HealthInsight
Our mission is to serve as a primary agent in focusing community energy to achieve the
quality and effectiveness of health care.
History and Now
• HealthInsight is the QIN-QIO serving Nevada, New Mexico, Oregon and Utah
• We have been focused on reducing hospital-acquired conditions (HACs) and readmissions through this work.
• We served as the subcontractor to our local hospital associations to perform PfP activities on both HEN 1.0 and HEN 2.0.
• The HealthInsight team is committed to bringing about significant and lasting improvement in this region.
• Our strategy to achieve HIIN objectives is structured using multistate efficiencies, shared expertise and significant local presence.
• Teaming and collaboration with state hospital associations, a patient safety commission and regional health improvement collaboratives will be key drivers for aligning the HIIN work with existing initiatives throughout our service region.
Goal
The HealthInsight HIIN’s overall goal is to assist participating hospitals in realizing a 20 percent reduction in patient harm and a 12 percent reduction in 30-day readmissions during the first two years of the program.
