Who can join Cal MediConnect?Most people who have both full Medicare and full Medi-Cal can join. You can tell if you have Medicare and/or Medi-Cal by the type of cards you have:

Do I have to enroll in Cal MediConnect?No. Enrollment is voluntary. If you do not enroll in Cal MediConnect and choose to stay in regular Medicare, you must choose a Medi-Cal managed care health plan.

You can also join a PACE plan (Program of All-Inclusive Care for the Elderly) if you are 55 years of age or older and need a higher level of care. PACE covers all Medicare and Medi-Cal benefits plus extra services to help seniors who have chronic conditions continue to live at home.

What if I do not do anything?The state will select a Cal MediConnect plan for you.

What are the benefits of Cal MediConnect?• All the services and benefits you currently receive under Medi-

care and Medi-Cal, including Long Term Services and Sup-ports. Long Term Services and Supports include In-Home Supportive Services, Community-Based Adult Services, Multi-purpose Senior Services Program, and Skilled Nursing Facilities.

• Additional benefits include a care manager and a care team of your choice.

• Additional benefits also include transportation and vision.

Can I see my same doctors after I enroll in Cal MediConnect?You need to check with the health plan to determine if your doctors are in the health plan’s network. If you are receiving care from a doctor who is not contracted with your health plan, you may be able to continue that care for up to 6 months (Medicare services) or 12 months (Medi-Cal services). How do I make a choice?You will receive notices from the state telling you how to:

• Select a Cal MediConnect plan

• Choose to stay in regular Medicare and only select a Medi-Cal managed care health plan

• Choose a Program of All-Inclusive Care for the Elderly(PACE), if you are eligible

If you do not make a choice, the state will select a Cal MediCon-nect plan for you.
