1. IntroductionVISITRAK is a wound measurement systemcomprising three components:

VISITRAK Digital Battery powered digital measuring tablet with stylus.

VISITRAK Grid Single use, 3-layer tracing grid.

VISITRAK Depth Single use, foam tipped, sterile depth indicator.

VISITRAK enables the measurement of wounddimensions (area, length, width and depth)producing a visible record of the dimensionsof a wound. The digital tablet converts a linetracing into a true area measurement and canbe used to calculate the % area change fromthe last area measurement. It is also possibleto measure the % non-viable tissue (or otherregions in the wound).VISITRAK is portable, easy to use and can beused from patient to patient.

Indications for useVISITRAK is indicated for wound areameasurement and can be used on all woundtypes. VISITRAK Digital is indicated for useonly with VISITRAK Grid and VISITRAK Depth.VISITRAK Digital will only function with theattached stylus.

Warnings and Precautions

VISITRAK Digital• Do not use the coversheet within / on a

wound.• Do not use VISITRAK Digital in a wet

environment. Serious electrical damage may occur.

• Do not use VISITRAK Digital in the presence of flammable mixtures.

• Do not take apart VISITRAK Digital.• Do not sterilise VISITRAK Digital. Damage

or destruction to the device may result.• Minimal pressure should be applied when

tracing the wound outline onto the VISITRAK Digital with the stylus.

CAUTION: Misuse of the device will causeinaccurate measurement and potentialdamage.

VISITRAK Grid• Do not fold or crease VISITRAK Grid.• Not all pens are suitable for use on the

tracing grid. Ensure that a permanent marker pen is used.

• Use minimal pressure when using a pen totrace the wound edge.

• Do not touch the surface within the sealedarea of VISITRAK Grid. This surface will come in direct contact with the wound.

• Do not use the product if the seals are opened or damaged.

VISITRAK Depth• Do not bend VISITRAK Depth.• Only insert the foam tipped end into the

wound.• Rest the tip on the wound bed using

minimal pressure. Care should be taken not to depress the wound bed.

How to use VISITRAK – Quick Guide

To get started, slide off the grey compartmentand insert batteries as indicated.

Trace around the edge of the woundmargin onto the grid using a permanentmarker pen. Care must be taken to remainwithin the grid area.

Remove the white backing layer fromVISITRAK Grid.

Whilst holding the coversheet/tracing gridsecurely on the surface of the unit, tracethe wound outline on the grid without liftingthe stylus. Ensure that tracing is started andfinished at the same point.

When processing is complete the devicewill BEEP and display the area measurementin cm2 in the digital display.

Place VISITRAK Depth into the deepestpart of the wound and measure the depthusing the scale.Dispose of VISITRAK Depth as clinicalwaste.

Remove the contaminated wound contactlayer from the tracing grid quickly andcarefully. Dispose of as clinical waste.

Place the tracing grid on the surface of theVISITRAK Digital unit, fitting the holes in thetracing grid over the pegs on the unit surface.If available, place the coversheet on top ofthe tracing grid and secure over the pegs onVISITRAK Digital.

Place the grid area over the wound.Position the top of the tracing grid in thedirection of the patients’ head.

The switch is located under the top-right ridgeof the device. Press this to turn on VISITRAKDigital.

If the wound is too large to fit within thegrid area of one tracing grid, overlap twotracing grids and trace the wound across bothgrids.

Place the stylus at a point on the woundtracing and press the switch on the stylus andwait until the tracing symbol appears in thedisplay.

VISITRAK Grid1. Remove the white backing layer fromVISITRAK Grid (Diagram A)

and place the sterile grid area over thewound (Diagram B).

Position the top of the tracing grid in thedirection of the patient’s head.

2. Trace around the edge of the wound marginonto the grid using a permanent marker pen

(Diagram C). Care must be taken to remainwithin the grid area.

Extra functions:Trace around the perimeter of any viable ornon viable tissue using the same tracing grid.Viable tissue includes areas of epithelialisationor granulation. Non-viable tissue includesnecrotic or sloughy areas.If the wound is too large to fit within the gridarea of one tracing grid, overlap two tracinggrids and trace the wound across both tracinggrids (Diagram D).

(This will produce two separate semicirculartracings whose areas will need to be addedtogether).

3. Remove the contaminated wound contactlayer from the tracing grid by peeling quicklyand carefully. (Diagram E).Dispose of as clinical waste.

4. Complete the patient identification andassessment information on the tracing grid(see ‘Symbols Guide’).

5. Place the tracing grid on the surface of theVISITRAK Digital unit (Diagram F), fitting theholes in the tracing grid over the pegs on theunit surface.

6. If available, place the coversheet on top ofthe tracing grid and secure over the pegs onVISITRAK Digital.

VISITRAK Digital7. To insert the batteries: slide off the greycompartment (Diagram G) and insert thebatteries as indicated.

8. Switch ‘ON’ using the power button locatedunder the top right edge of VISITRAK Digital.

Note: To reset VISITRAK Digital betweenpatients, maintain contact with the ‘C’ button.VISITRAK Digital has a power save function ifthe tablet is not touched for 3 minutes. This isindicated by the display showing the cursormove across the screen.Touch the tracing gridwith the stylus and the unit will reactivate. IfVISITRAK Digital is not touched with the stylusfor 8 minutes, it will automatically power downand will have to be switched on using thepower button.

Calculating a Single Wound Area

9. If available, place the coversheet on top ofthe tracing grid and secure over the pegs onVISITRAK Digital. Hold coversheet/VISITRAKGrid securely on the surface of the unit, placethe stylus at the start point of the tracing.Press the side switch on the stylus (thisshould be done in close proximity to the grid)until the tracking symbol appears on thescreen.

The segments of the display will illuminate insequence indicating that the digital tablet isready to collect and process data.

10. With minimum pressure retrace the woundoutline on the grid without lifting the stylus,starting and finishing at the same point andwithout going outside the grid area. The unitwill BEEP to show that the trace is complete.When processing is complete the device willBEEP and display the area measurement incm2 in the digital display.

Note: If the unit double BEEP’s, an error hasbeen made whilst retracing the wound area.Refer to the Troubleshooting section.

11. Write the area measurement in the ‘area’box on the tracing grid.Touching ‘C’ momentarily at this point willerase the area measurement.

To continue repeat steps 9 and 10.Note: Do not maintain contact with the ‘C’button unless you wish to reset the unit.If the stylus is not in contact with VISITRAKDigital for 3 seconds, the display will autoscroll and show the length and widthmeasurements automatically for 2 secondseach. Selection of the function boxes or ‘C’ willstop the auto scroll. The scrolling function willonly restart after selecting the area, length orwidth boxes.

Displaying Length and Width

12. After the original area has been calculatede.g. 88.9cm2, touch the box under the lengthor width symbol on the tracing grid with thestylus, to display the length or width incentimetres, as required.

13. Write the results in the appropriate boxes

on the tracing grid.Note: It is not possible to display length andwidth following the addition or subtraction ofwound areas or further tracing grids. If thelength or width symbol is touched at this pointthe display will show:

Calculating Area for Larger Wounds (morethan one tracing grid)Note: Calculating the area of a larger woundusing more than one tracing grid must beperformed prior to the calculation of the %non-viable or viable tissue.

14. After the wound area from one tracing gridhas been calculated, touch the ‘+’ button onthe keypad with the stylus.

15. Repeat steps 9 and 10 for the secondtracing grid.When processing is complete, the unit willBEEP and display the accumulated area incm2, for example, assuming that the secondarea was 70cm2, the display will change to:

When multiple tracing grids have been addedtogether, only the wound area can bemeasured so the length and width parameterscannot be displayed.Note: The same process should be followedin order to subtract or add further areas.Touching ‘C’ at this point will return the displayto the previous area measurement, which willflash until the next command is entered. Tocontinue touch the ‘+’ or ‘–‘ button and repeatsteps 9 and 10.

Calculating the Change in Wound Area16. After the wound area has been calculated,use the stylus to select the box under the areachange symbol. The display will change to:

17. Use the keypad on the tracing grid to enterthe previous area.

Touch each number in turn with the stylus,e.g. if the previous area was 127cm2, thenenter ‘1’, ‘2’, ’ 7’, ‘0’. If an error is made, touchthe ‘C’ button momentarily for each incorrectdigit. Re-enter the correct digits.

18. When the area entered is correct, touch‘OK’.

19. Assuming the new area is 88.9cm2 the unitwill then display the % change in area. This

equates to a 30% reduction in wound area.

Calculating the % Non-Viable or % ViableTissue

Note: Calculating the % viable tissue can bedone in exactly the same way as calculatingthe % non-viable tissue, by following the stepsbelow.20. After the wound area has been calculated,touch the box underneath the ‘non-viabletissue’ symbol with the stylus.

21. Repeat steps 9 and 10 for the first area ofnon-viable tissue. When processing iscomplete the unit will BEEP and display thenon-viable tissue area in cm2.

22. If there are additional areas of non-viabletissue, touch the ‘+’ button and repeat steps9 and 10 for each subsequent area of non-viable tissue.

As processing is completed for eachadditional area the unit will BEEP and displaythe accumulated area of non-viable tissue incm2, for example, assuming that the secondarea was 2.7cm2, the display will show:

23. Once all areas have been added, touchthe ‘OK’ button with the stylus. The device willthen display the non-viable tissue as a

percentage of the complete wound, e.g:10.7cm2 of 88.9cm2 = 12%. Touching ‘C’ at anypoint will return the display to the start of thenon-viable tissue procedure (step 20).

VISITRAK Depth24. Place VISITRAK Depth into the deepestpart of the wound using minimal pressure andat right angles to the wound bed andsurrounding skin (Diagram H).

25. Using your thumb, mark where you wouldexpect the skin surface to be and assess thedepth using the scale on the depth indicator.

26. Record the wound depth in the box underthe wound depth symbol.

27. Dispose of VISITRAK Depth as clinicalwaste.

4. Troubleshooting

You notice Possible causes Solution

The function symbol on the tracing grid hasbeen touched but the unit display has notchanged.

Area measurement does not appear in unitdisplay. Unit double BEEP’s during/after tracingis completeand unitdisplayflashesprevious areareading.

No length orwidth resultsavailable.

Incomplete tracing on digital tablet, which maybe due to:• The side switch on the stylus has not beenpressed close enough to the unit surface.• Tracing did not finish in the same place as itwas started.• The stylus has been lifted during tracing orsome point of the line has been repeated.• The stylus/tracing has overlapped the grid area.• The grid slipped on the VISITRAK device surfaceduring tracing of the wound outline.

Multiple tracing grids have been added together.The length and width parameters cannot bedisplayed following these functions.

The digital unit has an automatic power save(sleep) function if not touched after 3 mins withthe stylus.

The batteries have reached a level which isapproaching unacceptable for the unit to function.

Battery power is so low that there is not enoughenergy to power up the unit.

Touch the box underneath the appropriate functionsymbol on the tracing grid using the stylus.Do not press the side switch on the stylus.

Fit the holes in the tracing grid over the pegs on VISITRAKDigital. If available, place the coversheet on top of thetracing grid and secure over the pegs on VISITRAK Digital.Hold the coversheet/tracing grid on VISITRAK Digitalsurface using the unoccupied hand. Press the stylus sideswitch in close proximity to the tracing grid, and placethe stylus at the start point of the tracing to activate thetracking symbol in the display. Trace the wound outlineon the grid, whilst ensuring that minimum pressure isapplied to the grid by the stylus. Do not lift the styluswhile tracing and remain within the grid area. Start andfinish the tracing at the same point. You will hear anaudible BEEP to indicate a successful trace.

For large wound areas requiring multiple tracing grids,only the wound area can be measured.

Touch the tracing grid with the stylus and the unit willreactivate. Proceed with the next function.

New batteries should be purchased andinserted into the unit.

5. Maintenance and StorageReplacing the Stylus Nib• Insert the nib of the stylus into the gap of the metal ring.• Squeeze the metal ring to secure the nib and pull to remove.• Place a new nib directly into the tip of the stylus and press firmly to secure.

Storage• Ensure the VISITRAK Digital stylus is always returned to the unit recess

when not in use.

Cleaning ProcedureClean VISITRAK Digital, stylus and coversheet in accordance with standard infection controlprocedures, or as below:1. Wipe down all device surfaces using a damp soft cloth and a low level

disinfecting solution.2. Dampen another soft cloth with clean water and use this to wipe down all

surfaces to remove the disinfecting solution.3. Dry with a separate soft cloth.Do not immerse any part of VISITRAK or use a dripping wet cloth for cleaning.Do not sterilize the VISITRAK Digital or stylus, damage or destruction may result.Do not use plastic solvents or abrasive cleaners.

CautionCare should be taken to ensure that moisture does not seep into the electrical componentsof the unit or stylus.

The supplier ("Smith & Nephew") warrants, for aperiod of one year from the date of original invoice,that VISITRAK Digital ("the Product") is free fromdefects in materials and workmanship.In the event of a defect in the Product, Smith &Nephew's liability shall be limited to thereplacement of the Product or, at Smith & Nephew'ssole option, repair or refund of the purchase price.Smith & Nephew shall not be liable for any direct orindirect loss or damage (including without limitation,any loss of profit, loss of anticipated business,economic loss, loss of revenue, loss of goodwill, orsuch other similar losses) whatsoever or howsoeverarising, incurred as a result of the purchase, repairor use of the Product.

Any and all other express or impliedrepresentations, warranties or conditions (whetherimplied by statute or otherwise) including, withoutlimitation, any representation, warranty or conditionof satisfactory quality, merchantability, fitness for aparticular purpose or arising from a course ofdealing, usage or trade practice are expresslyexcluded. This warranty is governed by andconstrued in accordance with the laws of Englandand Wales and is subject to the exclusivejurisdiction of the English courts.

Any disassembly (other than required for battery orstylus nib replacement), modifications or repairperformed by persons not authorized by Smith &Nephew or any use of the Product for any purposeother than that for which the Product is designedwill void this warranty.

Ogni e qualsiasi altra dichiarazione, garanzia ocondizione espressa o implicita (sia essa implicitaper legge o altrimenti) includendo, senza limitarvisi,qualsiasi dichiarazione, garanzia o condizione diqualità soddisfacente, commerciabilità, idoneità perun particolare scopo, o derivante da una linea dinegoziazione, impiego o prassi commerciale, èespressamente esclusa. Questa garanzia è regolatada, e interpretata secondo, le leggi dell’Inghilterra edel Galles, ed è soggetta alla giurisdizione esclusivadei tribunali inglesi.

Qualsiasi smontaggio (tranne quanto necessario percambiare la pila o il pennino dello stilo), modifica oriparazione eseguita da persone non autorizzatedalla Smith & Nephew, o qualsiasi utilizzo delProdotto per qualsiasi scopo diverso da quello percui il Prodotto è progettato, invaliderà questagaranzia.

VISITRAK DIGITAL GARANTIEDe leverancier ("Smith & Nephew") garandeert, vooreen periode van een jaar na de oorspronkelijkefactuurdatum, dat VISITRAK Digital ("het Product")geen materiaal- of fabricagefouten bevat.

Indien het Product een fout vertoont, is deaansprakelijkheid van Smith & Nephew beperkt totde vervanging van het Product of (uitsluitend terbeoordeling van Smith & Nephew) reparatie ofteruggave van de aankoopprijs. Smith & Nephew isniet aansprakelijk voor enig directe of indirecteverliezen of schade (inclusief maar niet beperkt totenige bedrijfsschade, verlies van verwachte handel,financiële verliezen, verlies van inkomsten, verliesvan goodwill of soortgelijke verliezen) die in welkehoedanigheid en op welke wijze ook ontstaat alsgevolg van aankoop, reparatie of gebruik van hetProduct.

Enige en alle andere uitdrukkelijke of implicieteweergaven, garanties of voorwaarden (onderverordening of anderszins), inclusief maar nietbeperkt tot enige weergave, garantie of voorwaardevan bevredigende kwaliteit, verkoopbaarheid,geschiktheid voor een bepaald doel of naaraanleiding van een handelmethode, gebruik ofhandelspraktijk worden uitdrukkelijk uitgesloten.Deze garantie is opgesteld volgens de wetten inEngeland en Wales en valt onder de exclusievebevoegdheid van het Engelse gerecht.

Bij enige demontage (anders dan vereist voorvervanging van de batterij of stiftpen), wijzigingen ofreparaties uitgevoerd door daarvoor niet door Smith& Nephew bevoegde personen of enig gebruik voorhet Product voor een ander dan het oorspronkelijkedoel, vervalt deze garantie.
