Create an online evaluation (e.g. a course evaluation or survey)
Use an existing questionnaire template
Create a new template from scratch
Modify an existing template
1 Create a questionnaire or select an existing questionnaire
Click ‘Create new questionnaire’ and follow the
prompts to create a new template and add your questions
Click ‘Questionnaire Templates’ to see all existing templates, the you can edit it
after copying the identified template

Assign the survey to a group of recipients
If you’ve got a questionnaire template, click ‘Start new survey’, and then choose a template after adding a survey title and instructions,
On the next page, set the start, closing dates, and notifications, etc.
Once you are happy with all the settings, the next page allows you to assign the survey to the right
respondents (course site participants or an adhoc group)
Select your questionnaire template and configure the settings
