DHS selects Rise to implement new person-centered thinking

Rise was one of four agencies inthe state to be selected to par-ticipate in a yearlong profes-

sional technical assistance trainingprogram designed to implement andintegrate person-centered thinkingprocesses throughout the agency.This groundbreaking demonstrationproject is sponsored by the MinnesotaDepartment of Human Services (DHS)and the University of Minnesota’sResearch and Training Center onCommunity Living at the Institute onCommunity Integration (ICI).

“Rise has espoused person-centeredapproaches for decades,” saidPresident Lynn Noren. “We feel weare now at a pivotal time in our servicedelivery system which requires newthinking and ideas if we are to besuccessful in the future.”

Rise is the only employment/day serv-ices provider chosen for this initiative.The other three are Hennepin County,Mains’l, and Residential Services, Inc.in Duluth. Similar projects have takenplace in ten states, the UnitedKingdom, and Canada.

The basic premise of the program isthat person-centered thinking leads toperson-centered practices which leadto person-centered organizations, andultimately, person-centered systems.

“We envision a system in which peopleare supported in living the lives theywant in their own communities,” Lynnexplained. “At the same time, thedemand for services is increasing andpublic resources cannot possibly keepup. Rise needs to rethink and retoolour service delivery. Using person-centered approaches helps ensure thateveryone’s needs can be met. Thefuture system will require increasedcreativity and partnership with thecommunity at-large.”

“Rather than offering a broad array of

services to all individuals, person-centered approaches are designed tofocus on the specific interests andservice needs of each person,” VicePresident Tim Dickie added. “This is adramatic shift from today, as the poli-cy and regulatory expectations offer asimilar menu for everyone. The shiftwill require new practices and ideas,and Rise is proud to be part of thisinnovative approach.”

The yearlong training and technicalassistance is conducted in three phases by staff at ICI’s Research andTraining Center on CommunityLiving, with training and supportprovided by staff from both ICI andSupport Development Associates.

“We are excited about this uniqueopportunity for Rise and DHS staff towork together,” said Alex Bartolic,director of DHS’s Disability ServicesDivision. “Rise already is a leader inso many ways, and as steps are takento become an even more person-cen-tered organization, we will learn whatsystem changes at the state level areneeded to support these changes.Ultimately, we want to expand thistype of training and technical assis-tance to more organizations.”

A select group of frontline leaders iscurrently undergoing training tobecome “coaches” throughout Risewho will implement the person-cen-

tered thinking tools. The two groupswill meet regularly to share theirlearning. They will work in collabo-ration with the other grant partnersto problem-solve, generate newideas, and share best practicesthroughout the year.

In addition to planning and training,the ICI consultant may also partici-pate in additional activities for con-sumers, their families, and affiliatedsupport groups as the Rise teamrequests.

“We want everyone to be aware ofthe full range of choices available tothem,” Lynn noted, “so that we canmake a more significant transforma-tion to our services.”

“Rise values the partnerships we’veestablished with businesses andother organizations,” Dickie said.“One example is the new partner-ship with the Mounds View YMCA,where we recently opened an adultday service to serve individuals withsignificant disabilities. By partneringwith the YMCA, we can offer peoplehealth and fitness programs, social-ization with community members,and opportunities to grow vital rela-tionships. We are also continuing toassist people find competitiveemployment, and last year, doubledthe number of successful job place-ments.”

“I am very excited to work withRise’s leadership and coaches,” saidMark Olson, an ICI consultant. “Thegroup’s energy around what is possible is infectious.”

Shannon Meehan is working at her“dream job” on staff with theYMCA’s School Age Care, an after-school program at Valentine HillsElementary School in Arden Hills.She loves doing art projects, read-ing, and playing outside with thekids. Rise’s Customized JobDeveloper Sarah Kohl-Leaf assistedShannon in her pursuit of a careerworking with children.

On any given day, people atRise’s production facilitiesin Spring Lake Park and

Coon Rapids will likely be workingon orders for ConductiveContainers, Inc. (CCI). Their prod-ucts are easy to spot as most ofthem have hot pink foam parts thatstand out across the expansivework floors.

Since 1978, CCI has designed andmanufactured thousands of customengineered electro-static discharge(ESD) packages to protect sensitivecomponents for a wide range ofindustries, including high-techelectronics, health care, aerospace,military, and computing. Theirproprietary conductive corrugatedCORSTAT ™ material quicklybecame the “industry standard” forESD safe packaging.

Today, CCI manufactures packageswith ESD foams, vacuum-formedplastic, injection-molded plastic,fluted plastic, and multiple combi-nations of each. Headquartered in

John uses his passion for genealogicalresearch to build a company helping others discover their family’s roots

Since he was 13 years old, JohnLuger has been fascinated byhis family’s lineage. He would

often query family members forinformation to help him constructtheir family tree.

A few years ago, John took his avidinterest to a higher level by start-ing a genealogical research compa-ny called History Hunter to helpothers learn more about their ownfamily roots. Having completed ayear’s worth of courses to earn hima certificate from the NationalGenealogical Society, John is wellon his way to being accredited bythe Board of Certification ofGenealogists.

“I have fun tracing people’s fami-lies for them,” said John. “I amreally good at research and do thebest possible job to get it doneright the first time. I have peoplegive me whatever they have and Itake it from there. Sometimes they

have already started and I can justkeep searching back further forthem; other people give me a fewnames and I basically start fromscratch.”

Letters of recommendation fromhappy customers include com-ments such as, “John’s workexceeded our expectations. Hisresearch was done professionallyand with great enthusiasm. It isevident that John really enjoysgenealogy research.”

Another customer contracted withJohn to research genealogy toshare with family who came fromaround the world to celebrate hergrandmother’s 90th birthday.“John supplied me with so muchmore than that. He was sensitiveto our individual family’s needs and his due diligence showed.”

Much of John’s research is doneonline at his home office in ForestLake, but he also frequentslibraries, county records offices,and the Minnesota HistoricalSociety to gather information.

John’s business has grown mainlyfrom satisfied customers whocome back for more as well as helpspread the word to others. He isclear about his ultimate goal: “Iwant to build this into a multi-mil-lion-dollar business and haveother researchers working for mearound the world.”

In addition to History Hunter,John works about 20 hours a weekwith Advanced Sales andMarketing (ASM) as a productfood and beverage demonstrator.

He started with ASM in March2010, and currently works in theForest Lake Walmart Supercenter.Employment Specialist JillJohnston with Rise’s EmploymentInnovations program in Cambridgeprovided John with career plan-ning and job placement services,and continues to meet with himregularly for follow-up support.

“I love this job – it’s very challeng-ing, but I think I’m good at it,” saidJohn. “I make my display look asattractive as possible so people willwant to stop. I also know all thequalities of the product I amdemonstrating. First impressionsare the ones that stick so I alwaysgive it my best. I am good withpeople and make sure every singlecustomer is thanked and wished agood day.”

Customers respond well to John’senthusiastic yet respectfulapproach; he has one of the highestsample-to-purchase ratios of ASM’sdemonstrators.

Jill has also been an encouragingadvocate of John’s History Hunterenterprise. “It’s exciting for me tosee how he has taken somethinghe’s so passionate about andturned it into a business. I knowJohn is doing what he loves andwill be successful at it.”

To date, John has traced his ownfamily back an astonishing 3,000years! “Yes, they did, in fact, keeprecords even way back then,” hestated.

Rise granted funding to provide six of the way vocational services are offered f

Grill Chef Antoine Simon is ‘inthe zone,’ happily cookingfor a long line of customers

at the Elements Café on the sixthfloor of the Science Museum ofMinnesota in downtown St. Paul.“I love sending guests off satisfiedafter a good meal,” he said proudly.

Growing up cooking with his mother and grandmother, Antoinehas always enjoyed feeding othersand was considered the “designatedcook and grill master” in the family.After being out of school for almost30 years, he was proud to earn anassociate of applied science degreein culinary arts from the Interna-tional Culinary School at the ArtInstitutes International Minnesotain September 2014. He hoped histraining would help him land agreat job in the Twin Cities.

Instead, being an ex-offender,Antoine experienced repeated disappointments which resulted inhis overwhelming frustration, hurt,and depression.

When Antoine’s mental health therapist at Hennepin CountyMental Health Center (HCMHC)asked him if he would like to partic-ipate in Rise’s Individual Placementand Support (IPS) program to helphim find a job in his career field, hejumped at the opportunity.

Based on years of its experience andpositive results, Rise was selectedby the Minnesota Department ofEconomic and EmploymentDevelopment (DEED) to offer six ofthe fifteen IPS employment programs funded in the state. Rise’sIPS programs are offered inHennepin and Anoka Counties, and Central Minnesota.

Lis Borucki Vukelich, a Rise IPSemployment consultant, beganworking with Antoine on his careerplan and job search.

“The beauty of the IPS program isthat it caters individually to theindividual and I was able to cus-tomize Antoine’s job search to meethis goals and aspirations,” said Lis.

IPS programs are closely integratedwith participants’ mental healthtreatment teams. Using a multidisci-plinary team approach, Rise staffwork in collaboration with statevocational rehabilitation counselors,mental health therapists, and othersupport team members to helpensure the individual finds a suit-able job and is successful.

During this process, Lis workedclosely with VocationalRehabilitation Counselor DeannaLangton and Brian Morseth,Antoine’s therapist. The three ofthem communicated on a regularbasis about Antoine’s progressregarding his employment searchand discussed ways they could best support him.

“If it hadn’t been for their support, Iwould have given up a long time agoafter having so many doorsslammed in my face,” said Antoine.“Six different restaurants offered mea job, only to rescind their offersonce my background check cameback my first day on the job. I hadpaid my debt to society, but no onewould believe that I could change,that I would make a great chef intheir kitchen.”

“Rise staff have long recognized thatwork promotes recovery and well-ness for people who have a mentalillness,” said Robert Reedy, directorof vocational services for Rise’sTwin Cities area programs. “Overthe years, Rise has developed a widearray of services to address theirspecific needs in attaining meaning-ful employment.”

“DEED and DHS (Department ofHuman Services) staff monitor theIPS programs closely with quarterly‘fidelity reviews’ to gauge our com-pliance with 125 standards,” saidRise Mental Health CoordinatorAnne Mornes. “Rise’s IPS programshave consistently scored high

because we have been using thesame kinds of evidence-based practices here for many years andbelieve wholeheartedly in theirvalue.”

“Once the person is referred to aRise IPS program by their mentalhealth therapist, our staff uses arapid job search approach,” saidRise Mental Health CoordinatorBecky Rother. “IPS staff also assistpeople obtain personalized, under-standable, and accurate informationabout their Social Security, Medi-caid, and other government pay-ments to help alleviate any concernthey have about losing benefitsbecause they are earning income.”

Ultimately, Antoine was offeredthree great jobs in one week! He andLis discussed the pros and cons ofeach before Antoine decided to takethe position with Elements Café.

“This job has changed my lifearound,” said Antoine. “Through allof my setbacks, I have appreciatedthe support I received to keep pursuing my dream. I also learnedlessons in perseverance andendurance. I plan to pay all this forward.”

“Antoine is a talented man ofintegrity and has a greatwork ethic,” said Lis. “Iknow he will continue to dowell.”

Sandy Shingobe had somebig goals – at the top of thelist were getting a new job,finding a new place to live,and buying a new car. “If you

want it badly enough, you can do it– be determined,” she said withconviction.

After several years working success-fully as a certified medical assistantin a Fairview clinic, Sandy’s careerwas curtailed after two surgeries.When her husband died in 1998,Sandy went into a major depression.She began receiving SSDI (SocialSecurity Disability Insurance) in2004 and worked temp jobs in medical assistance and customerservice to get by.

Sandy’s therapist recommended thatshe attend social and mental healthsupport activities at Bridgeview, adrop-in center in Fridley for adultsliving with serious and persistentmental illness. Julie Smith, a mentalhealth therapist from Bridgeview,referred her to Rise EmploymentConsultant NaTasha Olson whoworks with the IPS projects.

Highly motivated, Sandy applied forand took assessment tests on herown to work at Target; she washired in June 2014. With a steadypaycheck now, Sandy was able to

move into a larger apartment withassistance from Anoka CountyCommunity Action Program(ACCAP) last October. She wasthrilled to purchase a new car.

“I really enjoy my cashiering job atTarget and sometimes get to work intheir Minute Clinic giving shots andtaking blood pressures,” said Sandy.“Everyone I work with is very niceand it feels really good to be produc-tive again. Time will tell if I go backto work in the medical field. I tellmy friends at Bridgeview that work-ing has really helped me in so manyways. People can do anything theyset their minds to and if you don’tsucceed at first, just try and tryagain.”

LaNay Koralesky from MinnesotaVocational Rehabilitation Servicesand NaTasha worked to help Sandramaintain her current benefits.

“Paperwork from the governmentcan be confusing,” said Sandy. “Itreally helps me to have NaTashagive me advice and guidance, espe-cially about my benefits.”

For now, both Antoine and Sandywill continue to meet with Lis andNaTasha on a weekly basis to helpensure they continue to be success-ful on the job. Gradually, Rise staffwill reduce their support.

All of Rise’s IPS programs have people on a waiting list for services.“We’re hoping the state will increasefunding for these services which willenable us to assist those who reallywant to get back into the workworld,” said Anne. “We are con-vinced that using this approach willgreatly enhance the employmentservices Rise offers people withmental health disabilities and ulti-mately, lead to prolonged success.”

Sandy says that her job atTarget has helped her in somany aspects of her life.

With a promise tocustomers to ship ordersplaced by 5 p.m. the same

day, employees at MagnetoPower’s wholesale warehouse anddistribution facility in Anoka are incontinual, synchronized motion asthey fill and ship orders formanufacturers, servicing dealers,and retailers throughout NorthAmerica.

At the end of a long line of otherpackagers, Brittany Alt-Willardaccepts a bin of assorted productsthat have been “picked” fromMagneto’s stock of 40,000 outdoorpower equipment and replacementparts, scans each piece to verifythem against the original order,and packages it all up for shipping.

Brittany is steadfast in her work,paying careful attention to thedetails of every order. She and herco-workers are meticulous in theirwork and are proud of Magneto’s99.5% accuracy shipping rate.

Brittany originally came toMagneto as part of a temporaryRise work team for a specificshort-term project. OperationsManager Craig Thompson said shewas a stand-out from day one andasked her to stay on working afterthe original project was completed.

Sue Featherly provided follow-upsupport services through Rise’sCommunity-Based Training andEmployment (CBTE) program,meeting with her twice a month forthe past four years.

Her work attitude, focus, and relia-

bility were what prompted Craig tohire Brittany in May.

“Brittany has done an outstandingjob in every process in ourwarehouse,” Craig said. “She hasalways been eager to learn newtasks, and with positive feedback,has really grown in all aspects ofthe job. She is a joy to have hereand fits in well with everyone.”

“I love everything about my job,”said Brittany. “It’s really busy, butI have nice co-workers and we helpeach other out. I was very excitedto get hired as a Magneto employ-ee and am proud of my work.”

“Brittany has continued to expandher skills and self-confidence inthe past three years,” said Sue.“She has made a lot of changes inher life because of this job and Iknow how much it means to her tonow be a Magneto employee. I amso proud of her.”

Prior to being hired by Magneto,Sue helped Brittany with benefitsplanning so that she would still beeligible for Social Securitybenefits, enabling her to live semi-independently with some sup-ports. Craig also submitted a letterto the Social SecurityAdministration in support.

Brittany enjoys living in her ownapartment in Coon Rapids andtakes the bus to work. One of herco-workers, though, is happy topick her up on rainy or cold days.

“I’m tired at the end of the dayfrom being so busy, but it’s a goodtired,” Brittany admitted. “I hopeto work here at Magneto for a longtime.”

Minnesota Consortium forCitizens with Disabilities, MaryRichards is proud that her advo-cating efforts to help repeal theMedical Assistance for EmployedPersons with Disabilities(MA-EPD) premium increase havebeen successful!

In her testimony before theMinnesota State Legislature thisspring, Mary told representatives:“I live independently in my condoin St. Anthony Village. I have livedwith lifelong disabilities from abrain injury since birth that challenge me every day. My braininjury has caused learning disabil-ities, memory issues, depression,and balance issues. Despite this, Isuccessfully graduated from highschool, earned an associate’sdegree at college, and became cer-tified as a licensed practical nurse.

“I have held a variety of jobs over

the years. In 2006, I was unable tocontinue working following surgery. As a result, I had to go onMedical Assistance (MA) with alarge spend-down ($349 permonth).

“Today, I am proud to be workingagain at Rise Data Ability doingdata entry part-time so I enrolledin the MA-EPD. This programallows me to work part-time andkeep my eligibility for MA. I stillhave very little income, butinstead of paying a large spend-

down, I now pay a premium.”

But Mary said she was shockedwhen her premium almost tripledto $128 a month. “When you arealready living on the edge like Iam, it makes a big difference.”

She was also one of 26 people fea-tured in a Faces of Disabilityexhibit which hung outsideMinnesota legislators' offices in St.Paul.

Smith and Minske elected to Rise Board of Directors

The Rise Board of Directors elected two newcommunity volunteers on March 18 to servethree-year terms.

Rachael M. Smith (right) lives in Brooklyn Center andworks for both the Osseo Area School District as aneducational support para-professional and for Anoka-Hennepin School District #11 as a childcare assistant.A graduate of Hennepin Technical College, Rachaelenjoys volunteering at St. Vincent DePaul Church andwith the Pageant of Hope, a national pageant provid-ing opportunities for girls with special challenges,which recognized her with the Mentor Award. Rachaelis also a Special Olympic athlete, competing in basket-ball, bocce ball, and golf.

“I am very excited to be a member of the Rise Boardof Directors,” said Rachael. “I look forward to learningnew things and participating in another excitingadventure. With hard work and determination, I havebeen able to earn a degree and achieve my career goalof working with children. I am excited to work with anorganization that helps people achieve their goals.”

Sheila Minske, Minneapolis, is a senior Value Streammanager for Medtronic in Brooklyn Center. She firstpartnered with Rise in 2011 when she was instrumen-tal in securing a grant from the Cummins Foundationto support Rise’s Art Speaks program at thePowderhorn Art Fair in Minneapolis. Her interest inRise continued to expand and in 2014, when Sheilawas the director of Manufacturing Engineering andnew Production Introduction at Cummins PowerGeneration, she helped Rise launch its first product,

an adaptive switch to help people with disabilities activate electronic devices.

Sheila credits her father for inspiring her with his pas-sion for helping people with disabilities. During col-lege, she worked as a job coach at Goodwill Industries.Sheila takes great pride in her senior design project atthe University of Toledo which automated a hot/coldwater dispenser for people with limited upper bodymobility, enabling them to be more independent. Shehas volunteered to modify kids’ toys with adaptiveswitches for Courage Kenny for the past eight years.Sheila and her wife live in Minneapolis with theirthree-year-old twins.
