    This atlas is a series of photographs ranging from low to high magni cations of the indi -vidual tissue specimens. The low magni cation images should be used for orientation,while the higher magni cation images show details of cells, tissues, and organs. Al -though every effort has been made to faithfully reproduce the colors of the tissues, a full

    appreciation of histological structure is best achieved by examining the original speci -mens with a microscope. This atlas is a preview of what should be observed.

    The photomicrographs found in this atlas come from the collection of microscope slideused by medical, dental and undergraduate students of histology at the University ofMinnesota. Most of these slides were prepared by Anna-Mary Carpenter M.D., Ph.D.during her tenure as Professor in the Department of Anatomy (University of MinnesotaMedical School).

    Each tissue specimen, in its entirety, has been digitized with a high resolution 40Xor 60X lens to generate virtual microscope slides. The Virtual Microscope Collection

    includes additional slides which complement and extend the core slide collection. Pro -ducing the virtual slide collection and developing the web site for their presentation wasdone with the very capable assistance of Todd C. Brelje Ph.D.

    The drawings that appear in the atlas are the product of Jean E. Magney, who is ac -complished both as an histologist and an artist.     The gastrointestinal tract is a hollow muscular tubethat starts at the esophagus and ends with theanus. It is divided into four regions, the esopha-gus, stomach, small intestine and large intestine.The esophagus is a passage for transporting foodto the stomach. The stomach adds gastric juicesto begin digestion. It is divided into three histologicregions: cardiac, fundus/body and pyloric. Thesmall intestine is the principle site for digestion andabsorption. It transfers chyme from the stomach tothe large intestine and is divided into three regions:duodenum, jejunum and ileum. The large intestinehas the main function of re-absorbing water fromthe chyme and adding mucus to facilitate transportof the feces. The parts of the large intestine are thececum, appendix, colon, rectum and anal canal.

    G ENERAL P LAN FOR H OLLOW T UBULAR O RGANS

    The walls of hollow organs have four layers or tu-nics: mucosa, submucosa, muscularis externa andadventitia or serosa.

    Mucosa (mucous membrane): Mucous mem-branes line internal passages and provide a barrierbetween the tissues of the body and the externalenvironment. The membranes are constantly wetand lubricated by mucus. The mucosa has threeparts: an epithelium, lamina propria and muscu-laris mucosa. The epithelium varies in differentregions depending on its function (i.e. protective,secretory or absorptive). The lamina propria isa connective tissue layer that supports the epithe-lium and contains small arteries, veins, lymphaticsand nerves. Lymphocytes and plasma cells arealso frequently seen in this layer. When glands arefound in this layer they are referred to as mucosalglands. The muscularis mucosa , when present,consists of two or three layers of smooth muscle. Itfacilitates localized movement of the mucous mem-brane, aiding expression of secretions and move-ment of uid across the surface of the epithelium.

    Submucosa: The submucosa is a layer of -broelastic connective tissue that supports the mu-cosa. Found in this layer are blood and lymphaticvessels and nerves. Parasympathetic gangliafound in this layer are called Meissners submu-cosal plexus . When glands are found in this re-gion (esophagus and duodenum) they are referredto as sub-mucosal glands.

    Muscularis externa: This is a separate layer notto be confused with muscularis mucosa. Themuscularis externa consist of two thick layers ofsmooth muscle and inner circular layer and anouter longitudinal layer. Between the layers is a

    vascular plexus and an autonomic nerve plexusassociated with small parasympathetic ganglia of(Auerbachs) myenteric plexus . The muscularisexterna maintains tonus in the tube and propels lu-minal contents by peristalsis.

    Adventitia or serosa : This outermost layer isdense irregular connective tissue. When it blendswith connective tissue of the surrounding area it isan adventitia . If it has a free surface projectinginto the peritoneal cavity it is covered with a singlelayer of mesothelial cells (epithelial cells derivedfrom mesoderm) and is called a serosa .

    E SOPHAGUS

    The epithelium is strati ed squamous and non-ker -atinized. This is a thick layer of 40-60 cells mea-suring 300-500 um. This is supported by a laminapropria . A well developed muscularis mucosa ispresent (200-300 um) and surrounded by the sub-mucosal region. Submucosal mucous glands arescattered in this region. The muscularis externa consists of an inner circular layer and an outer lon-gitudinal layer. In the upper third of the esophagusthe muscularis is skeletal muscle. In the middlethird both smooth and skeletal muscle is presentand in the lower third only smooth muscle is pres-ent. The myenteric plexus of nerves and ganglia(Auerbachs plexus ) are found between the innerand outer layers of the muscularis externa. A tu-nica adventia is present.

    S TOMACH

    An abrupt transition occurs at the cardio-esoph-ageal junction , where strati ed squamous epi -thelium gives way to simple columnar epithelium.The simple columnar epithelium ( surface mucouscells ) dips into the lamina propria to form gastricpits (150-300 um deep). Gastric glands (simpletubular branched) empty into the bottom of thegastric pits. The base of gastric glands rests ona muscularis mucosa. The submucosa is quiteprominent and contains numerous arteries, veins,lymphatics and nerves. In the stomach the muscu-laris externa consists of three layers: a discontinu-ous inner oblique layer, then an inner circular layerand an outer longitudinal layer. When the stomachis empty the surface is thrown into folds ( rugae ).

    The stomach is divided into three histological re-gions (cardiac, body/fundus, pyloric) based on theiranatomical location and appearance of the glands.The cardiac region of the stomach is a narrow rimof tissue around the esophageal opening. Thecardiac glands are short tubular glands that are

    Chapter 14 Gastrointestinal tract 224coiled at the base. The glands consist mostly ofmucus secreting cells. Parietal cells may be foundin these glands. The fundus and body make upmore than 90% of the stomach and have the samehistological appearance. The glands of the bodyand fundus are straight tubular and have three re-gions: The upper third is the isthmus and emptiesinto the gastric pits , the middle third is the neck and the bottom third is the base . There are vetypes of cells associated with the glands. Regen-erative cells are found at the boundary betweenthe isthmus and the gastric pit. These cells are fewin number and not readily distinguished in routinepreparations. These cells divide and migrate up-wards to replenish the surface mucous cells anddownward to replenish the rest of the cells in thegastric glands. Mucous neck cells are found inthe isthmus and neck region. These cells are scat-tered among parietal cells and secrete an acidicform of mucus. Parietal cells are distinctive eo-sinophillic cells with a centrally located nucleus andsecrete hydrochloric acid. The eosinophilia is dueto the large quantity of mitochondria in these cells.Some parietal cells are also be found in the base ofthe gland. The primary cell type in the base is thechief cell which has a basophilic cytoplasm in itsbasal region. Chief cells secrete pepsinogen andgastric lipase. Gastric enteroendocrine cells arepart of the diffuse neuroendocrine system (DNES)are few in number and secrete enteric hormones(these can not be identi ed with H&E). The pylo -ric region has short coiled tubular glands that onlysecrete mucus chief cells and parietal cells areabsent.

    S MALL I NTESTINE

    One of the main functions of the small intestine isnutrient absorption. Specializations for increasingsurface area for absorption involve three magni-tudes of folds or projections.

    Circular transverse folds (1. plicae circu-lares or valves of Kerckring ) of the en-tire mucosa (with a core of submucosa)project permanently into the lumen. Theplicae are prominent in the duodenum and

    jejunum and diminish in the later part of theileum.

    Villi2. are projections (evaginations) of themucous membrane (with a core of laminapropria) into the lumen. The shape of villivaries in the different regions of the smallintestine: They start as tall, narrow, n -ger-like projections in the duodenum andevolve to a short broad leaf-like projectionin the distal ileum.

    Microvilli3. are cytoplasmic projections (1-2

    um in length) on the surface of the simplecolumnar epithelial cells. These surfaceprojections make up the striate border ofintestinal epithelium.

    The second main function of the small intestineis digestion and is dependent on secretions fromthree types of glands:

    Exocrine glands (liver and pancreas) de-1.liver their secretions (bile and digestive en-zymes) into the duodenum by way of thecystic duct and main pancreatic ducts .

    Submucosal glands2. . Submucosal glandsare only found in the duodenum (Brunnersglands). They secrete mucus and resem-ble the pyloric glands of the stomach.

    Intestinal crypts3. (glands) are invagina-tions of the surface epithelium down intothe underlying lamina propria.

    Cell types found in the intestinal epithelium in-clude:

    Simple columnar epithelium1. absorptivecells have a microvillus (striate) border andare involved in nutrient digestion and ab-sorption.

    Goblet cells2. secrete mucin.

    Columnar3. crypt cells transport secretoryIgA

    Paneth cells4. at the base of intestinal cryptsproduce antibacterial substances. Thesecells have very eosinophillic secretiongranules due to their content of lysozyme.

    M cells5. occur in regions where lymphoidnodules abut intestinal epithelium. Herethe columnar cells are replaced by thecuboidal to squamous M cells. M cells be-long to the mononuclear phagocytic sys-tem of macrophages and antigen present-ing cells.

    Stem cells are located in the base of the6.intestinal crypts

    Enteroendocrine cells7. (DNES) producehormones and are not readily distinguishedin routine preparations.

    The lamina propria forms the core of the villi andsupports the intestinal glands, is highly vascularand rich in lymphocytes and plasma cells. Themuscularis mucosa lies at the base of the glandsand sends bers into the core of the villi. The sub-

Meissners sub-mucosal nerve plexus is found in this layer andcontrols the muscularis mucosa. In the duodenumsubmucosal glands are found. The muscularisexterna , is responsible for peristalsis, and has aninner circular and outer longitudinal layer. Auer-bachs plexus of nerves is found between the twomuscle layers.

    The small intestine is divided into three regions:duodenum, jejunum and ileum. The pyloric stom-ach transitions to the duodenum at the pyloricsphincter (thick inner circular layer of the muscu-laris externa). The duodenum is the shortest seg-ment (25 cm) and receives secretions from the liver(bile) and pancreas (digestive enzymes). A distin-guishing feature of the duodenum is the presenceof submucosal glands (Brunners glands, theirappearance differs from pyloric glands only with re-spect to where they are located i.e. submucosal vs.mucosal). The jejunum and ileum have a similarappearance. Lymphoid tissue in the lamina propriaprogressively increases from the jejunum to the il-eum. In the ileum, permanent clusters of lymphoidnodules ( Peyers patches ) become a prominentfeature. Villi become shorter, broader and haveincreasingly larger lacteals (blind ending lymphoidvessels in the core of villi) in the ileum. Frequencyof goblet cells and Paneth cells increases as oneprogresses from the duodenum to the ileum.

    L ARGE I NTESTINE

    The main function of the large intestine is to re-absorb water and to consolidate and transport thefecal mass. The parts of the large intestine arethe cecum, appendix, colon, rectum and anal ca-nal. The cecum and colon are histologically indis-tinguishable. Having no villi, the inner surface issmooth and even. The intestinal glands (cryptsof Lieberkuhn ) are frequent and closely packedtogether. The glands are simple straight tubularglands and quite long (>600 um). The two ma-

    jor cell types are simple columnar absorptivecells with striated border and numerous goblet cells. Paneth cells may or may not be present.Enteroendocrine cells may be seen at the baseof the crypts. Lymphocytes are common in thelamina propria. The muscularis mucosa is found atthe base of the glands. The submucosa is well de-veloped with prominent blood and lymph vessels.Meissners submucosal nerve plexus is easilyseen in the colon. The muscularis externa con-sists of an inner circular layer and an unusual outerlongitudinal layer. The outer layer is gathered intothree distinct bundles ( taenia coli ) that are equallyspaced around the gut. Between the muscle layersthe numerous ganglia of Auerbachs plexus are

    seen. The colon is mostly covered by a serosa.The appendix is a 4-6 cm blind ending diverticu-lum descending from the cecum. Its epithelium issimilar to the colon, but with fewer goblet cells. Thecrypts are short (150-250 um) in comparison to thecolon. Enteroendocrine cells are found in the baseof the crypts. Numerous lymphocytes and nodulesare present in the lamina propria. When nodulesare present M cells are frequently observed in theepithelium overlying the nodules. The muscularismucosa is very thin. The muscularis externa is in-ner circular and outer longitudinal layers of smoothmuscle. The appendix is covered by a serosa.
