freely, it wants to clot. When you treat venous insufficiency, the swelling goes down and there are fewer places for the blood to pool.

The National Institute of Health states that over 50 percent of patients with a DVT have no sign or symptom of the blood clot. It also stated that sometimes the first sign of a DVT is a blood clot in the lungs. This can be a serious or even fatal occurrence.

This was very informative because most doctors, and the public, have the perception that you will always know when you have a DVT. The fact is that most will not have any symptoms. It is important that your DVT risk factors are taken into consideration when making treatment decisions.

When we treat patients with complex vein disease, it is known that the risk for DVT will decrease and leg health will increase. There are so many reasons to be proactive about treating vein disease. Decreasing your risk of a DVT, especially if you have other risk factors, should be an important consideration. Yes, you may look better in a pair of shorts after treatment, but that just is a happy bonus. The many health benefits far outweigh the cosmetic bonus.

JCMG Podiatrists, William Duke, DPM and JP McAleer, DPM, have more than 25 years of combined experience in all aspects of foot and ankle care including surgical and conservative means of treatment. Dr. McAleer completed his residency at Mt. Sinai Hospital in New York. He earned his doctor of podiatric medicine from New York College of Podiatric Medicine.

The podiatrists not only see patients in their JCMG location, located on the lower level of the JCMG Medical Building, but they also work closely with diabetic patients at the wound center located at SSM Health St. Mary’s Hospital. They provide cosmetic services, such as Botox for excessive sweaty feet, dermal fillers and laser fungal nail treatment at JCMG Laser & Vein Center.

“When I started in 2006, my goal was to bring world-class foot and ankle care to the Jefferson City community. Dr. McAleer and I continue to provide the latest treatment options to our patients,” says Dr. Duke.

JCMG WELCOMES PAIN MANAGEMENT & ANESTHESIA COUPLE

JCMG is pleased to welcome a dynamic new couple to the group, Drs. Blythe & Clint Harris.

Blythe (Bante) Harris, MD, has returned to her hometown to establish and oversee anesthesiology services for the group.

Blythe received her medical degree from the University of Missouri-Kansas City. She completed her anesthesia residency at the University of Iowa Hospital and Clinics.

“I’m excited to be back in my hometown. Growing up in Jefferson City, I watched JCMG grow into a medical power house. When the opportunity became available to help transition anesthesia services internally, I thought it was a great time for me to come back to the community where I was raised and give back,” says Blythe.

The couple comes from Des Moines, Iowa, where Clint Harris, M.D., provided chronic and interventional pain medicine services. Clint received his medical degree from the University of Iowa College of Medicine. He completed an internship in general surgery at the University of Texas Health Science

Center in San Antonio. His residency in anesthesia and fellowship in pain management were completed at the University of Iowa.

Clint is board certified by the American Board of Anesthesiology in anesthesiology and pain medicine. He treats chronic pain conditions with a focus on interventional pain medicine. He joins Dr. Karl Haake in JCMG Pain Management.

“Pain management is challenging. Each patient is different. My goal is to create a unique treatment option for each patient, to address the difficulties they’ve been dealing with, and improve their living,” says Clint.

In their spare time, the couple enjoys spending time with their three young sons, and balancing black and gold for both the Hawkeyes and the Tigers.

Dr. Johnson has 14 years experience as a board certified infectious disease specialist. He has been in practice in Columbia since completing his training in 2000. For the past 12 years he served as the Infection Control Committee Chairman at Boone Hospital Center. Dr. Johnson completed medical school and his infectious diseases fellowship at the University of Missouri-Columbia. While in training during 1999 and 2000, he won the Department of Internal Medicine Student Teaching Award “Outstanding Fellow.” He also completed an internal medicine residency at St. John’s Mercy Medical Center in St. Louis. Dr. Johnson enjoys working with patients with complicated infectious disease conditions, especially those with HIV. Dr. Johnson said, “My wife and I are both physicians and we made our home in mid-Missouri over 30 years ago. This is where we raised our children and built our careers and I am very pleased to be able to join Dr. Donald Miller and JCMG so that I can continue to serve my patients and colleagues here at home.”

JCMG is pleased to announce that four area students, pursuing nursing or pre-medicine degrees, were recently awarded scholarships by the group. The JCMG Health Sciences Scholarships were established in 2009 because of the increasing demand for physicians and nurses due to the aging population and increase in life expectancy. Since that time, the group has invested $28,000 in the medical education of area students.

All scholarship winners reside within the JCMG service area. JCMG is very interested in encouraging students to work in the healthcare field and hope that they will return to their hometown after they receive their medical education.

Several JCMG physicians participated in a bill signing ceremony on Thursday, May 7 with Missouri Governor Jay Nixon. Senate Bill 239 marks historic changes in Missouri’s medical malpractice laws.

Cyber crime—we hear it almost every week. A major retailer has announced a data breach or security leak. A unique phishing scam has swept social media. The result is always the same: cyber criminals have compromised individuals’ private personal information, and the hackers plan to use their ill-gotten data to wreak financial havoc on the victims.

These simple, but effective, common precautions to online identity theft are still very important. Change passwords frequently, use letters (both capital and lowercase), numerals and special characters ($,%, #) in your passwords, do not use the same password for all sites, and never respond to an email asking for personal information. Be intentional in taking steps to protect your identity and passwords.

Fifty percent of men will receive a cancer diagnosis sometime in their lifetime. However, men are half as likely as women to visit a doctor for annual exams and preventive services that could catch potential problems when they are most treatable.

June is recognized as Men’s Health Month, sponsored by the Men’s Health Network, to raise awareness of preventable health problems and encourage early detection and treatment. And with Father’s Day coming up June 21, this a great time to remind the men in our lives of how important their health is to us.
