THE VIAGRA EFFECT

Widespread use of Viagra has opened the lines of communication between men and their physicians

Discussion of ED treatment lay lead to larger discovery of underlying health concerns

ED patients are lucky. Huge link to metabolic syndrome/cardiovascular health

ED prompts men to seek treatment Treatment can SAVE LIVES

Copyright 2016, Laura Neville ND, LLC

GRAVE CONSEQUENCES Low T can lead to:

Insulin resistanceDiabetesHigh blood pressureHeart DiseaseElevated TG and CholesterolDepression

Copyright 2016, Laura Neville ND, LLC

METABOLIC SYNDROME Metabolic syndrome is a cluster of at

least three of five of the following medical conditions: abdominal (central) obesity elevated blood pressureelevated fasting plasma

glucose/elevated insulin levelshigh serum triglycerides low high-density lipoprotein (HDL) levels

Metabolic syndrome is associated with the risk of developing cardiovascular disease and diabetes.

LOW T + HIGH ESTROGEN Estradiol is a growth hormone and can

cause enlargement of prostate cells in men

Prostate hyperplasia “BPH” High estrogen causes SHBG to rise=

even less free (active) T available. Vicious Cycle!

AROMATIZATION Testosterone

Aromatase

Estradiol

**Fat cells are superb at this!!

SEX HORMONES: FREE VS. BOUND

Fat Loving Dissolve in oil and fat, but not in a

water-like environment such as blood Proteins (such as SHBG) are needed to

“carry” the hormones around in the blood

When “carried” they are bound to proteins and are inactive

3-5% of hormones are free/unbound 95-97% are bound/inactive and act as a

reservoir

T TO THE RESCUSE Raising T levels can stop the cycle

and treat metabolic syndrome Can turn insulin resistance around!

“Testosterone replacement therapy improves insulin resistance, glycemic control, visceral adiposity and hypercholesterolemia in hypogonadal men with type II diabetes”

Kappor D et al. Euro J Endocrin 2006;154: 899-906

PROSTATE CANCER Testosterone DOES NOT CAUSE prostate

cancerResearch out of Harvard over last several

years has shown that testosterone is not the cause of prostate cancer…but elevated estrogen may be!

ESTROGEN DOMINANCESUSAN (54 YEARS OLD)

“I have never had a weight problem, until now! I have always exercised regularly, eaten a pretty healthy diet, but now I’m gaining weight, especially around my middle…where is this belly coming from!?

“I have even been exercising more and eating less to stop the

gain. Nothing is helping. The pounds seem here to stay”

PROGESTERONEBALANCES ESTROGEN

Copyright 2016, Laura Neville ND, LLC

HORMONE CYCLES

Copyright 2016, Laura Neville ND, LLC

PROGESTERONE Produced by the ovaries. Surge in

second half of the cycle, after ovulation Small amount produced by the adrenal

glands Drug companies make a synthetic

hormone called progestin PROGESTINS ARE NOT PROGESTERONE

Different molecular structureThus vastly different effects in the bodyMany negative side effects

Copyright 2016, Laura Neville ND, LLC

PROGESTERONE Promotes sleep Calming Reduces

cholesterol Helps estrogen Increases scalp

hair Burns fat for

energy Relieves

depression Diuretic

Reduces artery spasms

Maintains pregnancy

Balances estrogen effects on breast, brain, uterus

Promotes normal cell death

Helps body utilize glucose/improve insulin resistance

Helps thyroid gland

WOMEN’S SYMPTOM CHECKLISTLOW ESTROGEN

Hot Flashes Night Sweats Vaginal Dryness Incontinence

**Not as common as one would assume

LOW ESTROGENJANICE (49 YEARS OLD)

“I’m not myself anymore. I wake up 3-4 times per night, drenched in sweat. In the morning, I feel more tired than when I went to bed. At work, I have even had to leave a business meeting because I was so uncomfortable and embarrassed. You can actually see the hot flashes; my face turns beet red!”

MENOPAUSE Defined as cessation of menses for 12 consecutive

months Menopause is a gradual transition- may take up to 10

years to occur. Transitional period is termed peri-menopause Progesterone levels decline significantly Estrogen levels change as well, though rarely decline as

the same rate as progesterone Leads to an imbalance= estrogen dominance Risk factor for estrogen-dependent cancers,

osteoporosis, dementia and CVD DHEA, testosterone, and cortisol levels may also

fluctuate, compounding the symptoms picture Belly fat, scalp hair loss, weight gain, low libido, burn

out, rapid aging.

ADRENAL DISORDER

Copyright 2016, Laura Neville ND, LLC

ADRENAL DISORDER The cells in the body are constantly

breaking down and repairing. A large portion of the repair process

occurs during sleep Sleep deprivation/stress can age the

body very quickly. Repair is not taking place!

WOMEN AND TESTOSTERONE

Testosterone made in ovaries and adrenal glands

Women have 9 times less T than men Still very vital part of health Improves sleep Improves sense of well being and vitality Improves bone density Helps burn fat/slows fat build up

Copyright 2016, Laura Neville ND, LLC

BREAST HEALTH

Estrogen and progesterone impact breast tissue Estrogen= cell growth and breast development Progesterone= balances the growth effect of

estrogen Imbalance= breast pain, cysts/lumps, fibrocystic

changes, tenderness, change in breast size

Copyright 2016, Laura Neville ND, LLC

ESTROGEN E1= Estrone- 50-70% less active than E2. Made

mainly in fat cells. Highest after menopause E2= Estradiol- strongest. Made mainly in ovaries E3= Estriol- the most protective form of

estrogen. Only 10% as active as E2. Cannot be converted to E2 or E1

Dr. Henry Lemmon researched breast cancer patients and discovered that they had low levels of E3

EQ= E3/E1 + E2

Copyright 2016, Laura Neville ND, LLC

SALIVA VS. SERUM Saliva testing= easy, measures

free/unbound portion of hormones. Represents the active hormone that is

available to tissues

Copyright 2016, Laura Neville ND, LLC

DHEA Made in both ovaries and adrenal glands Important for libido and arousal Promotes sense of well being and

motivation Improves immune function Improves REM sleep Improves memory

Copyright 2016, Laura Neville ND, LLC

WHY? High stress lives Much different pattern than in past days Adrenal glands and ovaries struggle to

produce the necessary hormones Estrogenic world Xenoestrogens Plastics- BPA (endocrine disruptors)

Copyright 2016, Laura Neville ND, LLC

TEST THEN TREAT Sacred selfishness- take care of yourself first!

You cannot pour from an empty cup. Proactive action Make time for enjoyable activities Feed you body whole foods Deep belly breathing- 100 per day Exercise- at least 20 mins, 3 X per week Water Intake: ½ body weight in ounces per

day (unless kidney disease or congestive heart failure)

Optimize sleep! So important Laugh BHRT
