Ceramide infused product (CeraPlus) NEW

The CeraPlus skin barrier is designed to maintain adhesive properties, and features a formulation to help:

• Protect the skin’s natural moisture barrier

• Maintain good peristomal skin health

• Decrease transepidermal water loss (TEWL).

Viewing Option window NEW

• The viewing option is utilised by breaking the perforated seal

• Stoma and output can be observed clearly and privately

Karaya 5Nature’s Remarkable Skin Protector

Karaya 5 is a natural skin barrier that is well suited for skin problems or for sensitive skin where reactions may have occurred to other types of skin barriers.

Karaya 5 is based on Karaya gum, a natural product derived from the stericulia urens tree that grows primarily in central and northern India.

Karaya is a safe, natural product that is used as an additive in a number of foods, ice cream, puddings and laxatives.

Note: As a service to the ostomy community and being the only all naturally derived barrier on the market, Hollister will continue to provide the Karaya 5 range to meet the specific needs of those who require this product.

• Protects skin against caustic action of digestive enzymes

• Inhibits bacterial growth and attendant infections and abrasions

• The “buffering action” of karaya protects skin against the caustic effects of digestive enzymes.

• Maintains pH of 4.5 in the presence of acid or base substances

• Neutralises digestive enzymes (pancreatic & bile) found in stoma output

• Maintains skin’s natural defence, ie. ‘acid mantle’ against micro-organisms

• Inhibits/stops bacteria growth because of it’s natural pH of 4.5 and low moisture content

• Easy to use and remove…cleans with soap and water and leaves no residue.

Enterocutaneous Fistula Management

Key Goals for Nursing Management

• Protect the skin – The output from a fistula can be very irritating to the skin. Broken skin contributes to pain and discomfort. A skin barrier can protect the skin from contact with the drainage.

• Contain the output – If the output is minimal, gauze dressings may be sufficient for containment. However, if the volume of the output from the ¬fistula is significant or frequently overwhelms the dressing, a pouch may be more appropriate. Using a pouch can be more efficient and can protect healthcare providers from exposure to bodily fluids.

• Measure the output – With a high output fistula, loss of fluids and electrolytes can be a major concern. Measuring the drainage is easier using a pouching system. Sometimes connecting a pouch to a bedside collector can relieve the stress on the pouch, so the wear time improves.

• Manage costs – Care of fistulas can be costly in terms of supplies and human resources. Using a pouching system and appropriate accessories can be more cost-effective and manageable than frequent dressing changes.

• Control odour – Containment of the drainage in a pouching system, even in small quantities, can manage odour effectively. Other accessory products also can assist with odour control when the pouch is changed or emptied.

• Provide patient comfort – Effectively managing a fistula can contribute to patient dignity. Protecting the skin, collecting the discharge, containing costs, and controlling odour all contribute to patient comfort.

Conditions of Sale and Return of Products

Liberty Medical currently provides Ostomy Associations with free into store delivery and is paying a premium freight delivery service to provide many Associations with overnight delivery. The cost of freight has increased substantially over recent years while returns from the Stoma Appliance Scheme have declined; Liberty Medical can no longer afford to absorb the additional costs.

Expected Delivery time-frames into Associations

• Up to 2 business days : Metro - VIC/ NSW/ ACT/SA

• Up to 3 business days: QLD, SEQ and Victorian regional areas

• Up to 5 business days: WA / TAS / Regional Qld including FNQ

The 2 business day timeframe for Metro VIC/ NSW / ACT/ SA is based on industry standard regardless of warehouse location. It is likely the Associations based in close proximity to Liberty Medical’s warehouses should receive their orders before this with a standard delivery service.

If an Association requires an overnight delivery service this will be available; however the additional premium freight service will be passed onto the Association. A quotation for an overnight delivery service will be provided upon receipt of phone call from the Association into Liberty Customer Service prior to dispatch.

All orders to be received by Liberty Medical before 1pm EST to allow for the above delivery time frames.

*Minimum Ordering

Liberty Medical reserves the rights to request a minimum order value of $300 per order. All orders less than $300 will attract a standard freight charge of $27.50 + GST.

*Excludes all clauses stated under patient deliveries.

Written purchase order for all orders

• Liberty Medical will only accept written orders, either via fax or email. Phone orders will not be accepted.

• Liberty Medical to provide order confirmation.

• All orders to be received by Liberty Medical before 1pm EST to allow for same day processing.

Limit of 10 FIS (free into store) orders per month for Association

• Currently a significant number of Associations are placing daily orders. The cost of placing an unlimited number of orders per month is borne by Liberty Medical and this is not sustainable for the company. It is of primary importance that orders are consolidated to reduce the growing costs of freight, handling and administration.

• If an Association places more than 10 orders per month the additional orders will be billed at full shipping and handling cost. Associations will be advised when nearing 10 orders for the current month.

Please note the 10 Free into Store orders per month excludes new patients and new users of Liberty Medical products. If there are extenuating circumstances this needs to be negotiated on a case by case basis.

Patient deliveries

(a) Association Delivery

- New Patient orders to be delivered by standard freight to the Associations at agreed delivery time-frames without any additional charge.

- If the order is required overnight the Association will be charged the additional premium freight service. A maximum of 1 month’s supply can be ordered. A quotation will be provided upon receipt of phone call from the Association into Liberty Customer Service prior to dispatch.

(b) Direct to Patient (should be the exception)

- When a Direct to Patient delivery is required by either standard freight or overnight, Liberty Medical will charge the Association the additional premium fee. A maximum of 1 month’s supply can be ordered. A quotation will be provided upon receipt of phone call from the Association into Liberty Customer Service prior to dispatch.

(c) Direct to New Patient (for new product listed on Stoma Appliance Scheme)*

- Liberty Medical will waive the direct to new patient delivery fee where the new patient is ordering products which are newly listed on the Stoma Appliance Scheme. New products are defined as those listed on the scheme within the last 6 months. A maximum of 1 months’ supply can be ordered.

Liberty Medical appreciates the importance of new patients receiving their orders as quickly as possible and these orders will be given high priority.

Please note the new patient deliveries are excluded from the 10 Free into Store orders per month.

Change in Prices

Liberty Medical endeavours to advise the trade of fluctuations in prices of products and/or freight. Where possible Liberty Medical will endeavour to provide notice to these fluctuations.

Payment

Accounts are to be settled within 30 days from the date of Invoice.

Revision of Terms of Payment

If Liberty Medical at any time considers that conditions do not warrant delivery of the goods on the terms of payment stated herein, Liberty Medical reserves the right to change the term of payment and/or require payment as a prerequisite of delivery and will notify the customer accordingly.

Handling

Liberty Medical will select the mode and rate for transport of goods for standard orders. When special handling/order is requested the Association will be charged the additional premium freight service, the cost of the special service will be added to the invoice. A quotation will be provided upon receipt of phone call from the Association into Liberty Customer Service prior to dispatch.

Orders

All orders are subject to acceptance of Liberty Medical’s Conditions of Sale. Liberty Medical shall not be liable for any failure to perform the contract of supply of any materials or for any delay or non-delivery due to strikes, fires, explosions, flood, riot, lock outs, injunction, interruption of transportation, accidents, inability to obtain supplies at reasonable prices, war or apprehension of war, force majeure, government action, or any other circumstances beyond Liberty Medical’s control.

Return of Goods

Liberty Medical will accept returns of certain merchandise from the customer under the following conditions:

1. All returns must be arranged through Liberty Medical Customer Service Department at which time an RGA number will be provided. Goods sent without approval will not accepted and will be re-directed back without issue of credit, at sender’s expense.

2. Returns must be made within 14 days from the invoice date. Goods received after 14 days will not be issued with a credit.

3. Only full boxes of merchandise will be accepted for credit.

4. Goods accepted for credit where there has been no fault on the part of Liberty Medical, will be charged a restocking fee by adjustment of credit issued as follows: $25.00 or 25% of value of goods returned, whichever is higher.

5. Any returned stock received damaged will not be accepted and be re-directed back without issue of credit, at sender’s expense.

6. Freight for goods returned due to purchaser error will be at the expense of the purchaser and subject to all the above clauses. The purchaser must organise their own freight back to Liberty Medical warehouses

7. Where “extenuating” circumstances are out of the control of the Association, the Association must notify Liberty Medical in writing. Hollister and Dansac Sales Managers at Liberty Medical will review each case on its merits to determine if waiving of any fees is to apply.

*Liberty Medical will endeavour to complete all credits within 14 days of lodgement with Customer Service.

Non-Returnable Merchandise

1. Sterile Products (except as a result of Supplier shipping error), or by arrangement.

2. Discontinued items.

Product Liability

The product liability of Liberty Medical in relation to any occurrence shall never exceed the limit of Liberty Medical’s current product liability insurance.

Property

(a) Property of the goods shall remain with the Seller and the Seller has clear title to the goods until such time as:

(i) Full payment is made for all amounts owing by the Purchaser to the Seller so that the Purchaser’s total indebtedness to the Seller under these Conditions of Sale is discharged: or

(ii) The Purchaser sells the goods to his customers in the ordinary course of their business.

(b) If the Purchaser fails to pay any amount of the Purchasers indebtedness to the Seller under these Conditions of Sale when it is due to the Seller or, an event of default as specified in subsection (d) hereof occurs the Seller may withhold notice and without prejudice to any of its other rights and remedies recover and/or resell the goods or any of them and may enter upon the Purchasers premises by its servants or agents for that purpose.

(c) Until full payment the Purchaser shall store the goods in a way that clearly manifests the Seller’s title.

(d) Notwithstanding the provisions of these Conditions of Sale payment shall become due immediately upon the Purchaser committing any act of bankruptcy or the Purchaser (being a company) committing any act which entitles any person to apply to wind up the Purchaser, or a Receiver or Receiver and Manager of the Purchaser is appointed.

(e) The Purchaser acknowledges that until his total indebtedness under these Conditions of Sale to the Seller is discharged he holds the good as bailee of the Seller and that a fiduciary relationship exists between them.

(f) In the event of sale of goods to his customers the Purchaser in his position as fiduciary shall:

(i) Assign to the Seller the benefit of any claim against a customer, and

(ii) Account fully to the Seller for the proceeds of the sale (unless and until the Purchaser’s total indebtedness to the Seller under these Conditions of Sale is discharged).

Limitation of Liability

Liberty Medical’s liability for any claim whatsoever in respect of products sold hereunder shall not in any case exceed the price of the product in respect to which the claim is made and customer assumes all risks and liability for the products purchased from Liberty Medical and their use upon and after delivery including their effect upon any person.

Any and all warranties or conditions express, implied or statutory are excluded except those which cannot be lawfully excluded.
