In the morning, there will be a programme on “Mother & Baby FriendlyHospital” Initiative (MBFHI) on 29 January 2017 organized by IMA - TNSB - NHB &UNICEF. In theevening, therewillbeour regularmonthlymeeting, installationofnewofficebearersby the statepresidentanda familydinner. So,we inviteallmembers tokindlyjoinuswithyourfamilytogracethisoccasion.

We request all our members to attend the monthly meeting regularly. Introduction

The UNHSP Program aims to identify all babies with significant permanentbilateralhearinglossby3monthsofageandforthosechildrentobeabletoaccessappropriateinterventionby6monthsofage.Thetestusesquickandsimplehearingtest. The test uses quick and simplemethods to check thehearing of all newbornbabies.

Onetotwobabiesinevery1000arebornwithahearinglossinoneorbothears.Thishearingscreeningtestwillallowthosebabieswhodohaveahearinglosstobeidentified early. Early identification is known to be important for the Speech andlanguagedevelopmentofthechild.Mostbabiesbornwithahearinglossarebornintofamilieswithnohistoryofhearingloss.

Babywillbeofferedthehearingscreeningtestwithinthefirstfewweeksoflife.Thehearingscreeningisusuallydonebeforeyouleavethematernityunit.

EtiologyofHearingLoss

Therearefourtypesofhearingloss:conductive,sensorineural,mixed,andcentral.Morethan50percentofhearingimpairmentinchildrenisthoughttobegeneticandnotrelatedtoinfectious,anatomic,orothernoninheritedcauses.

AnadmissiontotheNeonatalIntensiveCareUnit(NICU)formorethantwodaysincreasesthelikelihoodofthepresenceofhearingimpairment10-fold.Earlyscreeningprogramsfocusedonchildrenwithknownrisks.However,recentstudiesindicatethat19to42percentofprofoundlyhearing-impairedchildrenwillbemissedwithtargeted,riskfactor–basedscreening.

RationaleforUniversalScreening

Screeningforhearinglossinnewbornsisbasedontwoconcepts.First,acriticalperiodexistsforoptimallanguageskillstodevelop,andearlierinterventionproducesbetteroutcomes.Second, treatmentofhearingdefectshasbeenshown to improvecommunication.Data from cohort studies indicate that diagnosis and interventionbeforesixmonthsofagecan improve languageandspeechacquisition inhearing-impairedchildren.

ScreeningTests

Atpresenttherearetwoscreeningtestsavailable.

Theyare:

AutomatedOtoAcousticEmission(AOAE)screeningtest.

AutomatedAuditoryBrainstemResponse(AABR)screeningtest.

AutomatedOtoacousticEmission(AOAE)screeningtest.

This is a simple& most common hearing screening test carried out by anaudiologist.Itisusuallydonewhileyourbabyisasleeporsettled.UsingaportableOAEequipment,asmallsofttippedearpieceisplacedintheouterpartofbaby'searwhichsends clicking sounds down the ear.When an ear receives sound, cochlea usuallyproducesanecho.TheportableOAEscreeningequipmentcanpickupthisresponse.ThisiscalledtheAutomatedOtoAcousticEmission(AOAE)screeningtest.

TheAOAEscreeningtestonlytakesafewminutes.Itdoesnothurtthebabyandisnotuncomfortable.

Theresultswillbegivenimmediatelyafterthescreeningtest.Resultmayshowclearresponsewhichisinterpretedas“pass”ornoclearresponsewhichisinterpretedas“Refer”

A clear response (“pass”)from both ears - This means that baby is unlikelytohaveahearingloss.

ANoclearresponse(“Refer”)fromoneorbothofmybaby'sear-Thismeansbabyneedsasecondhearingscreeningtest(or)DefinitiveDiagnosticHearingTest.

Alotofbabiesneedtohaveasecondhearingscreeningtestbecausethefirstscreendidn't show a clear response from one or both of the baby's ears. This does notnecessarilymean that baby has a hearing loss. Some common reasons, other thanhearingloss,forhavingasecondscreeningtestorDefinitiveDiagnosticHearingtestare:

Babymayhavebeenunsettledatthetimeofscreening;

Theremayhavebeenbackgroundnoisewhenthescreeningtestwascarriedout;or

Babymayhavefluidoratemporaryblockageintheearafterbirth.Thisisvery commonandwillpasswithtime.

DiagnosticAudiologicalAssessment

These group of tests help to confirm the final result. The diagnostic test includesdetailedhearingassessmentwith

ABR(Auditorybrainstemresponse),

DiagnosticOAE,

ImpedanceAudiometry

ASSR(AuditorySteadyStateResponse)

FreeFieldAudiometry,

IfchildisdiagnosedtohavehearinglossinNewbornDiagnosticHearingtest,thenchildwillbereferredforEvaluationandTreatment.Takingintoconsiderationvariousfactors like the degree of hearing loss, specific medical diagnosis and parentalpreferencechildmaybeadvisedforhearingaidsorcochlearimplant.

Abstract

ThelatedetectionofPermanentCongenitalandEarly-onsetHearingLoss

(PCEHL)oftenhassevereeffectsonlinguistic,speech,cognitiveandeducational

development in affected children. Since Newborn Hearing Screening (NHS)

allowsmostPCEHLtobedetectedearlyenoughforoptimalintervention,the

prospectsofitsintroductioninthedevelopingworldarereviewedinthispaper.

It is observed that a simple generalisation on the feasibility ofNHS for the

developing countries seems inappropriate in view of the diversities in the

healthandsocio-economicstatusofthesecountriesandtherecentfavourable

reports of universal newborn hearing screening from the region. NHS

empowers parents to make timely choices that will allow their hearing

impairedchildrentobegivenagoodstartinlifeandbefullyintegratedintothe

wider community. It also compels attention towards the development of

essentialhearinghealthcareservices,besidesthespecificdocumentedbenefits.

Existing child-healthcare structures such as the Expanded Programme on

Immunisation(EPI),BabyFriendlyHospitalInitiatives(BFHI)andIntegrated

Management of Childhood Illness (IMCI) provide opportunities for the

introductionofsomeformofNHSinmanyofthesecountrieswhereroutineor

systematicchildhoodhearingscreeningdoesnotexist.

Limitedfunding,manpowershortages,inadequatesupportservices,low

public awareness and the uncertainty regarding the commitment from

healthcare practitioners may present some challenges but these are not

insurmountable.

Pilotstudiesarenecessaryineachcountrytoprovideempiricaldatathat

willguidehealthcareproviderswhowishtointroducesuchaprogrammeatany

levelofhealthcaredelivery.
