We’re part of your health care team

Most of us visit our family doctor to take care of our day-to-day health care needs. And we use the services of our local hospital when we need emergency, medical or surgical care. But where do you turn when you need help with your health care needs at home or at school?

That’s where your local Mississauga Halton Community Care Access Centre (CCAC) can help.

Like your family doctor and local hospital, the Mississauga Halton CCAC is an important part of your health care team. Think of us as your personal health advisor for connecting you with in-home, community-based services and/or long-term care options.

We coordinate professional health care services for people of all ages, from birth through to end-of-life, with a particular focus on helping older adults, who are at high risk, get the help they need to live safely at home for as long as possible. Our services are funded through the Mississauga Halton Local Health Integration Network (MH LHIN), so there is no direct cost to you.

To help you get the care you need when and where you need it, our highly qualified case managers:

• conduct detailed health assessments of your care needs to determine whether you are eligible for services

• coordinate professional health care services in your home, at school, or at community-based clinics. These services might include nursing care, occupational therapy, physiotherapy, personal support, speech and language pathology, social work or nutritional counselling (these service costs are funded by the MH LHIN)

• provide information about community-based services that can help you with preparing meals, housecleaning and transportation to appointments, (some services are provided for a fee)

• provide referrals to community-based services, such as adult day programs and provide information about bathing services and other supports from outstanding local community-based agencies

• coordinate the application process for admission to long-term care facilities

• provide end-of-life care and coordination

When it comes to your health, life can often throw you a curve ball when you least expect it. Whether you are having surgery in the hospital, have a child who is struggling at school due to a significant health issue, are caring for a loved one with dementia, feel you can no longer manage living on your own at home or you have been diagnosed with a life-limiting illness, you may find yourself wondering how you are going to get through this.

We’re here to help.

Our case managers are your personal health care coordinators for setting up health care services in your home, at school and in the community:• nursing • occupational therapy• physiotherapy• personal support • speech language therapy• social work• dietetic services

We are your personal health care advisors for long-term care and other care options: • exploring options for long-term care• helping with the long-term care application process• providing relief for family caregivers

How to access services

Contact the Mississauga Halton CCAC. You can call about yourself or on behalf of a family member, friend or neighbour who needs support.

Phone 310-CCAC (2222). No area code needed.8:30 am – 9:00 pm, seven days a week

The most important first step you can take if you think you need help is to contact us. The friendly team in our Contact Centre will ask you a few questions to better understand how we can help you.

There may be one of two ways we can meet your needs:

1. Confirming if you qualify for CCAC health care and support services If it sounds like you might benefit from professional health care services coordinated through the Mississauga Halton CCAC, our Contact Centre staff will connect you with one of our case managers. To better assess your needs, our case manager will ask you more questions over the phone. This assessment may take 20 to 40 minutes depending on the reason for your call.

a) Urgent needs Although rare, there are times when it is clear following the phone assessment that someone is in urgent need of CCAC services. If this is the case, we will arrange for professional health care services to start immediately until we can perform a more in-depth assessment in the home.

b) Non-urgent needs When you call our contact centre, our team assistants will ask you a few questions and, based on your response, they may schedule the phone assessment with you for a later date. After this initial phone assessment, a case manager may visit you at your home within 14 days to conduct a more detailed health care assessment.

2. Providing you with information about other community services We often receive phone calls from people who don’t require health care services, but who need help to enable them to continue living safely and independently in their own homes. This might include help doing laundry, housecleaning, home maintenance or preparing meals. Or it may include help with transportation to health care appointments or connecting with a support group. Our Contact Centre team references a list of valuable community-based services and will be happy to provide you with the names and phone numbers of agencies that might be able to help you. Please note that some of these organizations may charge a fee.

Arranging for CCAC services when you are in the hospitalIf you are in the hospital, your health care team may ask one of our Mississauga Halton CCAC hospital case managers to meet with you to talk about what supports you might need in place once you return home. Or you can ask your hospital nurse or doctor to contact our hospital-based case manager to arrange for a meeting with you before you are discharged. Or call us directly at 310-CCAC (2222). You will want to talk to our case manager if you need:

• short-term support following surgery (e.g. wound care for an incision, rehabilitation following hip or knee surgery)

• long-term support to help you to continue living safely and independently at home (e.g. nursing care, occupational therapy, physiotherapy and personal support)

• end-of-life care at home (e.g. pain and symptom management, advanced care planning)

• transitional care (e.g. convalescent care) to regain your strength before returning home

• help applying for admission to long-term care

Our services focus on individuals who have high needs and who are at risk of losing their independence. We have hospital case managers available onsite at The Credit Valley Hospital and Trillium Health Centre, Georgetown Hospital, Milton District Hospital and Oakville Trafalgar Memorial Hospital.

Connecting you with the right services

So what are the next steps after you place that call to the Mississauga Halton CCAC? If the questions posed by our Contact Centre team assistant indicate you or your loved one may qualify for CCAC services, we will send one of our highly qualified case managers to your home, usually within 14 days to meet with you.

Your personal health care advisorThink of our case manager as your personal health care advisor. He or she will be your main point of contact with the CCAC and will work with you and/or your loved one to identify which services will best meet your individual short or long-term health needs. When our case manager visits you in your home, he or she will:

• ask you questions about your health care needs and how you are currently managing (health care assessment) while being sensitive to diversity and culture

• talk to you about who is currently helping to support you

• walk around your home to assess your home for safety

• speak with your family caregiver to find out how they are doing and to see if they have any needs as well

Your case manager will use this assessment and experienced judgment to confirm if you require Mississauga Halton CCAC services and to coordinate the right mix and level of services based on your needs. While you receive CCAC services, your case manager will check in with you regularly to see if your needs have changed. Your case manager will also refer you to other community-based services that may further support your needs like meal delivery, housecleaning or friendly visiting.

If you don’t require Mississauga Halton CCAC services at this time and other organizations are more appropriate to meet your needs your case manager will recommend other valuable community supports that may be able to help you.

Your health care assessmentAll CCACs in the province use a common set of provincial assessment tools to determine health care needs. Based on your assessment, your case manager will either develop your care plan or refer you to community service organizations for services. Generally, most of the services we provide are best suited for people who are considered at high risk of losing their independence and ability to continue living at home.

Your personalized care planIf you qualify for Mississauga Halton CCAC health care services, your case manager will develop a personalized care plan, which will include a schedule outlining which health care professionals will be coming into your home, their contact information, how often they will be visiting and on what days and times. Your case manager will take the worry out of scheduling these visits by coordinating them on your behalf. All health care services coordinated by the Mississauga Halton CCAC are provided by contracted service providers that share the same values and commitment to high quality client-focused care that we do.

Finding a family doctor with Health Care Connect:In 2011/12, we collaborated with local family doctors and helped 1,702 people find a family doctor. With just 770 citizens requesting a family doctor, our region has one of the lowest rates of residents without a primary care physician in Ontario.

Specialized Services

Children’s health services

In-home health care servicesWe support families in the care of their infant, child or youth with complex medical and health care needs until their child is 18 years of age or 21 years of age if their child is attending school and accessing School Health Support Services.

Our highly qualified case managers contact the family within 72 hours of admission and works with families to support the development of their child. The family maintains the role as the primary caregiver. Within 14 days, our case manager will complete an assessment and develop a personalized care plan for the child. In some cases, your child’s health care needs may be met by attending one of our community nursing clinics.

Speech and language pathology services for pre-school children who qualify are available through ErinoakKids (for children up to the age of five) and through our School Health Support Services program, starting in Grade One.

School Health Support ServicesYou want your child to have every opportunity to attend, learn and take part in activities at school. But this can sometimes be challenging for children who have physical or other health care needs.

We work closely with the Public, Catholic and French school boards in your community, and with private schools, to identify and support school-aged children who are struggling in the classroom due to a significant physical or health issue. For example:

• a medical diagnosis can hamper a child’s ability to attend school unless the child has support with things like gastric tube feedings, catheterization, insulin injections or suctioning and monitoring

• fine motor delays can make it hard for a child to grasp a pencil or pen affecting their ability to print or write

• oral motor problems can affect a child’s ability to communicate with the teacher and classmates

• an awkward gait or frequent falls can make it difficult for a child to join classmates in the school’s physical education program

We work with school staff, parents and regulated health care professionals to transfer learning to parents and school teams to ensure support is integrated into the school environment. Please note that these services are only available during the school year and are not available in the summer months.

Contact your child’s school principal or special education resource teacher about making a referral to the Mississauga Halton CCAC for your son or daughter.

What services are provided?No two children have the same health needs. Based on an individual assessment of your child’s needs, our case managers will work with you and your child’s teacher to coordinate health support services in the classroom which may include one or more of the following:

• nursing• personal support• physiotherapy• speech and language pathology • dietetic services

How do I know if my child is eligible for services?To be eligible for School Health Support Services, your child must:

• be enrolled in a publicly-funded, private or home-school, up to the age of 21 years

• be identified by the school as needing School Health Support Services programs to attend, learn and/or take part in activities at school

• have a valid Ontario health card

If you aren’t familiar with School Health Support Services and think it might benefit your child, contact the school principal or your school’s special education resource teacher about making a referral to the Mississauga Halton CCAC for your son or daughter. More than 3,000 children currently receive support through this program

Did You Know?The Mississauga Halton CCAC conducts assessments on behalf of the Ontario Ministry of Children and Youth Services to determine if a child’s family is eligible for enhanced respite funding. This grant is paid to families who are caring for a child who is medically fragile and/or depends on a technological device and who needs care 24 hours a day, 365 days a year. Families may be eligible for up to $3,500 per child per year. The children of eligible families are re-assessed every year.

Specialized Programs

Programs helping you move from hospital to home safelyWhether you have been in the hospital receiving medical care following an illness or injury, or you are recovering from surgery, sometimes you may need care to safely manage on your own at home.

Based on a referral from your hospital health care team, one of our CCAC hospital case managers will meet with you to assess your needs. If you require significant support to

return home and you are over 16 years of age, you may benefit from our Wait at Home or Wait at Home Enhanced programs. Those programs provide more support than our traditional programs and enable you to either return home or return home and wait for long-term care.

Convalescent Care programThe Convalescent Care program provides 24-hour care to people who require specific medical and therapeutic services in a supportive environment. The program will help you recover your strength, endurance and functioning before returning home following a stay in hospital. This program supports clients able to benefit from two hours of rehabilitation daily. This program operates out of Post Inn Village.

The average length of stay is four to six months. There is no fee for this program.

Restore ProgramThis short-term care program was designed to help you return to independent living with the support of a specialized team of health care professionals who will help you to regain your physical strength. If you qualify for the program, you will move from the hospital to a semi-private room in the Restore program at the Cooksville Care Centre in Mississauga. The average length of stay in this program is four to six weeks.

Home First

Alternatives to long-term careWorking together with our hospital and long-term care facility partners, we believe it is important to exhaust every available option to help someone who has high needs to remain living safely at home instead of moving into long-term care. This Home First philosophy of care is supported by a number of programs designed, highlighted below.

Wait at Home programWe help you move from hospital to home safely, where you can take the time you need to apply for long-term care and wait in the comfort and safety of your own home. One of our highly qualified case managers will assess your needs and assist in completing your long-term care application.

Wait at Home Enhanced programWe provide an enhanced level of in-home health care services to enable clients to move safely from hospital to home while they consider whether a long-term care facility is the best choice for them.

Stay at Home programWe provide enhanced care for people who recognize that they can manage their care needs at home instead of having to move into a long-term care facility. In fact, in 2011/12, 98.2 per cent of our clients in our Stay at Home program remained safely at home and out of long-term care facilities.

Short-Stay programWe help you recover from your hospital surgical and medical treatments at home. Our hospital-based case managers will develop your personalized care plan. You do not need to wait.

Client case study

Alan’s Story: Wait at Home Enhanced program a life-line for Alan and Shelley

Imagine walking into your bathroom at home and blacking out, only to wake up in hospital several days later, partially paralyzed and unable to speak. How do you begin the long road to recovery? Will you ever be able to return home?

This is exactly what happened to Alan. At just 65 years of age, a massive stroke left him completely paralyzed on his left side and unable to speak. Alan’s wife, Shelley, became his primary caregiver, but she is physically much smaller than Alan and required considerable assistance to meet his care needs.

To help with Alan’s continuing recovery and to ensure he could remain safely at home for as long as possible, Helen Bell, our case manager and a registered nurse with more than 20 years of experience, became Alan’s personal health care advisor. Helen assembled his personal care team, including personal support workers (PSW), a social worker, an occupational therapist (OT), and a physiotherapist (PT).

Alan’s physiotherapist designed an exercise program to maximize his physical recovery and abilities. The occupational therapist worked with Alan to determine his equipment needs to enable mobility and ensure his home was safe and easy to move around. Personal support workers visited three hours a day, seven days a week, to help with bathing, toileting, dressing, and other activities of daily living.

It is a long, ongoing road to recovery for Alan, but with the personal guidance, support and care arrangements from Helen, he is starting to regain his quality of life. Recently Alan produced a digital slide show of photographs from vacations he and his wife took over the years. This is an amazing accomplishment considering the immense challenges he has faced to regain functionality and independence since the stroke.

Traditionally, someone in Alan’s situation would have had only one option – to enter a long-term care facility. However, because of the care and support provided by the Mississauga Halton CCAC, Alan and his family view long-term care as a last resort.

Client case study

Health Care at Home with Stay at Home: A life-line for clients

Our client Eva is 86 years old with Alzheimer’s dementia. She was referred to the Mississauga Halton CCAC from hospital, after she fell at home and fractured her hand.

During the first visit to Eva’s home, Natoya Hylton, her case manager, saw that Eva was unable to manage her own personal care or manage daily activities of life. Yet Eva’s son and daughter were adamant that their mother not move to a long-term care facility.

Natoya increased Eva’s personal support and ordered occupational therapy and physiotherapy to help Eva regain her strength and avoid another fall. Natoya also arranged equipment for her mobility and bathroom safety.

Monitoring Eva closely, Natoya found that Eva was becoming increasingly frail. When

the personal support worker prepared breakfast for her, she came back in the evening to find Eva hadn’t eaten and was sitting in the dark. When Eva drank from a cup with dish soap, thinking it was juice, she was taken to the emergency department. That is when Natoya admitted Eva into our Stay at Home program so she can remain at home safely.

Eva now receives personal support worker assistance three times a day, to help with her personal care and prepare her meals, ensuring Eva’s health and nutritional needs are met.

It’s been a year since Eva was placed in the Stay at Home program and she has never returned to hospital. Eva wanted to stay at home and with our services, she can live safely in the comfort of her own home.

End of Life Services

End-of-life health and support servicesComing to terms with the fact that you or a loved one has a life-limiting illness can be difficult. Talking about your illness with your family and sharing with them how you would like to live out the final months or weeks of your life can be even harder. But it is important these conversations start early, so that you can experience the best possible quality of life and so that your family can fulfill your wishes in the final days of life if you are no longer able to speak for yourself.

Supporting you and your family in the final 12 months of lifeThe Mississauga Halton CCAC provides enhanced palliative care services to adults in the last 12 months of life. Our palliative case managers are experts who help you live as pain and symptom-free as possible and to support your wish to die in your place of choice, whether that is at home, or in a hospice.

If you qualify for services, we will:

• arrange for you to meet with one of our palliative care case managers who will work with you and your family to identify the professional health care services that will best meet your needs;

• link you to a nurse who specializes in end-of-life care and who can help you and your family with advanced care planning and with accessing the many palliative care resources available in your community.

Your palliative case manager will remain a constant throughout your final months of life regardless of whether you move between settings (from hospital to home, a son’s or daughter’s home, to hospice) and will always be available to answer questions and support you through this difficult time.

The role of the palliative case managerYour palliative care case manager will talk to you and your family about the importance of advanced care planning including:

• your goals for care• assigning a substitute decision maker and Power of Attorney • discussing where you want to die

Your case manager and nurse will work together to help your family access government assistance programs (e.g. Employment Insurance (EI), family caregiver leave for palliative care) and access respite care for caregiver relief; coordinate the involvement of palliative care volunteers and make sure every member of your health care team (family doctor, social worker, hospice volunteers, pharmacist, occupational therapist, physiotherapist, personal support worker) is communicating with one another about your care.

Did you know?• Palliative care is also available for children through our

Children’s Services team.• The Mississauga Halton CCAC has three case managers who

are palliative advanced practice nurses. They assist, teach and mentor the members of a client’s health care team to broaden their understanding of palliative care practices and pain and symptom management.

This letter is going to sound like a tribute at The Oscar’s, but in the scheme of life, you and your team at the Mississauga Halton CCAC, if awards were available, would win the Oscar.

I want to thank you on behalf of mother, and my entire family, for the kindness, patience, empathy, service, equipment, support, humour, professionalism, and TLC that you offered, not only to Mother, but to my sister and me, in Mother’s care.

Our sincere thanks to the individual and unique contributions of nurses and friends to the care of Mother.

And you, Marlene, who arranged, educated, supported, advised, for the past two years. Without your management of Mother as a client Marlene, I know that we would not have had the attention and kindness that we received. Your ever-present humour and optimism made possible the comfortable end of life that we all wanted for Mother.

When I was worried and unsure, you were also cheerily there for me, with the explanations and information that I needed to be strong for Mother.

The Mississauga Halton CCAC and your team were life-lines, angels, and provided the holistic care that not only the client/patient needed, but supported the family. I learned so much about palliative care and what is available in the community. Mississauga Halton CCAC should be proud to have you on their team, and proud of their elder-care services.

Long-term care facilities

Applying for long-term careWhen it is no longer possible for you to remain living at home through a combination of in-home and community support services, one of our case managers will become your personal health care coordinator to help you and your family apply to live in a long-term care facility.

Your Mississauga Halton CCAC case manager will:

• conduct a health assessment with you to make sure long-term care is the most appropriate place to meet your current care needs

• talk to you about government-funded long-term care facility options so that you can make an informed decision about where it is you would most like to receive your ongoing care

• walk you through the application process• provide you with a checklist that you can

use to tour long-term care homes in your area;• ask you to identify up to five long-term care homes on

your application• help you complete the necessary paperwork

Once you apply for long-term care, you must be ready for an offer from any of the five choices on your list at any given time.

Tips for family caregivers If you are caring for a loved one with dementia, here are some triggers that might help you decide when it’s time to pick up the phone and call the Mississauga Halton CCAC for support.

• Do you have a plan in place in case your loved one starts wandering (e.g. door alarms)?

• Has your loved one been physically or verbally aggressive towards you?

• Do you have someone to talk to who understands what you are going through?

• Are you aware that there are support groups for caregivers?• Have you ever been so frustrated with your loved one that

you were afraid of what you might do?• How and when do you get a break from caregiving?

Choosing a long-term care facilityIt’s not always easy to know what questions to ask and what to look for when choosing a long-term care home. The following are just a few tips we share with clients when we meet to discuss applying to long-term care. For a more detailed checklist and to take a virtual tour of the 27 long-term care facilities in our region, visit www.healthcareathome.ca.

• What are the accommodation fee co-payments?• What services are included in the fees (e.g. personal

supplies)?• What is the long-term care facility’s policy on restraints and

medication?• Can my loved one continue receiving care from his or her

family doctor after admission to the long-term care facility?• Is the staff doctor a geriatrician?• Is a doctor on call 24 hours a day?• Is the staff doctor willing to take time to talk to family

members regarding medical care for my loved one?• What is the ratio of nurses to residents?• Are special needs and preferences recognized by the long-

term care facility?• If English is not my loved one’s first language, will an

interpreter be available when needed?• Can married couples room together?• How many residents live in the long-term care facility?• Are there regular fire drills for all staff (including part-time)?

Relief for family caregivers

Relief for family caregiversWhen you care for a loved one with significant physical challenges and/or health issues in your home, we appreciate the time, energy and commitment it takes to meet their round-the-clock needs. No matter how committed you are, everyone who takes on this role needs to take a break. A lack of

sleep, feeling overwhelmed or impatient and a quick temper are all signs that perhaps it’s time to recharge your batteries.

The Mississauga Halton CCAC can help connect you with the services in your community that can give you that much-needed time out.

Respite services for caregiversWhen you need an extended break from your caregiving responsibilities, you may be able to place your loved one temporarily in a long-term care facility.

A select number of long-term care facilities offer a short respite program to give caregivers some much-needed relief. The maximum length of stay is 60 consecutive days and no more than a total of 90 days per year (starting from the day of admission). Space is limited and bookings can often be made up to a year in advance. A standard, provincial government-set fee is charged for this service.

Adult day programs Adult day programs can provide family caregivers with a break during the day by offering your loved one:• a place where they can participate in a regular schedule of social

and recreational activities• an opportunity to develop friendships and participate in groups with

individuals of similar abilities and challenges• an environment where they are nurtured, stimulated and can

have fun

For a list of adult day programs in your community, contact the Mississauga Halton CCAC. We can help identify the day programs that would be most suitable for your loved one. There is a fee for this program.
