MIDDLETOWN — MiddletownMedical conducted its firstCommunity Seminar onHereditary Cancer Risk Testinglast month a full house in Chester, and the presentation was met withmuch success.
Thanks to Dr. Jennifer Reich, apioneer in the field, those in atten-dance were treated to a slew ofhugely important information ofthis topic.
“The response from this seminarhas been overwhelming, and we’reso pleased to provide such invalu-able education to the communi-ty,” said Lori Marl, OperationsDirector.
Hereditary Cancer Risk Testingoffers people with a family or per-sonal history of breast, ovarian, colon and uterine cancers peaceof mind, as mutations or varia-tions in genes passed from parentto child can drastically raise the
risk of cancer (in some cases 100percent) – and knowing if you’re atrisk allows you to take action toreduce this risk in advance.
In fact, up to 10 percent of can-cers are due to factors that arepassed from one generation to thenext.
Dr. Reich practices InternalMedicine in Middletown Medical’sMiddletown and Chester loca-tions, and has long been a favoriteof patients. In 2008 and 2010 to2013, she won the Patient’s ChoiceAward, along with theCompassionate Doctor’s Awardfrom 2010-13. She is a graduate ofthe University of Florida’s College
of Medicine, and is Board Certifiedby the American Board of InternalMedicine (ABIM).
Middletown Medical is a multi-specialty physi-cians’ group dedicated to providing patientswith top quality, comprehensive care in a warmand caring environment. Opened in 1984,Middletown Medical now includes nine loca-tions throughout Orange and Sullivan counties(Middletown, Bloomingburg, Chester, Ellenville,Liberty, Monticello, Port Jervis, Warwick andWurtsboro), including Immediate Medical Care(Urgent Care walk-in services) at theMiddletown, Chester and Ellenville locations.Open seven days a week with no appointmentnecessary, please visitwww.MiddletownMedical.com for more informa-tion, or call 342-4774.

Dr. Spina follows father’s chiropractic footstepsSTORY AND PHOTOS
BY MARGARET BRUETSCH
LIBERTY — Dr. Mark Spina, a chi-ropractor of 30 years, shares thesame office building that his fatheralso used.
Located at 21 Mill Street, Liberty, itis now home to Advanced Health &Medical and offers other servicesincluding pain relief and rehabilita-tion, evaluations, physical therapy, chiropractic care and nerve testing.
Advanced is made up of varioushealth care professionals includingMedical Director Dr. Avi Davachi,MD, Dr. Orsuville Cabatu, MD(physiatrist), Michelle Guanzon(physical therapist) and Dr. Spina.
Call 292-8810 or fax 295-9156.Dr. Spina recently sat down with
the Democrat for a Q&A.Q: Where did you go to school?A: I went to school at Life
Chiropractic College in Atlanta,Georgia.
Q: Why did you choose to be achiropractor?
A: My dad is a chiropractor and I
saw that he helped a lot of people. And I grew up with the viewpoint ofthe body should be healthy and nomedications and things like that. SoI sort of wanted to follow in his foot-steps.
Q: What does it offer that regularmedicine might not?
A: It’s like a hands-on approach towork on balancing the nerve sys-tem, and that nerve system controlsthe different parts of the body.
The nerve system is like the com-munication system of the body, andthe nerves are the wires that com-municate and do the communica-tion.
And what chiropractors do is try torealign the spine so that that propercommunication can take place anda body will normally heal and func-tion properly.
Q: Do you feel chiropractic hasbeen accepted within wider med-ical community?
A: For the most part yes.… it has alittle bit to go but it’s been more andmore accepted.
Dr. Mark Spina became a chiropractor hoping to follow in his father’s footsteps and hasbeen doing so for 30 years.

Crystal Run Healthcare Community Seminar:
‘Getting Tomorrow’s Healthcare Today’ BY ANYA TIKKA
ROCK HILL — If there was onething to take away from the CrystalRun Community Health Seminar atRock Hill earlier this year, it was thathealth care is changing, like it or not.
Managing Partner and CEO HalTeitelbaum, MD, JD, MBA, gave a slidepresentation to explain the changes tohealth care in general, what theAffordable Care Act means in practiceand what Crystal Run has to offer.
Joining Dr. Teitelbaum was panel ofhealthcare experts from Crystal Runincluding Chief Operating Officer Dr.Michelle A. Koury, Chief Medical andChief Medical Information Officer Dr.Gregory Spencer, as well as Co-ChiefClinical Transformation Officers Dr.Jonathan Nasser and Dr. Scott Hines..
Dr. Teitelbaum briefly described thehistory of the Affordable Care Act,popularly known as Obamacare, whose aim, he said, is to increase
quality while decreasing cost. “The country is broke. The quality
of health care is not good. You canhate or like Obamacare – but healthcare has to change,” he said, and
cited alarming statistics about thestate of health care in US.
“Life expectancy is below Australia,Japan, and Great Britain. We are num-ber 50 in the world in infant mortality, nine spots behind places like Cuba. This is because of problems withaccess to health care, although wehave the smartest people in the world. The first step of providing qualityhealth care is providing access,” hesaid. “We need to focus on individualsand prevention, and we need to stopthese unsustainable cost increases.”
One of the first things to do, is to askif the operation, procedure, screening,or x-ray is necessary, and if it’s going to“increase quality and length of life, it’stotally worth it. But if not, it’s a totalwaste," Dr. Teitelbaum explained, adding, “Frankly, when we do proce-dures, we put people at risk. We needto ask, if we don’t fix it, what wouldhappen? If the answer in your specificquestion is ‘nothing,’ why should we
do it? Why should we put you at risk?”He noted, “We want to transform
from ‘How many visits did you haveto the doctor’ to ‘How can we besttake care of you.’ In some cases, maybe the best way of taking care ofyou is touching base with you byphone in three weeks or threemonths, rather than dragging you tothe office. Maybe you can monitoryour own blood pressure with ourhelp and a device at home. We haveto focus on what is going to get youbetter rather than what is going tomake us money. Yes, we have to stayin business, but we have to make thisin a way that makes sense to all of us.”
Crystal Run is one of the firstnationally accredited AccountableCare Organizations, whose innova-tive concept is that physicians shouldbe responsible and accountable forthe quality, cost, and the overalldeliver of care to the patients. It’s alsoimportant to get the patients
Chief Operating Officer Dr. Michelle A. Kourytalked about the benefits of the AffordableCare Act (Obamacare) at the seminar.

involved in the process. In response to a question from 73-
year-old cancer patient Jean Langefrom Damascus, PA, about the longterm availability of both care anddrugs under Obamacare, Teitelbaumsaid, “Yes, government has cut provi-sion for Medicare from the budget, about 700 billion dollars over the next10 years. Do we like it? No. But neitherDemocrats nor Republicans want tofund Medicare, the bill is too high.”
Obamacare’s basic approach is notrationing patients’ healthcare, butrationing payments to providers.
“I believe the government is doing itsbest to fund patients and taking fund-ing away from providers, which is pre-cisely why hospitals are upset, becausethey are going to be paid less,” Dr. Teit-elbaum said. “Recently, the Obamaadministration was making calls toreduce the availability of certain drugsunder Medicare. The truth of the mat-ter is, some brand name drugs likeNexium are a rip-off. They are chemi-cally identical with much less expen-sive drugs. The question to ask is, do wewant the manufacturer to get millionsor billions of dollars, or do we want toput that money into Medicare, cancer
patients, patients with heart disease?”Regarding Obamacare, Dr. Koury
had this to say: “The country’s econo-my has collapsed, and the cost ofhealth care is one of the contributingfactors. One of the ways to help theeconomy to recover is to better allo-cate our resources in health care.”
She cited that despite all the moneyspent in US, the country is behind incritical health care as shown in thepresentation compared to othercountries, so the high percentage ofGDP devoted to health care costs can-not be justified.
She continued, “It is my educatedopinion that in 25 to 30 years, mygrandkids are going to be learning intheir history class, hopefully in publicschools, that March 23, 2010 is a land-mark in US history, because… [of thepassage of] the Affordable Care Act.”
Dr. Teitelbaum talked about themuch-criticized websites and theirtroubled introductions.
“New York State has its own websiteNewyorkstatehealth.gov. I wasappointed by Department of Health tobe on the commission for the

Access Physical Therapy & Wellness voted‘best overall business’ in Tri-State Area
Doctor of Physical Therapy Innocent Rwakonda, left, and his wife, Munya Rwakonda, accept the 2014Bizzy Award for Best Overall Business on behalf of Access Physical Therapy & Wellness, from ChamberPresident Robie Mackerly, right, at the Chamber’s 24th Annual Spring Banquet at the Best WesternInn at Hunt’s Landing in Westfall Township, PA. Bizzy Award winners were nominated by Chambermembers and selected by public vote. Chamber Executive Director Charlene Trotter also thankedRwakonda for the company’s sponsorship of the dinner. Based in Campbell Hall, Access(accessptw.com) is a family-owned company with 19 offices in four states, three in the Tri-State Area.In Sullivan County, Access is at 39 Old Monticello Road, Ferndale. Call 295-0101. Access PhysicalTherapy & Wellness is open weekdays 7 a.m.–8 p.m., Saturdays 8 a.m– noon. They accept most insur-ances – and new patients can almost always be seen within 24 hours. changes, but I take no credit that itrolled out well, went almost flawless-ly,” he noted. “Basically, the states thatrefused to set up their own exchanges, mostly with Republican governors, had a mess. New York State did anamazing job. If you live in New York, and don’t have insurance, you shouldcheck out the exchange. In manycases you can get a subsidy on yourpremium, and cost share when you goto the doctor’s. Cost has gone down inNew York. Why? Because it alreadyhad certain basic good quality healthinsurance.”
Because Obamacare requires bettercare, in some states where you hadinsurance that virtually paid for noth-ing when you got sick, the premiumsdid go up, but as a result you actually
have real insurance, Dr. Teitelbaumexplained.
Dr. Nasser elaborated, “Preventionis very important. For the people withthe most serious conditions, half ofthem reported not seeing a doctor inyears, so there’s an opportunity to getcaught up now.”
Chances for cure are much better inmany conditions if they are caughtearly.
Under Obamacare, you cannot beassigned to a different group by anunderlying condition in terms of risk.If you’re dying of cancer, you can stillbuy health insurance, Dr. Teitelbaumnoted.
“If you are a patient, this is amazing, good stuff. We’re spreading the riskamong large population, resulting inlower rates for patients,” he said.

The name says it all:Body, Mind & SpiritSTORY AND PHOTOS
BY MARGARET BRUETSCH

The Democrat recently sat downwith Dr. Noeth for a Q&A.
Q: Where did you go to school?A: Actually, I’m a Monticello High
School graduate, [graduating] in1984. From there I went to AdelphiUniversity and I got my Bachelor’s intheir biology/pre-med program… in1988. And then from there I went toLife Chiropractic College in Atlanta,Georgia, and I graduated from therein 1992.
Q: Why did you choose to be a chi-ropractor?
A: Well, when I was a kid my momhad asthma, and she was in the hos-pital, on ventilators, she was on allkinds of asthma medication. And mydad was like “That’s it, I’m taking youto this chiropractor.” Within threemonths she was totally off the med-ication, asthma-free and a new per-son, so we started going for familymaintenance care.
Q: What does chiropractic offerthat regular medicine might not?
A: It’s more of a natural, drug-free,holistic approach. We try to findimbalances in the body before they
are full-blown diseases where theywould need to have surgery or med-ication. And our basic thing is wework with the spinal column and thenervous system, in that the nervoussystem controls every other systemin your body. So sometimes thingswill get out of alignment in your bodyand that will cause your nervous sys-tem to malfunction and if left therefor a period of time your body startsto malfunction. So it’s not alwayspain. Most people think if you have aback pain you go to the chiropractor. But only 10 percent of the nerves areactually pain nerves, so if you have apain nerve that’s being pinched you’llhave pain. The other 90 percent ofthe nerves in your body control howyour body functions. So if it’s press-ing on a function nerve, you wouldn’tnecessarily have pain but your body’snot going to function properly.
There are different types of chiro-practors, I’m a holistic chiropractor. In that we use other natural thingslike nutrition, herbs, homeopathyand acupressure and massage. Otherthings in addition to the chiropracticcare to get your body back into bal-

ance and functioning at its optimum.Q: Do you feel it’s been accepted
within the wider medical communi-ty?
A: As time goes on… natural health-care and alternative healthcare isactually surpassing traditional med-ical care in people’s first choice as faras dealing with something that hasgone awry in their body. More peopleare seeking natural cures, peopledon’t want to just take a drug to basi-cally hide or suppress their symp-toms. They want to get to the root ofthe problem. “Why isn’t my bodyworking? What’s wrong with me?” Somore and more people are choosingalternative health care and doctorsare actually being forced to recognizeit because it’s not going away. Andmore and more people are choosingalternative health care. They startedcovering chiropractic care throughinsurance companies in the ’80s, sothat’s 30 years ago. So it is being moreaccepted as far as being complemen-tary, not necessarily you have tochoose one or the other but a lot oftimes it’s good to have something thatcomplements each other.
