can easily read both in OPD & while doing sonography. Section 19 ( 4)

Be 100% safe & secure wear dress code e.g. white apron with your Name & degree

on black plate to avoid unnecessary harassment as in Rajasthan(Ganganagar 12 drs

clinic Usg machine were sealed) rule 18 (8)

Before doing sonography first fill up FORM-F with full (in detail and declaration by

patient and person doing sonography) signature of patient then & then do

sonography (if accidently diagnosed pregnancy while doing sonography for abdomen

or pelvis) you fill form f before patient leaves your clinic.

Always maintain Pndt register 9(1) 1—serial no 2—name & address of men &

woman 3- name of husband/ father 4—date on which they first reported for

counseling, procedure& test

DO IN PCPNDTACT Maintain & preserve of all record (form f, consent form, sonography plate. slides,

referral chits) for minimum period of 2yrs( in event of any legal proceeding is pending against you then one has preserved all records till final disposal of legal proceeding) Rule 9 ( 6 )

Print FORM-F must be as per provisions of the Act1994 & New Notification on 31st January 2014 one should fill all Section A, B, C, D,. To avoid unnecessary conflict & breach of law.

In case of Non use of machine. Change of owner ship-management, cessation of function immediately surrender PNDT registration certificate to appropriate authority Rule 6 ( 6)

Keep at least one copy of PNDT BARE ACT in local/English language in waiting room rule 17 (2).

Always inform authority before 30 days in advance about any changes by you regarding employs, place, and add of new sonography machine company brand & no of machine.( IF YOUR CLINIC UNDER RENOVATION & TEMPORARY(6TO 15MONTHS) YOU SHIFTED YOUR ALL MACHINE & STAFF IN DIFFERENT AREA &PLACEBUT YOUR NAME MUST BE AS A OWNER ORIN RENT AT THAT PLACE NO NEED OF SEPARATE OR NEW PNDT REGISTRATION REQUIRED) RULE 13

Renewal date of registration should always be remembered & do application of renewal before 30 days to Appropriate authority office in 2 letter pad (Form A) with cheque of one half of fee ( e.g Rs 12500 or 17500)with seal& signature of govt. staff on receiving copy(pl have acknowledgement of the same from authority) if no answer from A A. in 90 days of application your registration is consider as deemed renewed so nothing to worry.

Always sent FORM-F monthly report to Government authority in pndt office before 5th of next month without any fail. Rule 9 ( 8 )

Always co-operate Government authority whenever they visit for inspection at your clinic .

Never become panic & excited if notice is issued against but be careful before giving signature or commission of offence on inspection report take time to consult your medico legal advisor about it.

DO IN PCPNDTACT If sealing of your machine took place Demand Xerox copy of all record

Seize by A.A. with valid signature of Govt authority and select intelligent medico legal lawyer for further proceeding.

Always give answer of show cause notice if issue by Authority relating to violation of pcpndtact in given time to government authority as prescribed in notice ( e.g. 7 0r 10 days)

Now in a computer day (As we give prescription of medicine & treatment in printed form) if one has to make habit of maintain records on computer or other electronic equipment, a printed copy of the record shall be taken & preserved after authentication by a person responsible for such record RULE 9 (7)

In Vitro Fertilization (IVF) centres, ART (Assisted Reproductive Technologies) and Magnetic Resonance Imaging (MRI) machines would also be registered under Pre Natal Diagnostic Techniques (PNDT) Act.

Always write his/ her name & designation in full under his/ her signature in FORM-F-- rule 18(9)
