periodontal benefit for diabetics and pregnancy
provides 100% coverage for extra gum treatment,

the largest PPO network in Minnesota and surrounding
communities, plus a national PPO network of more than
120,000 providers, HealthPartners helps ensure your
employees keep that relationship strong and costs low. A
large PPO network means greater member discounts.
Little PartnersSM dental benefitA child’s overall health is a priority. Little Partners provides
100% coverage at network dentists for kids 12 and under.
All small group employer plans include Little Partners
and many large groups choose the benefit. With no
coinsurance, no deductibles and no annual maximum,
kids get the care they need.
Bundled convenienceWhen you bundle HealthPartners medical and dental,
you’ll save time and reduce overall costs. With one
account team, one member services number, one website,
reporting and more, you’ll simplify administration and
improve employee experience. Bundled pricing is available
for small group plans when a qualifying HealthPartners
small group medical plan is paired with dental.
Extra benefitsUnlike other plans, dental sealants are covered for all
age groups to help prevent cavities. Plus, for dental
implants, our plans provide coverage for both the surgical
placement and cost of prosthetics. Other plans typically
cover what a bridge or dentures would cost and apply that
amount to the cost of dental implants.
Healthy DiscountsSM ProgramMembers can save money on goods and services that
promote overall health. These include hearing aids,
orthodontics, vision, fitness equipment, retail discounts
and much more.
Online experienceEmployees get access to superior online tools to view
benefit details, check claim status, search claims history,
find a provider, view enrollment materials and chat with
Member Services.

MouthWise MattersA key part of lowering overall health care costs includes improving oral health.
HealthPartners MouthWise Matters is a platform for oral health education, offering tips
and insights to achieve better health outcomes. HealthPartners provides meaningful
dental benefits to keep your employees healthy and your costs down.
DiabetesThose living with diabetes are at an increased risk of gum
disease, which can harm blood sugar. HealthPartners
covers extra checkups, cleanings, root scaling and
planing 100% at in-network dentists, even if the annual
maximum is met. Disease and Case Management
services are also available. Other plans may only cover
extra checkups and cleanings.
PregnancyDental risk factors increase by more than 25% for pregnant
women. Cavities are common and gum disease can lead
to other harmful conditions. Pregnant women at risk of
gum disease get 100% coverage for extra, non-surgical
periodontal services to help keep mom and baby healthy.
Extra services include additional checkups, cleanings, root
planing and scaling.
What is root planing and scaling?Root planing and scaling remove bacteria and are the
clinically recommended way to manage gum disease.
Scaling removes tartar under the gum line and root
planing smooths rough spots on the tooth where
germs stick.

A narrow, high-value network solution featuring
HealthPartners Dental Group. It includes 25 clinics in the
Twin Cities metro area and St. Cloud.
Select
A narrow network with 300 providers featuring
HealthPartners Dental Group and Park Dental. Locations
include the Twin Cities metro area, St. Cloud, Rochester,
Duluth, Owatonna, Somerset and Hudson.
Open Access
This network features more than 2,900 PPO providers in
MN and bordering communities and more than 120,000
PPO providers from coast to coast.
Distinctions II (small group)
This is a tiered network solution with two benefit levels.
Benefit level 1 features the Select network of more than
300 providers and benefit level 2 includes the Open
Access network of 2,600 PPO providers in Minnesota
and bordering communities and more than 120,000
PPO nationwide.
Distinctions III (large group)
The Distinctions III network for large groups is a tiered
solution with three benefit levels. Benefit level 1 features
HealthPartners Dental Group clinics and benefit level 2
includes Park Dental clinics. Benefit level 3 is made up of
our Open Access network of 2,600 providers in Minnesota
and bordering communities and more than 120,000 PPO
providers nationwide.

Voluntary vs. contributory plans
A voluntary plan is when there’s no employer
contribution required. The employee pays the
total premium. A contributory plan is when
the employer pays 50 percent or more of
the premium.

benefit levels and includes more than 2,900 dentists
throughout Minnesota and bordering communities, and
85% of Twin Cities metro-area dentists.
Benefit level 1
Features two of Minnesota’s most respected dental groups
made up of more than 300 providers — HealthPartners
Dental Group and Park Dental Clinics.
Benefit level 2
Includes more than 2,600 dentists throughout Minnesota
and bordering communities and more than 120,000 PPO
providers nationwide.
The tiered network approach gives employees the option
to see a dentist within a narrower network with better
benefits (benefit level 1) or a dentist in a larger PPO
network (benefit level 2).
For small groups, there are six Distinctions products to
choose from with varying coinsurance amounts and
annual maximums up to $3,000.
There are also two Voluntary Distinctions plans available
that give employees great benefits even if there’s no
employer contribution. There are no waiting periods for
Voluntary Distinctions plans in benefit level 1.
Distinctions is a great solution for Twin Cities area companies, including St. Cloud.

Dental Options® Dental PlanDental Options is all about choice – for you and your employees. You choose a plan and
then your employees pick a network. It works like this:
1. Employer chooses a planAs the employer, you have two plan options to choose
from to offer your employees. Only the plan you
choose will be available to your employees during
open enrollment.
Plan options include:
Dental Options 1000
This plan has a $1,000 annual maximum
Dental Options 2000
This plan has a $2,000 annual maximum
2. Employees choose a networkOnce you’ve selected a plan, each employee will then
pick the network that best fits their needs.
There are three network options:
Dental Select
A narrow network, with the lowest premiums,
focused on providers that consistently offer the
best combination of total cost of care and quality
management. The Select Network includes
HealthPartners Dental Group and Park Dental clinics.
Open Access
Includes more than 2,900 PPO dentists throughout
Minnesota and bordering communities and more than
120,000 PPO providers nationwide.
Any dentist
Employees can visit any licensed dentist at any time at
the in-network benefit levels. Providers we contract
with in our Open Access network offer great discounts,
saving members money.

What’s an annual maximum?
The annual maximum is the amount the dental
plan will pay out for dental services before your
employee has to start covering the rest. With a
$1,000 annual max, any amount over $1,000 is
the employee’s responsibility.

Open Access Advantage(Employer-sponsored and voluntary)
Giving employees a choice is nice. And with the Open Access Advantage plan, employees can see
any dentist they want. What makes it different from our Open Access Dental plan is that the plan gives
employees an option to see an in-network dentist and lower their out-of-pocket costs. For example,
the in-network option will have a higher annual max and cover more of the cost of several services.
Open Access Advantage uses the Open Access network with 2,900 PPO providers in Minnesota and
bordering communities, plus 120,000 providers nationwide.

Covers 100 percent of exams, cleanings, sealants and X-rays at in-network dentists.
Preventive Plus
Includes the Little PartnersSM dental benefit and covers a percentage of Basic I services such as
fillings, simple extractions and endodontics; it doesn’t cover Basic II services such as oral surgery
and surgical periodontics.
